Chronic kidney disease (CKD) is a leading condition in terms of prevalence and overall health impact. With the increased life expectancy of the CKD population and the improvement in medical care, controlling musculoskeletal complications remains a tough challenge. Patients with CKD are prone to falls, fractures and sarcopenia, enhancing the risk of death. A multitude of mechanisms contribute to fractures, and treatment is suboptimal; therefore, prevention must stand out as a key step. This review aims to provide an overview of the most relevant data regarding the impact of nutrition on bone di

Hypercalcemia, a common electrolyte imbalance, requires accurate differential diagnosis to guide appropriate management. PTH-dependent hypercalcemia, predominantly caused by primary hyperparathyroidism (PHPT) and rarely by familial hypocalciuric hypercalcemia (FHH)-mainly due to heterozygous loss-of-function mutations in the CASR gene encoding the calcium-sensing receptor (CaSR)-now includes acquired hypocalciuric hypercalcemia (AHH) as an emerging disease entity. Initially identified as analogous to FHH, AHH was characterized by blocking antibodies targeting the CaSR. However, our research ha

Nephrogenic diabetes insipidus (NDI) is a rare disorder characterized by the inability of the kidneys to respond to arginine vasopressin, leading to polyuria, polydipsia, and chronic dehydration. This condition often results in severe complications, including growth failure, behavioral problems, and irreversible brain damage, particularly in young children with hereditary forms. Early diagnosis and effective management are critical for minimizing these complications and improving patients' quality of life. Current treatments focus primarily on symptom management through dietary interventions,

AIM: The aim of this study was to determine whether water or solute transport plays a critical role in AQP3-mediated urine concentrating ability, using AQP3 knockout (AQP3-KO) mice and a novel mouse model in which the AQP3 gene coding region is replaced with AQP4 cDNA (AQP4-KI).
METHODS: AQP3-KO and AQP4-KI mice were characterized using Western blot and immunofluorescence to confirm the absence of AQP3 and the in situ replacement of AQP4. Urinary output, osmolality and urea concentration were measured in mouse models under various conditions, including water deprivation, acute urea loading an

Volatile anesthetics, while increasingly utilized in intensive care medicine, are associated with significant renal adverse effects. A critical safety concern-particularly with sevoflurane-involves its potential impact on renal function. Pathophysiologically, inorganic fluoride levels exceeding 50 µmol/L are recognized as a threshold for nephrogenic diabetes insipidus, a condition generally considered reversible. Additionally, the sevoflurane degradation product "compound A" has been implicated in direct renal tubular and glomerular toxicity. Specifically, exposure has been correlated with glo

Differentiating central diabetes insipidus (CDI), nephrogenic diabetes insipidus (NDI), and primary polydipsia (PP) in pediatric patients with polyuria-polydipsia syndrome (PPS) remains a clinical challenge. The water deprivation test (WDT) is the traditional gold standard; however, it is time-consuming, burdensome, and prone to equivocal results. Stimulated copeptin, a surrogate marker of vasopressin, has emerged as a promising diagnostic alternative. We conducted a prospective, observational, cross-sectional study involving 27 pediatric patients (ages 2-17) presenting with PPS. Each patient

Bartter syndrome (BS) is a rare autosomal recessive salt-wasting tubulopathy caused by genetic defects in the thick ascending limb of the loop of Henle. Type IVa BS, presents with severe salt wasting, sensorineural hearing loss, and impaired urinary concentrating ability. We report a 6-month-old female infant with antenatal type IVa BS due to a homozygous BSND variant, who developed secondary congenital nephrogenic diabetes insipidus (NDI), complicating her early clinical course. She presented with failure to thrive, recurrent dehydration, severe electrolyte imbalances, and profound polyuria.

PURPOSE OF REVIEW: Renal bone disease has significant detrimental effects on both cardiovascular and bone health. It is important to understand that the pathophysiologic mechanisms are different from traditional causes of bone disease; and thus, the monitoring and treatment of this disease process requires special attention. Although new guidelines are overdue, progress has been made in the treatment of hyperphosphatemia as well as investigational therapies for renal osteodystrophy.
RECENT FINDINGS: At a time when the treatment of hyperphosphatemia in chronic kidney disease was once diet and

