Chronic kidney disease (CKD) affects 14% individuals worldwide. Changes in bone and mineral metabolism in CKD are ubiquitous (CKD- Mineral and Bone Disorder) including renal osteodystrophy with a fracture incidence of up to 5 times greater than the general population. While PTH lowering is the mainstay of therapy for decades this has not reduced fractures in patients or improve bone mechanics in pre-clinical models. This suggests additional factors involved in the pathogenesis of renal osteodystrophy such as uremic toxins due to either increased production or decreased renal excretion. One gro

Secondary hyperparathyroidism (SHPT) is a common complication of chronic kidney disease. Prolonged stimulation of the parathyroid glands can lead to severe mineral bone disorders. Uremic tumoral calcinosis (UTC) is a rare but debilitating manifestation of prolonged hyperparathyroidism characterized by periarticular calcium-phosphate deposits. This case describes the progression of SHPT to multifocal UTC in a patient with autosomal dominant polycystic kidney disease (ADPKD). A 46-year-old female with ADPKD-related end-stage renal disease (ESRD) was evaluated for renal transplantation and found

Secondary hyperparathyroidism (SHPT) is a major component of chronic kidney disease-mineral and bone disorder (CKD-MBD), reflecting progressive disturbances in mineral metabolism, endocrine signaling, skeletal remodeling, and parathyroid-gland biology. Traditionally, preoperative parathyroid hormone (PTH) has been used primarily as a biochemical threshold for surgical referral. However, persistent PTH elevation in advanced CKD-related SHPT may reflect more than isolated endocrine activity; available evidence suggests it integrates parathyroid-gland remodeling, receptor resistance, skeletal tur

Chronic kidney disease-mineral and bone disorder (CKD-MDB) is a systemic disorder that occurs as a complication of advanced chronic kidney disease. It includes biochemical alterations (calcium, phosphorus, parathyroid hormone, vitamin D), abnormalities in bone turnover and mineralization, and vascular and soft-tissue calcifications. The development of secondary hyperparathyroidism and profound alterations in bone remodeling culminate in renal osteodystrophy, which contributes to adverse cardiovascular outcomes and increased mortality. This narrative review article aims to summarize the role of

Chronic renal disease (CRD) is a progressive, irreversible loss of renal mass and function, posing a significant health concern. Secondary hyperparathyroidism caused by hypokalcemia in CRD often leads to renal osteodystrophy (ROD), a bone pathology resulting from alterations in mineral metabolism. ROD typically occurs early in renal failure and progresses as kidney function deteriorates. Marked jaw enlargement remains a rare but severe complication of the ROD, causing significant facial disfigurement. This case series highlights the four cases of ROD with extensive orofacial involvement, empha

The pathophysiology of chronic kidney disease (CKD)-related mineral and bone disorder (CKD-MBD) extends far beyond simple mineral imbalances. Evolving from the traditional understanding of secondary hyperparathyroidism, the conceptual framework has recently been updated to recognize 2 distinct but overlapping clinical syndromes: CKD-associated osteoporosis, which encompasses the significantly increased fracture risk and microarchitectural deterioration in this population; and CKD-associated cardiovascular disease, which accounts for vascular and structural cardiac abnormalities, including medi

Chronic kidney disease-mineral and bone disorder (CKD-MBD) is a well-recognized complication of end-stage kidney disease (ESKD), encompassing abnormalities in calcium, phosphorus, parathyroid hormone (PTH), vitamin D metabolism, and bone turnover. In advanced cases, secondary or tertiary hyperparathyroidism may lead to severe skeletal disease, including osteitis fibrosa cystica and brown tumors, which can mimic hematologic malignancies. We present the case of a 47-year-old man with childhood-onset focal segmental glomerulosclerosis (FSGS), two prior kidney transplants, and long-standing dialys

Metabolites mediate inter-organ communication and this metabolic crosstalk often goes awry in systemic diseases, including chronic kidney disease-mineral bone disorder (CKD-MBD), where CKD disrupts skeletal homeostasis, with significant bone loss and increased fracture risk observed in patients. While metabolites from the injured kidney have been identified to disrupt bone function, whether metabolites from the bone influence kidney function over the course of CKD progression is less-understood. Given that bone-derived factors such as FGF23 and sclerostin influence CKD-MBD disease progression,

While [18F]-FDG PET/CT is primarily used to evaluate malignancy, it may occasionally reveal metabolic or endocrine abnormalities. A 45-year-old male with ESRD on hemodialysis presented with generalized bone pain and radiologic suspicion of malignancy. [18F]-FDG PET/CT demonstrated diffusely increased skeletal uptake and multifocal lytic bone lesions, consistent with metabolic bone disease secondary to renal osteodystrophy. Incidental

BACKGROUND: The use of small doses of vitamin D3 analogs or metabolites is common in the prevention of renal osteodystrophy. Their use in the treatment of osteoporosis has been limited by their potent adverse effects, causing hypercalcemia and hypercalciuria. We evaluated a vitamin D2 analog, i.e., 1α-hydroxyvitamin D2 (doxercalciferol), that has a lower incidence of adverse effects.
METHODS: In a 1-year pilot randomized double-blind, placebo-controlled study, 55 postmenopausal women (aged 60-71 years) with osteopenia were treated for a year with either doxercalciferol or placebo, and 41 wome

