A Case of Misdiagnosed Pediatric Congenital Nephrogenic Diabetes Insipidus due to AQP2 Variants.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is a rare disorder of renal water handling, often presenting in early infancy with severe polyuria and hypernatremia. Differentiating CNDI from central diabetes insipidus (CDI) remains challenging, and delayed identification may result in inappropriate therapy, leading to adverse growth and neurodevelopmental outcomes. CASE PRESENTATION: We present the case of an 8-month-old male infant with a history of suspected CDI since birth. He was treated with desmopressin (dDAVP) without clinical benefit. The absence of the posterior pituita

Copeptin and urine concentrating capacity predict kidney function decline in lithium-treated patients.

INTRODUCTION AND OBJECTIVES: A common early side effect of lithium treatment in bipolar disorder is a reduced urine concentrating capacity possibly progressing to arginine vasopressin (AVP) resistance (AVP-R), also known as lithium-induced nephrogenic diabetes insipidus (Li-NDI). Long term lithium therapy can also lead to chronic kidney disease (CKD) with kidney function decline. Due to lack of markers to identify Li-NDI, it is often overseen even though prevalence and impact on quality of life are substantial. Moreover, the correlation between reduction of urine concentrating capacity and CKD

Tonicity drives collecting duct maturation in the mammalian kidney.

Organ maturation is a fundamental biological process and a major challenge for organoid-based regenerative medicine. During kidney maturation, osmolality increases in the renal medulla for urine concentration, yet whether this extreme environment conversely contributes to kidney maturation remains unclear. Here we show that high tonicity, i.e., salt, drives morphological, transcriptional, and functional maturation of medullary collecting ducts (CDs) primarily via nuclear factor of activated T cells 5 (NFAT5). This occurs both in vitro and in vivo. Combining high tonicity with maturation-promot

Gut-liver axis mediates the ameliorative effects of North Hawthorn polysaccharide on liver inflammation and oxidative stress in hyperlipidemic mice via Nrf2 signaling pathway.

Elderly patients are at increased risk of developing lower respiratory tract infections compared with younger patients. In this population, pneumonia is a serious illness with high rates of hospitalisation and mortality, especially in patients requiring admission to intensive care units (ICUs). A wide range of pathogens may be involved depending on different settings of acquisition and patient's health status. Streptococcus pneumoniae is the most common bacterial isolate in community-acquired pneumonia, followed by Haemophilus influenzae, Moraxella catarrhalis and atypical pathogens such as Ch

Severe burns complicated by EDKA, CDI, and NTIS: A case report and literature review.

RATIONALE: Severe burns trigger sustained hypermetabolism marked by protein hypercatabolism, insulin resistance, and relative insulin deficiency. These effects are exacerbated in diabetic patients, increasing susceptibility to acute metabolic crises. PATIENT CONCERNS: We report the first case of a burn patient with type 2 diabetes mellitus (T2DM) concurrently presenting with euglycemic diabetic ketoacidosis (EDKA), central diabetes insipidus (CDI), and non-thyroidal illness syndrome (NTIS). Following severe burn and inhalation injury, he exhibited agitation, delirium, and hypotonic polyuria.

Sodium and Water Homeostasis in Children: Pathogenesis, Diagnosis, and Treatment.

Maintaining homeostasis in the body through water and sodium management is essential, and the central nervous system and kidneys play a key role in this process. However, knowledge of the diagnosis and treatment of these conditions in pediatric patients is still unsystematized. There are no up-to-date guidelines on managing children with sodium imbalance. Since sodium shifts are inextricably linked to water changes in the body, they should always be pondered together. Each of the sodium disorders should be considered in the context of changes in the vascular volume, whether it is hypo-, eu-, o

Use, efficacy, and safety of desmopressin for congenital nephrogenic diabetes insipidus in children: a nationwide survey.

Congenital nephrogenic diabetes insipidus (CNDI) is characterized by resistance of the distal nephrons and collecting ducts to arginine vasopressin (AVP). High doses of 1-deamino-8-D-arginine vasopressin (DDAVP), a V2-receptor-selective agonist, are effective in some cases. The present study aimed to demonstrate the use, efficacy, and safety of DDAVP and the characteristics of patients who responded to this treatment. The present, retrospective, multicentric, observational survey of patients with CNDI receiving DDAVP was based on a previous, nationwide survey conducted by the Japanese Society

Reversible Nephrogenic Diabetes Insipidus Induced by Lithium: A Case Report.

Lithium is an effective mood stabilizer but may cause nephrogenic diabetes insipidus (NDI) by impairing the renal collecting duct response to arginine vasopressin (AVP). We report a 52-year-old woman on long-term lithium therapy who presented with diarrhea, fatigue, polyuria, and confusion. Initial evaluation showed hypernatremia (serum sodium 156-159 mmol/L), low urine osmolality (101 mOsm/kg) despite serum osmolality of 286 mOsm/kg, daily urine output of 6.5-7.5 L, and a lithium level of 1.65 mmol/L. Renal function was preserved. Intravenous 5% dextrose was administered for free-water replac

Identification of a novel large deletion mutation in the AVPR2 gene responsible for hereditary nephrogenic diabetes insipidus in an infant: a case report.

