Clinical and genetic analysis of X-linked nephrogenic diabetes insipidus caused by a novel AVPR2 mutation (NM_000054.6:exon3:c.245G>A (p.Cys82Tyr)) in a Chinese boy.

X-linked nephrogenic diabetes insipidus (X-NDI) is a rare congenital disease caused by inactivating mutations of the vasopressin type-2 receptor (AVPR2), characterized by impaired renal concentrating ability, dramatic polyuria, polydipsia and risk of dehydration. This study aims to elucidate the pathogenic mechanisms associated with a novel variant in the AVPR2 gene, which has been implicated in X-NDI. Whole exome sequencing (WES) was employed to identify genetic variants, complemented by bioinformatic analyses to predict the functional impact of these mutations. A male patient, aged 11.5 year

Nephrogenic Diabetes Insipidus: Three Decades of Clinical Reality.

Introduction Nephrogenic diabetes insipidus (NDI) is a rare condition caused by renal resistance to the action of antidiuretic hormone (ADH) at the level of the distal tubule, resulting in impaired urinary concentration and consequent polyuria. NDI may be hereditary, most commonly X-linked due to AVPR2 gene mutations, or acquired. Objective To characterize the clinical features, management strategies, and outcomes of patients with NDI followed at a tertiary pediatric nephrology center. Methods A retrospective observational study was conducted, including pediatric patients diagnosed with NDI be

Lithium-induced kidney injury and fibrosis: A versatile model to explore cellular pathways of injury and repair.

Lithium-induced kidney injury is commonly associated with the development of nephrogenic diabetes insipidus. Longer term lithium exposure is associated with the development of chronic interstitial fibrosis. The mechanisms of lithium-induced kidney injury are multifaceted, affecting many intracellular cell signaling pathways associated with cell cycle regulation, cell proliferation, and subsequent increased extracellular matrix formation and interstitial fibrosis. Amiloride has been demonstrated to have multiple pleiotropic actions, independent of its competitive antagonism of ENaC, in reducing

A small molecule stabilizer rescues the surface expression of nearly all missense variants in a GPCR.

Reduced protein abundance is the most frequent mechanism by which rare missense variants cause disease. A promising therapeutic avenue for treating reduced abundance variants is pharmacological chaperones (PCs, also known as correctors or stabilizers), small molecules that bind to and stabilize target proteins. PCs have been approved as clinical treatments for specific variants, but protein energetics suggest their effects might be much more general. To comprehensively assess PC efficacy for variation in a given protein, it is necessary to first assign the molecular mechanism explaining all pa

Novel AVPR2 mutations in congenital nephrogenic diabetes insipidus: clinical characteristics and genetic analysis.

OBJECTIVES: Congenital nephrogenic diabetes insipidus (NDI) is a rare hereditary disorder caused by mutations in two critical genes: arginine vasopressin receptor 2 (AVPR2) and aquaporin 2 genes (AQP2). Mutations in AVPR2 gene, which are predominantly X-linked, account for a significant proportion of cases, particularly in men. Nevertheless, research on this condition in the western Chinese population remains limited. METHODS: Eleven participants from nine families with NDI were screened for AVPR2 mutations. Their clinical features were documented, and genotype-phenotype associations were inv

Lysine acetylation of aquaporin-3 does not improve lithium-induced nephrogenic diabetes insipidus.

Aquaporin-3 (AQP3) is expressed in the basolateral membrane of the renal principal cell, contributing to vasopressin-mediated water reabsorption and urine concentration. We reported that post-translational acetylation of lysine 282 of AQP3 promotes water permeability. In this study, we hypothesized that AQP3 acetylation may improve polyuria in a mouse model of lithium-induced nephrogenic diabetes insipidus (Li-NDI). Wild type, AQP3 acetylation (K282Q), and deacetylation (K282R) mimetic mice were fed a lithium-containing diet or a control diet for 14 days. Body masses and spot urines were colle

Nephrogenic Diabetes Insipidus Affecting Three Males in Two Generations-Case Report and Review of the Literature.

Background: Nephrogenic diabetes insipidus (NDI) is defined as the inability of the kidney to concentrate urine owing to the insensitivity of the distal nephron to the antidiuretic hormone, arginine vasopressin. NDI is a heterogeneous rare autosomal dominant or X-linked disease. Objective: We present a family with nephrogenic diabetes affecting three males in two generations. Methods: We report two boys with NDI: a 4-month-old infant who was treated for fever, vomiting, and failure to thrive, and his 10-year-old uncle (the mother's brother), who was admitted concurrently for consuming 11 L of

Functional characterization and cAMP-mediated rescue of a novel truncating AVPR2 mutation causing nephrogenic diabetes insipidus.

