Navigating the Therapeutic Tightrope: Refractory Lithium-Induced Nephrogenic Diabetes Insipidus in a Patient With Advanced Chronic Kidney Disease.

Lithium is the most common and effective mood stabiliser in the management of bipolar disorder. Lithium-induced nephrogenic diabetes insipidus (NDI) is a well-recognised complication of long-term mood stabiliser therapy. However, management becomes exponentially more complex when therapeutic options, such as thiazides or amiloride, are contraindicated due to advanced chronic kidney disease (CKD) or electrolyte instability. We are reporting the case of a man in his 60s with a 40-year history of lithium use who presented with acute-on-chronic kidney injury (AKI) and severe hypernatraemia (164 mm

Central Diabetes Insipidus as a Rare Cause of Polyuria.

Central diabetes insipidus (CDI) is a rare condition characterized by arginine vasopressin deficiency, leading to persistent polyuria and polydipsia. Although it can present at any age, idiopathic CDI in young adults is uncommon and diagnostically challenging. Inflammatory conditions such as infundibuloneurohypophysitis must be considered, particularly when magnetic resonance imaging (MRI) reveals a thickened pituitary stalk and absence of the posterior pituitary bright spot. Early recognition and differentiation from diabetes mellitus, nephrogenic diabetes insipidus, and primary polydipsia ar

From mechanisms to clinical practice: a review of diuretic therapy in pediatric nephrology.

Diuretics are a cornerstone of supportive therapy in pediatric patients, widely used for the management of edema, fluid overload, hypertension, and various other kidney diseases. Their clinical utility, however, is strongly influenced by developmental differences in kidney physiology, age-related pharmacokinetics, underlying kidney disease, and the risk of adverse effects unique to children. This review presents an extensive, evidence-informed synthesis of diuretic therapy in pediatric kidney disorders, encompassing pharmacology, mechanisms of action, indications, drug interactions, and safety

X-Linked Nephrogenic Diabetes Insipidus Associated with the AVPR2 c.964C>T (p.Pro322Ser) Variant: A Family Case Series.

Background: Nephrogenic diabetes insipidus (NDI) is a rare disorder characterized by renal resistance to arginine vasopressin, most commonly caused by pathogenic variants in the AVPR2 gene. While X-linked NDI classically affects males, heterozygous females may exhibit variable clinical expression. Certain AVPR2 variants are associated with partial NDI and milder phenotypes. Methods: We conducted a retrospective family study of a multigenerational Greek pedigree with suspected hereditary NDI. Clinical, biochemical, and pedigree data were collected through chart review and family interviews. Gen

Lithium in Bipolar Disorder: Renal Mechanisms, Nephrotoxicity Phenotypes, and a Shared-Care Pathway for Clinical Practice.

Lithium remains a cornerstone mood stabilizer for bipolar disorder, with strong evidence for relapse prevention and a unique association with reduced suicide risk. Its benefit is counterbalanced by renal adverse effects, ranging from impaired urinary concentrating capacity and nephrogenic diabetes insipidus (NDI) to chronic tubulointerstitial nephropathy with progressive loss of glomerular filtration rate in a minority of long-term users. Mechanistically, lithium enters collecting duct principal cells via the epithelial sodium channel, disrupts vasopressin-regulated water handling by downregul

Beyond Classical Diabetes Insipidus: A Retrospective Case Series Illustrating the Diagnostic Complexity and Diverse Etiologies of Polyuria-Polydipsia Syndrome.

Background Polyuria-polydipsia syndrome encompasses a heterogeneous group of disorders, including central diabetes insipidus (CDI), nephrogenic diabetes insipidus (NDI), and primary polydipsia. Despite presenting with similar clinical manifestations, these conditions differ markedly in their underlying pathophysiology, prognosis, and management. Distinguishing between them remains clinically challenging, particularly in patients with partial disease and in resource-limited settings where newer diagnostic biomarkers such as copeptin are not routinely available. This study highlights the diagnos

Severe recurrent hypernatremia and probable lithium-associated nephrogenic diabetes insipidus following pancreatectomy in a patient with schizophrenia: a case report.

