Primary small cell neuroendocrine carcinoma of the tonsil combined with squamous cell carcinoma of the hypopharynx: a case report.

BACKGROUND: Primary small cell neuroendocrine carcinoma (SCNEC) of the tonsils is extremely rare and is often misdiagnosed as squamous cell carcinoma. Cases coexisting with hypopharyngeal squamous cell carcinoma (HPSCC) have not been reported. The presence of concurrent aortic arch aneurysm further complicates treatment. CASE DESCRIPTION: A 71-year-old male presented with pharyngeal pain lasting over one month. Imaging and pathological examinations confirmed right tonsillar SCNEC (with approximately 80% Ki-67 positivity) and left HPSCC, along with concurrent aortic arch aneurysm and stage III

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

Temporary Left Ventricular Unloading With Impella as a Bridge to Emergency Surgery for Acute Mitral Bioprosthetic Failure.

Structural valve deterioration usually declares itself gradually, but a bioprosthetic leaflet that tears away from a stent post can produce catastrophic regurgitation without warning. In the mitral position, the left atrium has not had time to become compliant, and the resulting rise in pulmonary capillary pressure can flood the lungs within hours. A 68-year-old woman who had undergone mitral valve replacement with a bovine pericardial bioprosthesis 13 years earlier developed sudden dyspnea. Her oxygen saturation was 58%, the chest radiograph showed almost complete bilateral opacification, and

A Case of Eosinophilic Granulomatosis With Polyangiitis Presenting With Conjunctival Lesions During Treatment With IL-5 Inhibitors.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic rheumatic disease characterized by necrotizing vasculitis in the small vessels with eosinophilic inflammation. EGPA involves multiple organs, such as the heart, lungs, kidneys, gastrointestinal tract, nerves, and ophthalmic organs. Ophthalmic involvement includes conjunctival nodules, orbital myositis, orbital inflammatory syndrome, dacryoadenitis, and cranial nerve palsy. Notably, conjunctival nodules in patients with EGPA are very rare lesions but may present with atypical eyelid swelling. Herein, we present a case of EGPA wi

Uremic Optic Neuropathy as the Initial Manifestation of Advanced Chronic Kidney Disease.

Uremic optic neuropathy (UON) is a rare but potentially reversible complication of advanced kidney failure characterized by optic disc edema and visual decline that improves with dialysis. We report the case of a 45-year-old man with no prior diagnosis of chronic kidney disease who presented with 3 months of progressive bilateral blurry vision, eye pain, and exertional fatigue. Ophthalmologic examination revealed bilateral optic disc edema with reduced visual acuity. Laboratory evaluation showed profound azotemia (blood urea nitrogen 182.9 mg/dL; creatinine 29.5 mg/dL), hyperkalemia, anemia, m

Back-Table Reduction of a Deceased Adult Donor Kidney for Transplantation Into a Small Child: A Six-Year Follow Up.

BACKGROUND: Transplantation of adult-sized kidneys into very small pediatric recipients can be limited by pronounced donor-recipient size mismatch. Although intraperitoneal placement is often considered, extraperitoneal implantation may reduce abdominal morbidity, provided that the graft can be accommodated safely. CASE PRESENTATION: We report transplantation of a left kidney from a 46-year-old deceased adult donor into a 15-month-old boy weighing 8.5 kg with congenital nephrotic syndrome and previous bilateral nephrectomy. The 14-cm graft had three arteries of comparable caliber. Because the

Bilateral Renal Infarction Associated With Left Renal Artery Thrombosis and Predominant Left Renal Infarction: A Case Report.

Although uncommon, bilateral renal infarction and renal artery thrombosis should be considered a potential cause of acute kidney injury. Its nonspecific presentation frequently contributes to delayed diagnosis, thereby compromising timely intervention and increasing the risk of irreversible injury. While embolic and thrombotic mechanisms are the predominant etiologies, idiopathic renal artery thrombosis is rare. We report the case of a 42-year-old man who presented with sudden-onset left flank pain radiating to the left upper quadrant, associated with nausea and abdominal discomfort. He had a

A novel homozygous splicing variant in FRA10AC1: further delineation of the phenotype.