The analysis of the etiology, pathogenesis and clinical features of the appearance of pain syndrome in the maxillofacial region in patients with chronic kidney disease is presented. The mechanisms of the effect of uremic intoxication on the oral mucosa and the peripheral nervous system are considered. Specific conditions such as uremic stomatitis, trigeminal neuropathy, and burning mouth syndrome have been identified. Special attention is paid to the manifestations of bone mineral disorders (CKD-MBD), including renal osteodystrophy of the jaws. The approach to pharmacological pain relief is sy

CKD-MBD is a metabolic disorder characterized by disrupted mineral metabolism, leading to vascular calcification, renal osteodystrophy, and high fracture risk. Sclerostin, an osteocyte-derived Wnt inhibitor, is a key regulator of both bone loss and vascular calcification. This study explored the role of phlorizin (SGLT1 and SGLT2 inhibitor) on bone loss and metabolic profile in CKD-MBD. Calvarial osteoblasts were treated with phlorizin and evaluated for ALP activity, viability, mineralization, and gene/protein expression. Molecular docking and simulation studies were performed to check phloriz

INTRODUCTION: Axial spondyloarthritis (axSpA) is a chronic inflammatory disease characterized by entheseal inflammation, pathological new bone formation and structural lesions on spinal radiography. Chronic kidney disease (CKD) especially in its moderate to advanced stages manifests with abnormalities such as secondary hyperparathyroidism, renal osteodystrophy and vascular or ligamentous calcifications. This study aimed to compare radiographic spinal abnormalities in axSpA patients with CKD, axSpA patients without CKD, and CKD patients without inflammatory arthritis.
METHODS: This retrospecti

High bone mineral density (BMD) is common and sometimes an incidental finding. The causes are numerous. Among them, none has previously been attributed to total body irradiation (TBI). We present the case of a 56-year-old female patient with a history of T-lymphoblastic lymphoma at age 33 who was treated with allogeneic hematopoietic stem cell transplantation following a conditioning regimen including a single-fraction 10 Gray TBI. This patient was in complete remission but experienced several transplant-related late effects. She presented to the rheumatology outpatient clinic with chronic mec

BACKGROUND: "Dry mouth" syndrome (chronic hyposalivation) can be caused by a number of pathophysiological conditions such as acute and chronic stress exposure, abnormal body weight (both too high and too low ones), eating disorders (such as anorexia nervosa), metabolic syndrome(s), Sjögren's and Sicca syndromes, drugs and head/neck radiotherapy application. In turn, the chronic hyposalivation as a suboptimal health condition significantly reduces quality of life, may indicate a systemic dehydration, provokes and contributes to a number of pathologies such as a strongly compromised protection o

Emerging evidence highlights the potential of bioactive compounds, particularly polyphenols, as adjunctive therapeutic agents in the treatment of pancreatic cancer (PC), one of the most aggressive malignancies. This review focuses on epigallocatechin gallate (EGCG) and resveratrol due to their extensively documented anticancer activity, favorable safety profiles, and their unique ability to modulate multiple signaling pathways relevant to pancreatic tumorigenesis. Among polyphenols, these two have shown superior anti-cancer activity, epigenetic regulatory effects, and synergy with standard che

BACKGROUND/AIMS: Assessment of bone turnover for management of renal osteodystrophy is part of routine care in chronic kidney disease Stage 5 (CKD-5) patients. Measurement of intact parathyroid hormone (iPTH) is the most commonly used surrogate marker for bone turnover in these patients. The current study was conducted to evaluate the predictive value of the five most commonly used iPTH assays for bone turnover.
METHODS: In a cross-sectional study, 84 CKD-5 patients underwent bone biopsy and blood drawings for determination of iPTH and total serum alkaline phosphatase (AP).
RESULTS: Histolog

Bone turnover assessment and monitoring are essential for chronic kidney disease (CKD)-associated bone care. Patients with CKD suffer from significantly elevated fracture risk due to abnormally high or low bone turnover, which benefits from diametrically opposite treatments informed by patient-specific bone turnover data. However, a reliable, accessible, noninvasive bone turnover assessment and monitoring tool remains an unmet clinical need. Combining time-lapse (TL) analysis with high-resolution peripheral quantitative computed tomography (HR-pQCT) scans obtained over time allows for in vivo

Sclerosing bone dysplasias encompass abnormalities in bone density, divided into hereditary and nonhereditary forms. Primarily diagnosed through radiography, they are often incidental findings. Among the hereditary forms, the following stand out: osteopetrosis, osteopoikilosis, multiple diaphyseal sclerosis (ribbing disease), osteopathia striata, and Camurati-Engelmann disease. Among the nonhereditary forms, intramedullary osteosclerosis and melorheostosis present specific radiographic characteristics. The main differential diagnoses include osteoblastic metastases, tuberous sclerosis, and ren