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is characterized by impaired renal responsiveness to arginine vasopressin, causing defective urinary concentration and large volumes of dilute urine. Long-term lithium exposure is an important acquired cause. Perioperative fasting, restricted free-water intake, and ongoing fluid losses may unmask compensated lithium-associated urinary concentrating dysfunction and precipitate severe hypernatremia. CASE PRESENTATION: A 61-year-old man underwent laparoscopic resection of a pancreatic tail lesion. His history included schizophrenia, type 2 diabete

Drug spectrum and disproportionality signals for diabetes insipidus in FAERS: multi-method analysis, JADER cross-database assessment, and PubMed-based case-level evidence.

OBJECTIVE: The aim of this study is to characterize reporting patterns and non-treatment drug safety signals for diabetes insipidus (DI) in the FDA Adverse Event Reporting System (FAERS), evaluate signal stability using crude, adjusted, and shrinkage-based disproportionality methods, and assess signal reproducibility in the Japanese Adverse Drug Event Report (JADER). METHODS: We analyzed FAERS (2004Q1-2025Q4) and JADER (2004-2025) reports. DI cases were defined by MedDRA Preferred Terms "Diabetes insipidus" and "Nephrogenic diabetes insipidus"; central DI-related lower-level terms mapped to "

Impact of fluconazole in vasopressin 2 receptor-related congenital arginine vasopressin resistance.

INTRODUCTION: Arginine vasopressin resistance (AVP-R), previously called nephrogenic diabetes insipidus is a rare inherited disorder characterized by excessive urine output (often over 10 l/d) and a risk of life-threatening dehydration, resulting from kidney collecting duct insensitivity to arginine vasopressin. Currently, no curative treatment exists. Preclinical studies suggest that repurposing of anti-fungal fluconazole enhances AQP2-mediated water transport. Here, we present results from a clinical intervention study evaluating the effect of fluconazole on 24-hour urine output and urine co

Mechanistic Insight into the Role of Maresin-1 in Attenuating Lithium-Induced Nephrogenic Diabetes Insipidus in Rats via Regulation of cAMP/AQP2 Signaling and SERCA2b Dependant Endoplasmic Reticulum Stress.

BACKGROUND: Nephrogenic diabetes insipidus (NDI), one of the common complications of chronic administration of lithium, is characterized by severe urine concentrating defects. It is associated with aberrant regulation of aquaporin-2 (AQP2) proteins as well as endoplasmic reticulum (ER) stress. Therefore, we evaluated renoprotective effects of Maresin-1 (MaR-1) and its associated molecular mechanisms in Li-induced NDI. METHODS: forty-eight adult male Wistar albino rats were randomly distributed into four equal groups: control, MaR-1 control (40 ug/kg/day intraperitoneally), NDI (LiCl; 40 mmol/

[Acute Aortic Dissection Surgery Complicated by Nephrogenic Diabetes Insipidus].

Hypernatremia due to polyuria following cardiac surgery is rare. We report a case that required to identify the cause of massive polyuria. A 50-year-old male underwent aortic arch replacement for acute aortic dissection. Early postoperatively, he developed marked polyuria [over 3,000 ml within 6 hours after intensive care unit (ICU) admission, over 15,000 ml within 24 hours] and hypernatremia (peak value 187 mEq/l). We first assumed vasopressin deficiency due to cardiopulmonary bypass, and desmopressin nasal spray was somewhat effective. Continuous vasopressin infusion combined with massive 5

Dexmedetomidine-Associated Polyuria Mimicking Diabetes Insipidus.

UNLABELLED: Dexmedetomidine is a highly selective α2-adrenergic agonist widely used for sedation in the intensive care unit (ICU) and is generally considered well tolerated, with hypotension and bradycardia being the most commonly reported adverse effects. Rarely, dexmedetomidine has been associated with polyuria resembling a diabetes insipidus-like syndrome, particularly during prolonged infusions. We report the case of a 53-year-old male admitted to the ICU for severe alcohol withdrawal who developed profound polyuria during a 5-day dexmedetomidine infusion. Urine output progressively increa

[Lithium-induced electrolyte disturbances and catatonia].