Biallelic variants in FRA10AC1, encoding a component of the spliceosomal C complex that is crucial for functional mRNA processing, have been recently associated with a neurodevelopmental disorder characterized by developmental delay, variable dysmorphic facies, growth retardation, and corpus callosum abnormalities. Skeletal and congenital heart defects were observed in some patients. To date, only 10 patients from 6 unrelated families with FRA10AC1 variants have been reported in the literature. Herein, we describe a new patient harboring a loss-of-function variant in FRA10AC1. Our patient pres

Ultrasound-Based Kidney Stone Classification Using Kronecker Self-Organizing Map Forward Harmonic Network.

Kidney stone disease is a prevalent urological disorder that can result in severe pain, obstruction, and long-term complications if not detected and managed promptly. Traditional diagnostic approaches, particularly those relying on manual assessment of ultrasound images, often suffer from limitations such as subjective interpretation, dependency on radiologist expertise, and challenges in identifying small or complex stones. These constraints can lead to diagnostic delays and inconsistencies, especially in time-sensitive or resource-limited clinical settings. Therefore, the need for an intelli

Long-term efficacy and safety of first- and second-generation baroreflex activation devices in refractory hypertension: a single center retrospective analysis.

BACKGROUND: Baroreflex activation therapy (BAT) has emerged as a device-based therapy for resistant hypertension. Comparative evidence between first-generation (Rheos®) and second-generation (Barostim Neo™) BAT systems remains limited. METHODS: We retrospectively analysed all patients who underwent BAT implantation between 2006 and 2015 (n = 54) at our centre. The primary outcome was the change in 24h ambulatory blood pressure monitoring (ABPM) between device generations. Secondary outcomes included adverse events, major adverse cardiovascular events (MACE), kidney function, and device-relate

Occurrence and Clinical Outcomes of Acute Kidney Injury (AKI) Secondary to Acute Pyelonephritis in Hospitalized Patients at Dubai Health Facilities: A Retrospective Study.

Acute Kidney Injury (AKI) is a rare but significant complication associated with acute pyelonephritis, which, if left untreated, can cause severe renal impairment, thereby affecting renal structure and function. Upon reviewing the literature, we found that there are limited studies published on the outcomes of acute pyelonephritis. Our study, therefore, aims to evaluate the incidence and clinical outcomes of AKI in patients hospitalized with acute pyelonephritis at tertiary care hospitals under the 'Dubai Health' Institution.  We reviewed the medical health records of patients, noting details

Incidental crossed fused renal ectopia in a low-resource setting: Lessons from Ghana on diagnostic accuracy and imaging practice.

Crossed fused renal ectopia (CFRE) is a rare congenital renal anomaly in which both kidneys are fused and located on the same side of the midline, often remaining clinically silent but carrying important implications because of aberrant vascular and ureteric anatomy. We report an incidental finding of CFRE in a 55-year-old Ghanaian woman who underwent routine abdominal ultrasound, which demonstrated bilaterally empty renal fossae and a lobulated reniform structure in the right iliac region. The mass showed preserved corticomedullary differentiation with a small cortical cyst, and subsequent co

Renal cell carcinoma in bilateral fused pelvic kidneys: hybrid radical and nephron-sparing surgical management.

Renal malignancy in ectopic fused pelvic kidneys is exceptionally rare, with only a small number of cases described globally. We report a man in his early 50s with bilateral fused pelvic kidneys containing a large renal cell carcinoma extending across both renal moieties. Preoperative CT angiography demonstrated complex aberrant vasculature originating from the aortic bifurcation. Following selective arterial embolisation of the tumour-feeding vessels, the patient underwent radical nephrectomy of the left renal moiety and nephron-sparing partial nephrectomy of the right kidney.Despite the exte

Townes-Brocks Syndrome With Consistent Renal Hypodysplasia and Variable Extrarenal Features Across Three Generations of Serbian Family.