Vasopressin plays a central endocrine role in water homeostasis by activating the arginine vasopressin receptor 2 (AVPR2) receptor in renal collecting duct cells. Mutations in AVPR2 are a leading cause of X-linked nephrogenic diabetes insipidus (NDI), a disorder marked by renal insensitivity to vasopressin, leading to polyuria, polydipsia, and hypernatremia. We identified a novel truncating AVPR2 mutation (c.570dup; D191*) in a pediatric patient with NDI and investigated its molecular and functional consequences using a renal epithelial cell model. The D191* mutant exhibited marked reduction i

Failure to thrive in children with tubulopathies.

Failure to thrive (FTT) is a common clinical concern in pediatric patients with tubulopathies. Defective tubular handling of solutes, electrolytes, and acid-base balance contributes to impaired growth through both renal and systemic mechanisms. This educational review summarizes current evidence on the pathophysiological mechanisms, clinical presentation and management of FTT in pediatric tubulopathies.

Severe lithium-induced nephrogenic diabetes insipidus: The diuresis paradox.

We report a case of profound nephrogenic diabetes insipidus (NDI) in which renal resistance to antidiuretic hormone results in dilute polyuria despite normal circulating concentrations. A 28-year-old man with bipolar mood disorder presented to his local clinic with symptoms suggestive of lithium toxicity. Plasma lithium concentrations and thyroid-stimulating hormone (TSH) were taken, but results were not acted upon. One week later, he presented obtunded, severely dehydrated and in renal failure. His plasma lithium concentration was 4.3 mmol/L (toxic threshold >1.5 mmol/L) and TSH >100 mIU/L. A

Endocrine and Metabolic Challenges of Long-Term Lithium Therapy: Diagnostic and Management Considerations.

Lithium is a key treatment for bipolar disorder but is associated with significant endocrine and metabolic side effects, particularly lithium-induced hyperparathyroidism (LIH). Characterized by hypercalcemia and inappropriately normal or elevated parathyroid hormone (PTH) levels, LIH can lead to skeletal complications such as osteoporosis. Managing this condition is particularly challenging in psychiatric patients due to adherence issues with follow-up care. We describe the case of a 56-year-old Caucasian female patient with bipolar disorder on long-term lithium therapy (16 years), presenting

An Unusual Case of Lithium-Induced Nephrogenic Diabetes Insipidus.

We report a case of a 53-year-old woman with bipolar affective disorder on long-term lithium therapy who presented with confusion, acute right lower quadrant abdominal pain, fever, vomiting, and increased urinary frequency, with suspected urinary sepsis. Initial assessment revealed very high C-reactive protein (CRP), severe hyperglycemia, and acute kidney injury (AKI Stage 3). She was treated for urinary sepsis as computed tomography (CT) imaging demonstrated bilateral pyelonephritis, and Escherichia coli was isolated from blood cultures. Lithium was withheld initially due to renal impairment.

A rare case report on diabetes insipidus following living donor liver transplantation in patient with alcoholic liver disease.

A rare occurrence of nephrogenic diabetes insipidus (DI) following liver transplantation despite elevated plasma antidiuretic hormone levels and normal brain imaging findings is of interest. On the sixth day post-transplantation, the patient developed polyuria, a hallmark symptom of DI, prompts a comprehensive differential diagnosis. Desmopressin therapy was initiated, leading to rapid symptom resolution and full recovery. This case underscores the diagnostic challenges of DI in liver transplant recipients, particularly in differentiating between central and nephrogenic forms. Conventional dia

Copeptin in the diagnosis and management of renal tubular disorders.

Copeptin, a stable glycopeptide derived from the precursor of arginine vasopressin (AVP), has emerged as a valuable surrogate biomarker for AVP due to its stability and ease of measurement. This narrative review explores the physiological role of copeptin, its utility as a diagnostic and prognostic biomarker in different kidney diseases, and its clinical relevance in renal tubular disorders. The clinical application of copeptin as a diagnostic biomarker is best established in the differential diagnosis of polyuria-polydipsia syndrome (PPS), distinguishing nephrogenic diabetes insipidus (NDI) f

ENaC inhibitors for the management of lithium related polyuria: a systematic review.

BACKGROUND: By entering collecting duct principal cells via the epithelial sodium channel (ENaC), lithium is capable of inducing vasopressin insensitivity, resulting in excessive urine production, nephrogenic diabetes insipidus (NDI) and potential for other long-term forms of renal dysfunction. ENaC inhibitors (ENaC-I) such as amiloride have been shown in animal models to minimise this adverse effect, and while ENaC-I are often considered an effective strategy, the literature on ENaC-I for lithium-related polyuria has not yet been synthesised despite the importance of this topic.