In this case report, a 57-year-old man with bipolar disorder on long-term lithium treatment was admitted with catatonia. Laboratory tests revealed hypercalcemia with parathyroid hormone levels in the upper third of the normal reference interval, consistent with lithium-associated hyperparathyroidism, as well as nephrogenic diabetes insipidus. Pausing lithium and treating hypercalcemia led to a gradual improvement in catatonia. The case emphasises the importance of considering endocrine and electrolyte disturbances in cases of acute deterioration of psychiatric symptoms during lithium treatment

Recurrent Catatonia in the Setting of Urinary Tract Infection and Medical Comorbidity: A Case Report.

Catatonia is a neuropsychiatric syndrome characterized by disturbances in motor activity, affect, and autonomic function. Although often associated with primary mood or psychotic disorders, it can be precipitated or sustained by systemic infections such as urinary tract infection (UTI). We describe a 60-year-old man with recurrent catatonia and diagnoses of schizophrenia, bipolar disorder, and major neurocognitive disorder, whose latest episode of catatonia, described in this case report, was preceded by psychosocial stress and appeared to be exacerbated by polymicrobial UTI, bacteremia, and u

Approach to the Patient With Suspected Hypotonic Polyuria.

Investigation and management of hypotonic polyuria is a common challenge in clinical endocrinology. The 3 main causes, recently renamed to arginine vasopressin deficiency (AVP-D, formerly central diabetes insipidus), arginine vasopressin resistance (AVP-R, formerly nephrogenic diabetes insipidus), and primary polydipsia (PP) require accurate diagnosis, as management differs for each. This new nomenclature more accurately reflects pathophysiology and has now been adopted by the Systemized Nomenclature of Medicine (SNOMED). Advances in diagnosis over the last few years have centered around the u

Autoimmune Tubulopathies.

The renal tubule and collecting duct express a large number of proteins, all having putative immunoreactive motives. Therefore, all can be the target of pathogenic autoantibodies. However, autoimmune tubulopathies seem to be rare, and we hypothesize that they are underdiagnosed. This review summarizes the current knowledge on autoimmune tubulopathies. We elected to classify tubulopathies according to the segment that is targeted because this determines, at least in part, the phenotypic presentation. In the proximal tubule, autoantibodies can cause anti-brush border antibody disease, renal Fanc

Investigation of the effects of silymarin and vitamin C on kidney damage and aquaporin-2 downregulation in lithium-induced nephrogenic diabetes insipidus in rats.

Although lithium (LIT) therapy is key in managing bipolar disorder long-term, prolonged use significantly contributes to acquired Nephrogenic Diabetes Insipidus (NDI). This study examined whether combining Silymarin (SIL) with Vitamin C (Vit C) enhances protection against lithium-induced nephrotoxicity in rats, comparing their individual antioxidant effects as well. Rats subjected to Li exposure were provided with a standard commercial diet supplemented with 80 mmol LiCl per kilogram for 28 days. Concurrently, SIL and Vit C were administered orally at dosages of 200 and 100 mg/kg body weight,

Central and nephrogenic diabetes insipidus: updates on diagnosis and management.

Diabetes insipidus (DI) is a rare endocrine disease involving antidiuretic hormone (ADH), encompassing both central and nephrogenic causes. Inability to respond to or produce ADH leads to inability of the kidneys to reabsorb water, resulting in hypotonic polyuria and, if lack of hydration, hypernatremia. DI cannot be cured and is an unfamiliar disease process to many clinicians. This diagnosis must be distinguished from primary polydipsia and other causes of hypotonic polyuria. The main branchpoints in pathophysiology depend on the level of ADH pathology: the brain or the kidneys. Prompt diagn