BACKGROUND: Townes-Brocks syndrome (TBS) is a rare autosomal dominant disorder caused by pathogenic variants in SALL1, classically defined by dysplastic ears, anorectal malformations, and thumb anomalies. Renal involvement is a major determinant of prognosis. RESULTS: We report a Serbian family with multiple members with TBS and molecular confirmation of a SALL1 familial nonsense variant (c.1509C>A, p. Tyr503Ter). Detailed phenotypic data were obtained from paediatric and adult medical records. Seven affected patients across three generations exhibited a remarkably consistent renal phenotype

Not All Leg Swelling Is Volume Overload: Point-of-Care Ultrasound and Sirolimus-Associated Lymphedema After Combined Heart-Kidney Transplant.

A 65-year-old man, one year post combined heart-kidney transplant, presented with acute kidney injury, dyspnea, and progressive leg edema unresponsive to escalating diuretics. Despite suspicion for volume overload, multi-organ point-of-care ultrasound (POCUS) revealed bilateral A-lines without B-lines or effusions, a small collapsible inferior vena cava, and preserved biventricular function, all inconsistent with cardiogenic congestion. The edema was attributed to sirolimus-associated lymphedema. Diuretic cessation and transition from sirolimus to everolimus improved renal function. This case

Atypical Abdominal Ulcer Relapse of Granulomatosis With Polyangiitis Following Renal Transplantation With Bilateral Papillary Renal Cell Carcinoma: A Case Report.

Granulomatosis with polyangiitis (GPA) is a necrotizing small- to medium-vessel vasculitis primarily affecting the respiratory tract and kidneys, with cutaneous involvement recognized but less commonly presenting as deep ulcerative lesions. GPA may progress to end-stage renal disease (ESRD), sometimes requiring renal transplantation, while long-term immunosuppression is associated with an increased risk of malignancy, including renal cell carcinoma (RCC). We present a 30-year-old man diagnosed with GPA at age 19 after presenting with hemoptysis and renal failure, with positive cytoplasmic anti

Calibration and prediction of results after failed injection in SPECT renal dynamic imaging.

To predict bilateral renal curves and glomerular filtration rate (GFR) following unsuccessful tracer injection in SPECT renal dynamic imaging using a Transformer model, thereby obviating the need for repeat examinations. We retrospectively studied patients who underwent repeat imaging post-extravasation (2015-2025). Patient height, weight, serum creatinine, urea, uric acid, and renal dynamic curves were used as inputs to develop a Transformer deep learning model for calibrating curves and predicting GFR. Following injection extravasation during SPECT renal dynamic imaging, the proposed model a

A novel regional anesthesia technique for retrograde intrarenal surgery in high-risk patients: a prospective observational study.

BACKGROUND: Retrograde intrarenal surgery (RIRS) is a minimally invasive treatment for upper urinary tract stones. However, anesthesia management in high-risk patients with significant comorbidities remains challenging, as general anesthesia poses elevated cardiopulmonary risks and spinal anesthesia is often contraindicated. This study aims to describe a novel regional anesthesia strategy combining ultrasound-guided quadratus lumborum block at the lateral supra-arcuate ligament (QLB-LSAL) with an anterior approach of sacral plexus block (SPB) as a potential alternative for RIRS in this vulnera

Sophocarpine Alleviates Renal Ischemia-Reperfusion Injury by Mitigating Oxidative Stress and Mitochondrial Dysfunction via the SIRT1/PGC-1α Axis.

Background/Objectives: Renal ischemia-reperfusion injury (IRI) is a major cause of acute kidney injury and delayed graft function after kidney transplantation. Oxidative stress, mitochondrial dysfunction, and tubular epithelial cell apoptosis are central events in renal IRI. Sophocarpine (SOP), a quinolizidine alkaloid derived from Sophora species, has reported antioxidant and anti-apoptotic activities, but its effects in renal IRI remain unclear. This study investigated the role and function of SOP in renal IRI. Methods: A bilateral renal IRI mouse model and a hypoxia/reoxygenation (H/R) mode
