BACKGROUND: Chronic kidney disease - mineral bone disorder (CKD-MBD) is a syndrome that begins early in CKD, contributes to CKD-associated mortality, and includes components of FGF23 elevation, αklotho deficiency, CKD-stimulated vascular disease, and renal osteodystrophy. Hyperphosphatemia, occurring in later stages of CKD, is also driven by mechanisms of CKD-MBD, and has been shown to stimulate vascular calcification. In a mouse model of Alport CKD that is resistant to vascular calcification, we examine the effects of a high-phosphate Western-type diet on the CKD-MBD, and test whether the die

Chronic kidney disease (CKD) represents a major global health challenge, frequently resulting in the development of chronic kidney disease-mineral and bone disorder (CKD-MBD). Transient receptor potential (TRP) channels, particularly the TRPV (vanilloid), TRPC (canonical), and TRPM (melastatin) subfamilies, are crucial in CKD-MBD by regulating calcium homeostasis, bone remodeling, and vascular calcification. Pharmacological agents targeting TRP channels and traditional Chinese medicine therapies demonstrate promising therapeutic potential for CKD-MBD. This article explores the role of TRP chan

BACKGROUND: Hemodialysis (HD) patients frequently experience uremic pruritus, a condition that is both irritating and prevalent. Although its pathogenesis is not entirely understood, it is believed to be multifactorial. The objective of this investigation was to evaluate the renal osteodystrophy indices in patients undergoing hemodialysis with and without pruritus.
METHODS: This cross-sectional study categorized HD patients in 3 university hospitals in Mashhad, northeast Iran, during 2022 and 2023 into 2 groups: those with and those without pruritus, as determined by the diagnostic criteria.

KEY POINTS: Serum glycerol-3-phosphate (G3P) levels were significantly associated with higher fibroblast growth factor 23 levels in ESKD. Pathway and mediation analyses identified the pentose phosphate pathway as the top mediator of the relationship between serum G3P and fibroblast growth factor 23. The role of G3P in ESKD provides new insights into potential therapeutic targets for CKD-mineral and bone disorder.
BACKGROUND: Fibroblast growth factor 23 (FGF23) is a key regulator of mineral metabolism that is independently associated with mortality in patients with ESKD. Glycerol-3-phosphate (

Spinal surgery in patients with chronic kidney disease and end-stage renal disease on hemodialysis presents unique challenges due to systemic comorbidities, poor bone quality, and high perioperative risks. This review aims to evaluate outcomes, complications, and optimal surgical strategies in this high-risk population that reveals significantly higher rates of perioperative mortality (up to 17.2%), intensive care unit admission, and implant failure compared to nonrenal cohorts due to uremic toxicity, renal osteodystrophy, and cardiovascular vulnerabilities. Posterolateral fusion and minimally

Patients with chronic kidney disease (CKD) face elevated fracture incidences, but mechanisms underlying CKD-related bone loss remain unclear. Using the adenine-induced chronic kidney injury (AdKI) murine model, we identified that AdKI induces dysregulated glucose metabolism in bones and kidneys via in vivo and ex vivo metabolic tracing. Ex vivo 13C-metabolic tracing of osteocyte-enriched femora revealed accelerated citrate production from [1,2-13C]-glucose and [U-13C]-glutamine in AdKI mice. These metabolic changes were observed together with increased circulating citrate and Slc13a5 overexpre

OBJECTIVE: The onset of chronic kidney disease during childhood substantially increases the risk of cardiovascular complications in adulthood among affected patients. In such patients, chronic damage beginning in childhood has been linked to higher long-term mortality rates, and bone changes may also occur due to factors other than renal osteodystrophy. This study investigates the burden of cardiovascular risk among children with pre-dialysis chronic kidney disease and examines the relationship between skeletal mineral content and the progression of kidney dysfunction.