BACKGROUND: Hereditary nephrogenic diabetes insipidus (HNDI) type1 is a rare genetic disorder that results from mutations in the AVPR2 gene, which encodes the arginine vasopressin receptor 2. The primary clinical manifestations of this disorder encompass polydipsia, polyuria and urine with low specific gravity. At present, there is no curative treatment, and the principal treatment goal is to manage symptoms in order to prevent complications such as dehydration, brain injury, and hydroureteronephrosis. CASE DESCRIPTION: This case report presents a 7-month-old boy who presented with persistent

Real-world pharmacovigilance insights and clinical safety signal characterization of the classic mood stabilizer‑lithium carbonate: A disproportionality analysis based on FAERS data.

OBJECTIVE: Lithium carbonate is a classic mood stabilizer for bipolar disorder, yet real-world pharmacovigilance of its safety signals (especially underrecognized systems/populations) is lacking. This study detected disproportional ADR signals via FAERS to clarify its multi-system safety profile and gender/age-specific patterns. METHODS: FAERS data (Q1 2004-Q3 2025) for lithium carbonate as the primary suspected drug were extracted. Duplicate reports were processed per FDA guidelines; ADRs were coded by MedDRA (PTs/SOCs). Disproportionality analysis used ROR, PRR, BCPNN and EBGM algorithms, w

Severe Diabetic Ketoacidosis With Refractory Hypernatremia due to Hypokalemia-Induced Arginine Vasopressin Resistance: A Case Report and Literature Review.

Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes characterized by hyperglycemia, high-anion-gap metabolic acidosis, and ketonemia. Hypernatremia in DKA is uncommon and typically reflects disproportionate free water loss. Arginine vasopressin resistance (AVP-R), previously known as nephrogenic diabetes insipidus, may occur in the setting of hypokalemia, leading to polyuria, hypotonic urine, and worsening hypernatremia. We report the case of a 14-year-old previously healthy female who presented with new-onset severe DKA complicated by profound acidosis, hyperglycemia, h

Infant Nephrogenic Diabetes Insipidus: Challenges Leading to Delayed Management.

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is a rare, potentially life-threatening renal tubular disorder characterized by a decreased ability to concentrate urine due to antidiuretic hormone (ADH) resistance, leading to significant free water loss. The congenital form, most often caused by X-linked mutations in the AVPR2 gene, typically presents in early infancy. Due to its rarity and nonspecific clinical features in young children, such as failure to thrive, vomiting, and excessive fluid intake, diagnostic delays are common, especially in centers with limited prior experience in managi

Malnutrition and hypernatremic dehydration as the initial presentation of nephrogenic diabetes insipidus in an infant.

Congenital nephrogenic diabetes insipidus (CNDI), also known as arginine vasopressin resistance, is a rare inherited disorder of water homeostasis in which the kidneys lose their ability to concentrate urine, leading to polyuria, polydipsia, and a risk of hypertonic dehydration. We report the case of a 3-month-old infant with symptoms present for approximately 2 months, characterized by episodes of fever, irritability, and poor growth. Upon admission, severe malnutrition and dehydration were noted. A diagnosis of CNDI was made based on vasopressin levels, plasma and urine osmolarity values, hy

Proteomic profile of urinary extracellular vesicles from rats with lithium-induced nephrogenic diabetes insipidus.

Lithium (Li) salts have been widely used to treat bipolar disorder and unipolar depression for more than 50 yr. However, up to 40% of people taking Li develop nephrogenic diabetes insipidus (NDI). NDI is associated with cellular remodeling in the rodent kidney collecting duct (CD) and reduced aquaporin-2. Patients taking Li for more than 10-20 yr are at risk of chronic kidney disease, which ultimately can lead to end-stage renal disease, hemodialysis, transplantation, or death. The purpose of this study was to investigate whether the rat urinary proteome can be used as an indicator of Li-induc

Inhaled Sedation in the ICU.

Sedation is often required to facilitate comfort and provide necessary care for patients who are critically ill and require mechanical ventilation. Intravenous sedatives are the current standard of care in this patient population. Inhaled volatile agents that are routinely used for general anesthesia, like isoflurane and sevoflurane, are also being explored for their potential role in the ICU setting. Already, inhaled volatile agents are prescribed for routine ICU sedation in several countries, though their use for ICU sedation is not approved by United States regulatory authorities as of the

Clean intermittent catheterization reverses hydronephrosis in a child with congenital nephrogenic diabetes insipidus: a case report.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is most frequently caused by mutations in the AVPR2 gene. Patients exhibit persistent polyuria due to renal insensitivity to antidiuretic hormone. Chronic high urine output predisposes to bladder dysfunction and upper urinary-tract dilatation, notably hydronephrosis. Although pharmacotherapy can partially reduce urine volume, its capacity to reverse established hydronephrosis is limited. Clean intermittent catheterization (CIC), a mainstay in managing neurogenic bladder, warrants investigation regarding its utility in CNDI-associated
