Association of Bone Mass, Falls, and Vertebral Fractures in Older Patients on Hemodialysis: The Role of Comprehensive Geriatric Assessment.

BACKGROUND: End-stage renal disease (ESRD) requiring hemodialysis (HD) disproportionately affects older adults, who face increased risks of falls and fractures due to age-related changes, renal osteodystrophy, comorbidities and dialysis-related factors. Although bone abnormalities and fall risk have often been examined separately in patients with ESRD, few studies have evaluated vertebral compression fractures (VCF) in older HD patients while concurrently assessing skeletal, functional and geriatric risk factors. The role of Comprehensive Geriatric Assessment (CGA) in identifying multidimensio

SPECT/CT confirms metastatic pulmonary calcification on bone scintigraphy: Directing management in a dialysis patient.

We report a case of metastatic pulmonary calcification diagnosed by 99mTc-MDP bone scintigraphy with SPECT/CT in a 52-year-old man with end-stage renal disease on hemodialysis. The patient presented with bone pain and mild cough, and had severe biochemical derangements of chronic kidney disease-mineral and bone disorder. This definitive objective evidence directly prompted immediate escalation of medical therapy. The case highlights the pivotal role of functional imaging in both diagnosing this condition and catalyzing aggressive clinical management.

The Aesthetic Form of Perezhivanie: An Ethnographic Study of Becoming an Environmental Scholar-Activist.

End-stage renal disease is closely associated with changes in bone and mineral metabolism. In recent times, osteoporosis has become important among hemodialysis (HD) patients. In this study, the investigators sought to evaluate the relationship between bone mineral density (BMD) and biochemical markers of bone turnover among HD patients. A total of 70 uremic patients on a maintenance HD program for at least 1 y were enrolled in the study. All patients were treated with conventional bicarbonated HD for 5 h through the use of low-flux hollow-fiber dialyzers. Bone densitometry was measured by dua

Challenging Ureteroscopy in Complex Anatomy: A Case Report.

Ureteroscopy is a well-established minimally invasive modality for the management of ureteric calculi; however, technical difficulty may arise in the presence of impacted stones and distorted urinary tract anatomy. We present a case of challenging ureteroscopic lithotripsy in a 24-year-old man with a symptomatic impacted distal ureteric calculus measuring approximately 9 × 8 × 10 mm, associated with hydroureteronephrosis and markedly altered pelvic orientation due to kyphoscoliosis. The patient also had underlying metabolic bone disease with renal osteodystrophy, which further complicated peri

Incidence, risk factors and survival outcomes of post-transplant tertiary hyperparathyroidism in kidney recipients.

BACKGROUND: Post-transplant tertiary hyperparathyroidism (PT-tHPT) is a well-recognized complication following kidney transplantation, characterized by persistent excessive secretion of parathyroid hormone (PTH) despite improved renal function. It is potentially associated with an increased risk of cardiovascular events, renal osteodystrophy, pathologic fractures, graft loss, and mortality. AIM: To evaluate the incidence, risk factors, and outcomes of PT-tHPT amongst kidney transplant recipients. METHODS: A total of 887 transplant recipients who underwent transplantation between 2000 and 202

Second Chances or Setbacks: The Impact of Parathyroidectomy on Graft Failure Post-Kidney Transplantation.

BACKGROUND: Tertiary hyperparathyroidism is a common complication in kidney transplant (KT) recipients, often persisting despite successful graft function. However, the optimal treatment for improving long-term graft survival remains unclear. Parathyroidectomy (PTX) offers a definitive approach to restoring calcium homeostasis. This study aims to assess the effect of PTX on graft failure, the need for dialysis, and renal osteodystrophy in the 2 groups. STUDY DESIGN: A retrospective analysis of electronic medical records from a multi-institutional database (TriNetX) was completed. Patients dia

Uremia and parathyroid hormone have distinct effects on bone protein and gene expression.

INTRODUCTION: Chronic kidney disease (CKD)-associated osteoporosis is highly prevalent and increases the risk of fractures. Current therapy aims to preserve normal bone turnover, by maintaining parathyroid hormone (PTH) at relatively adequate levels. However, these patients continue to experience fractures. METHODS: To assess potential adverse effects of PTH on bone, we examined bone histology and osteocytic gene and protein expression in 76 patients receiving hemodialysis compared to that of 33 control individuals. RESULTS: Using bulk bone tissue RNAseq, we observed that CKD is associated w

Osteoporosis and CKD-Metabolic Bone Disease Under the Same Umbrella: Insights From a Joint Scientific Symposium.

Osteoporosis and chronic kidney disease (CKD)-metabolic bone disease (MBD) (CKD-MBD) are increasingly recognized as overlapping conditions, particularly in the aging population. Declining renal function and skeletal fragility often coexist, because CKD-MBD may develop in a skeleton already compromised by preexisting osteoporosis. Adynamic bone, often resulting from excessive suppression of parathyroid hormone (PTH) and now a common form of renal osteodystrophy (ROD), may histologically resemble low-turnover osteoporosis; distinguishing between the 2 under light microscopy remains difficult, an

Mid-term Clinical and Radiological Outcome of the BioBall Adapter System in Revision Total Hip Arthroplasty: A Retrospective Study.

AIM AND BACKGROUND: Revision total hip arthroplasty (rTHA) is a technically demanding procedure, particularly when the femoral stem is well-fixed. Removal of a stable stem increases operative time, morbidity, and risk of complications. The BioBall modular head-neck adapter system allows intraoperative restoration of leg length, offset, and version without stem removal. This study evaluates the mid-term clinical and radiological outcomes of the BioBall system in revision THA. METHODS: A retrospective, single-center observational case series was conducted including all rTHAs performed at Wright

Severe Manifestation of Hyperparathyroidism With Sagliker Syndrome: A Case Report.

