Since 1960, 17 patients with von Hippel-Lindau disease have had cerebellar hemangioblastomas removed at the Victoria General Hospital, Halifax, Nova Scotia. The 4 most recent patients have had renal arteriography as a part of their investigation. Though 3 of these had negative urography, all 4 had positive findings on arteriography. In 46 patients of the chronic hemodialysis program blood pressure regulation was studied according to variations of sodium and fluid balance. A strong relationship was observed between blood pressure and the amount of exchangeable sodium. The effects of acute administration of 25 hydroxycholecalciferol on bicarbonate reabsorption was studied in dogs. 25 hydroxycholecalciferol infused into the renal artery at a dose of 0.2 microgram/kg/hour led to a small but significant bilateral increase in bicarbonate reabsorption in intact dogs. 25 hydroxycholecalciferol had no effect on bicarbonate reabsorption in thyroparathyroidectomized dogs. The phosphaturia of bicarbonate administration was not altered by 25 hydroxycholecalciferol administration.

Eighty-three per cent of the affected members of a family with Hippel-Lindau syndrome had hypernephromas and cortical cysts of the kidney. A newborn female infant with marked bilateral renal enlargement resulting from an absolute increase in renal parenchymal mass is described. The number of renal lobes was increased and there was persistent subcapsular glomerulogenesis. In addition, the lungs manifested delayed maturation and contained multiple small cysts. Although the renal growth disturbance resembled that sometimes found in hemihypertrophy and the Beckwith syndrome, the associated pulmonary lesions and the absence of other anomalies make this patient distinctive.

Renal perfusion, filtration rate, and excretion can be evaluated with technetium-99m diethylenetriamine pentaactate (Tc-DTPA) using immediate and delayed imaging. A method for antegrade pyelography and pyelonephrostomy is described using a long, thin needle with a shorter, wider Seldinger cannula. Puncture with a thin needle reduces the risk during antegrade demonstration of the upper urinary tract and can also be used for puncturing collecting systems which are only slightly dilated. In a 2 year period five patients developed pathologically proved ischemic bowel disease (IBD) following either renal transplantation or bilateral nephrectomy in preparations for transplantation. This entity accounted for 42% of all major gastrointestinal complications in this transplant unit.