Nephrogenic diabetes insipidus (NDI) is a rare hereditary disorder characterized by renal resistance to arginine vasopressin (AVP), resulting in the kidneys' inability to concentrate urine. Approximately 90% of NDI cases follow an X-linked inheritance pattern and are associated with pathogenic variants in the AVPR2 gene, which encodes the vasopressin receptor type 2. The remaining 10% are attributed to mutations in the AQP2 gene, which encodes aquaporin-2, and may follow either autosomal dominant or recessive inheritance patterns. We present the case of a male infant, younger than nine months

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is a well-known adverse effect of lithium, which occurs in approximately 20%-40% of long-term lithium users. Although rare, there have been reports of central diabetes insipidus (CDI) associated with lithium use. Herein, we report a patient with suspected CDI associated with chronic lithium therapy. Furthermore, we conducted a literature search for cases with CDI and discuss the pathogenesis of this case based on previous reports.
CASE PRESENTATION: The patient was a 73-year-old man with bipolar disorder Type I. His psychiatric symptoms had bee

X-linked nephrogenic diabetes insipidus (X-NDI) is a rare congenital disease caused by inactivating mutations of the vasopressin type-2 receptor (AVPR2), characterized by impaired renal concentrating ability, dramatic polyuria, polydipsia and risk of dehydration. This study aims to elucidate the pathogenic mechanisms associated with a novel variant in the AVPR2 gene, which has been implicated in X-NDI. Whole exome sequencing (WES) was employed to identify genetic variants, complemented by bioinformatic analyses to predict the functional impact of these mutations. A male patient, aged 11.5 year

Histomorphometric analysis of an iliac bone biopsy remains the gold standard for the diagnosis of renal osteodystrophy (ROD), which comprises various histological lesions induced by chronic kidney disease (CKD). ROD belongs to the framework of CKD-associated osteoporosis. The use of bone biopsy in the routine management of CKD-associated osteoporosis has decreased over the past decades for various reasons, including diminishing expertise in performing the procedure, and major variability in processing bone samples and reporting of results. In this context, the European Renal Osteodystrophy gro

Introduction Nephrogenic diabetes insipidus (NDI) is a rare condition caused by renal resistance to the action of antidiuretic hormone (ADH) at the level of the distal tubule, resulting in impaired urinary concentration and consequent polyuria. NDI may be hereditary, most commonly X-linked due to AVPR2 gene mutations, or acquired. Objective To characterize the clinical features, management strategies, and outcomes of patients with NDI followed at a tertiary pediatric nephrology center. Methods A retrospective observational study was conducted, including pediatric patients diagnosed with NDI be

Lithium-induced kidney injury is commonly associated with the development of nephrogenic diabetes insipidus. Longer term lithium exposure is associated with the development of chronic interstitial fibrosis. The mechanisms of lithium-induced kidney injury are multifaceted, affecting many intracellular cell signaling pathways associated with cell cycle regulation, cell proliferation, and subsequent increased extracellular matrix formation and interstitial fibrosis. Amiloride has been demonstrated to have multiple pleiotropic actions, independent of its competitive antagonism of ENaC, in reducing

Reduced protein abundance is the most frequent mechanism by which rare missense variants cause disease. A promising therapeutic avenue for treating reduced abundance variants is pharmacological chaperones (PCs, also known as correctors or stabilizers), small molecules that bind to and stabilize target proteins. PCs have been approved as clinical treatments for specific variants, but protein energetics suggest their effects might be much more general. To comprehensively assess PC efficacy for variation in a given protein, it is necessary to first assign the molecular mechanism explaining all pa

OBJECTIVES: Congenital nephrogenic diabetes insipidus (NDI) is a rare hereditary disorder caused by mutations in two critical genes: arginine vasopressin receptor 2 (AVPR2) and aquaporin 2 genes (AQP2). Mutations in AVPR2 gene, which are predominantly X-linked, account for a significant proportion of cases, particularly in men. Nevertheless, research on this condition in the western Chinese population remains limited.
METHODS: Eleven participants from nine families with NDI were screened for AVPR2 mutations. Their clinical features were documented, and genotype-phenotype associations were inv

Aquaporin-3 (AQP3) is expressed in the basolateral membrane of the renal principal cell, contributing to vasopressin-mediated water reabsorption and urine concentration. We reported that post-translational acetylation of lysine 282 of AQP3 promotes water permeability. In this study, we hypothesized that AQP3 acetylation may improve polyuria in a mouse model of lithium-induced nephrogenic diabetes insipidus (Li-NDI). Wild type, AQP3 acetylation (K282Q), and deacetylation (K282R) mimetic mice were fed a lithium-containing diet or a control diet for 14 days. Body masses and spot urines were colle