Growing evidence indicates an interesting interplay between kidney and bone. The pathophysiological condition of the skeletal system is intricately associated with the normal functioning of the kidneys. This relationship is modulated by various factors, including calcium and phosphate, 1-α-hydroxylase, erythropoietin (EPO), klotho, fibroblast growth factor 23 (FGF23), bone morphogenetic protein-7 (BMP-7), and extracellular vesicles (EVs). These interactions are notably evident in conditions such as chronic kidney disease with bone mineral density (CKD-BMD), renal osteodystrophy (ROD), and oste

Quantitative information on the composition of bone, specifically the content of calcium phosphate mineral and organic matrix, is essential for accurate diagnosis of metabolic bone diseases such as osteoporosis, osteomalacia, and renal osteodystrophy, as well as for differentiating among these conditions. Conventional MRI fails to provide this information because these substances are solid and, therefore, yield no signal in conventional MRI scans, which typically employ spin or gradient echoes. In this report, we show how phosphorus and proton solid-state MRI yield the desired compositional in

BACKGROUND: Managing osteoporosis (OP) in chronic kidney disease (CKD) presents significant challenges due to altered bone metabolism. Given the lack of robust clinical trial data and a notable knowledge gap exists among nephrologists regarding an optimal management in this population, an expert consensus is crucial for developing tailored management strategies. This study aimed to gather an expert opinion to bridge this gap and establish consensus recommendations on the diagnosis and management of osteoporosis in CKD patients.
METHODS: A panel of 13 Indian and 1 international experts, includ

Fracture risk is a serious yet underrecognized complication among patients with chronic kidney disease (CKD), especially in those with stages G3-G5D. The overlap between CKD-Mineral and Bone Disorder (CKD-MBD) and osteoporosis leads to complex bone changes that increase the likelihood of fragility fractures. Studies show that 18% to 32% of CKD patients also have osteoporosis, and these individuals are more than 2.5 times as likely to suffer from fractures compared to those without CKD. In the advanced stages of the disease, fracture risk is up to four times higher than in the general populatio

First introduced in the 1950s following the development of the transiliac crest trephine and the advent of plastic embedding techniques, undecalcified bone biopsy revolutionised skeletal pathology by allowing in vivo assessment of mineralised bone. The addition of tetracycline double-labelling in 1969 enabled dynamic measurement of bone formation, and by the 1980s, histomorphometric nomenclature had been standardised, laying the foundation for quantitative diagnosis of metabolic bone disease. Despite these advances, the clinical use of undecalcified biopsy has waned, supplanted by surrogate to

Cystinosis is a rare autosomal recessive lysosomal storage disease caused by a defective lysosomal cystine carrier protein, cystinosin, resulting in formation and deposition of cystine crystals throughout the body. The renal manifestations of the disease have long been studied, but the musculoskeletal consequences of the disease are generally less well understood. Limb deformities, scoliosis, myopathy and low bone mineral density are associated with cystinosis and can lead to pain, fragility fractures, bone deformity, and difficulty ambulating. Although potentially exacerbated by renal disease

INTRODUCTION: The incidence of intertrochanteric (IT) fracture is rising because of increase in the survival of the elderly population with osteoporosis and also the increase in the number of motor vehicle accidents. The treatment of choice is normally surgical with internal fixation. The surgical options for these fractures commonly include fixation with dynamic hip screw (DHS) or cephalomedullary nailing. Due to its advantages over DHS, cephalomedullary nailing is the predominant procedure in many parts of the world. Various varieties of cephalomedullary nails are available. Short cephalomed

KEY POINTS: Multitrait analysis of genome-wide association study boosts the statistical power to identify novel genetic traits for fibroblast growth factor 23. A functional genomics approach aided network discovery to identify histidine-rich glycoprotein (HRG) and high-mobility group protein box 1 (HMGB1) as key regulators of cardiac disease in CKD. Integration of clinical and genetic data enhances the discovery power and is crucial for understanding the genetic underpinnings of mineral bone disorder related to CKD.

Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD) disrupts phosphorus and calcium metabolism, leading to skeletal disorders such as demineralization, weakened bones, and an increased fracture risk. These disorders, known as renal osteodystrophy, vary widely and are classified into secondary hyperparathyroidism, adynamic bone, and osteomalacia based on bone turnover. Fracture risk increases with the severity of CKD, particularly in advanced stages (creatinine clearance <15-20 mL/min), but even patients with mild to moderate CKD are at risk of fractures. Fractures of the Lesser Trochant