Sagliker syndrome represents a rare and severe manifestation of secondary renal osteodystrophy in patients with end-stage chronic kidney disease. We present the case of a 26-year-old woman on peritoneal dialysis with a one-year history of progressive craniofacial deformity involving both the maxilla and mandible. Laboratory tests demonstrated markedly elevated parathyroid hormone levels (1,936 pg/mL). Cranial CT revealed a diffuse "salt and pepper" pattern, serpiginous lytic trabecular changes, and focal ground-glass areas in the calvarium. Correlation of severe hyperparathyroidism with these

Temporomandibular Joint Abnormalities in Hemodialysis Patients: A Cross-Sectional Ultrasonographic Study from a Single Center in Italy.

Background/Objectives: Temporomandibular joint (TMJ) abnormalities are a common finding in the general adult population, but they have been rarely investigated in people receiving chronic dialysis. The TMJ is a unique synovial joint that can be altered by either inflammatory or degenerative processes, both of which are amenable to ultrasound (US) assessment. The aim of this study was to describe the pattern of TMJ involvement in a cohort of hemodialysis patients using TMJ ultrasound (TMJ US). Methods: This cross-sectional, single-center study evaluated the clinical utility of TMJ US to detect

Association of 25-Hydroxyvitamin D and Bone Turnover Markers Levels With Bone Mineral Density Measurement in Renal Transplant Patients.

OBJECTIVES: Chronic kidney disease is associated with mineral and bone disorders, including abnormalities in calcium, phosphorus, parathyroid hormone, and vitamin D metabolism, as well as bone diseases like renal osteodystrophy. Although renal transplant can improve some bone mineral disorders, osteoporosis and altered bone metabolism remain important issues due to factors like corticosteroid use and persistent hyperparathyroidism. Bone biopsies are the gold standard for diagnosing bone disorders but are invasive, leading to the exploration of noninvasive methods such as bone turnover markers

Clinical risk factors associated with bone biopsy findings in CKD-associated osteoporosis: insights from the Brazilian Registry of Bone Biopsy (REBRABO).

UNLABELLED: Chronic kidney disease-associated osteoporosis (CKD-OP) is a highly prevalent-yet frequently underrecognized-condition in patients with advanced CKD. The assessment of bone turnover, mineralization, and volume remains challenging in clinical practice, often requiring bone histomorphometric analysis. Using data from the Brazilian Registry of Bone Biopsies (REBRABO), we identified clinical characteristics associated with these histomorphometric parameters. These findings may help clinicians better characterize the underlying bone phenotype and support a more individualized approach t

Renal osteodystrophy in Egyptian CKD patients: observations from clinically indicated bone biopsies.

BACKGROUND: Renal osteodystrophy (ROD) varies among populations. Different genetic and environmental factors and prescription patterns may explain this variation. Egyptian ROD is not sufficiently studied. Although bone biopsy is the gold standard tool, no single study reported the actual ROD spectrum based on bone biopsy in Egypt or Africa. International guidelines must consider the different patterns in developing countries. METHODOLOGY: The ISN-sistership program enabled us to create an Egyptian bone biopsy consortium that provided a nationwide specialized CKD-MBD service. We included all CK

The Janus face of parathyroid hormone in renal osteodystrophy.

This editorial comment discusses novel insights into renal osteodystrophy from a parallel publication, highlighting that uremia per se independently impairs bone cell viability and induces skeletal parathyroid hormone resistance. Using multimodal bone analyses, Wagner et al. show that parathyroid hormone exerts compartment-specific effects: protective for trabecular osteocytes yet detrimental to cortical integrity via increased porosity and by bone fibrosis and increased resorption. These findings redefine optimal parathyroid hormone targets and emphasize addressing uremic toxicity to reduce f

Diagnostic agreement of bone histomorphometry for renal bone disease.

INTRODUCTION: An iliac bone biopsy with semi-quantitative histomorphometry is the gold standard for evaluating renal osteodystrophy (ROD). However, lack of standardization of the bone biopsy procedure, sample handling, reading and reporting represent barriers to research synthesis and clinical collaboration. This study assessed diagnostic agreement across European laboratories performing histomorphometry for evaluation of ROD. METHODS: Five-millimeter transiliac biopsy slides from 5 cases were converted to digital images for analysis by experts from 7 laboratories. Parameters included quantit

A need for normalization of bone histomorphometry parameters in chronic kidney disease.

PURPOSE OF REVIEW: Bone histomorphometry remains the only method capable of providing a direct, tissue-level assessment of bone remodeling, mineralization, and bone volume in patients with chronic kidney disease (CKD). The introduction of the TMV (turnover, mineralization, volume) classification represented a major conceptual advance by offering a unified framework to describe the heterogeneous skeletal abnormalities of renal osteodystrophy. However, nearly two decades after its introduction, the clinical and research application of the TMV system remains inconsistent. This limitation reflects

Renal osteodystrophy in pediatric patients. A systematic review and meta-analysis.

Chronic kidney disease (CKD) in children is associated with significant comorbidities, including mineral and bone disorder (CKD-MBD). A key manifestation of CKD-MBD is renal osteodystrophy (ROD), diagnosed by bone biopsy followed by histomorphometric analysis. In pediatric patients, ROD can lead to bone deformities and impaired growth. However, the limited use of bone biopsy in this population hampers the understanding of ROD patterns and severity. This systematic review and meta-analysis aimed to evaluate histomorphometric findings in children with CKD. We screened 5082 abstracts published up
