Complication rates after intraoperative tranexamic acid use in total joint arthroplasty patients with a history of solid organ transplantation: A retrospective cohort study.

BACKGROUND: Solid organ transplant (SOT) recipients undergoing total joint arthroplasty (TJA) may face elevated perioperative risk because of chronic immunosuppression, medical comorbidity, and organ-specific physiologic considerations. Although intravenous tranexamic acid (TXA) is widely used during TJA to reduce perioperative blood loss, evidence regarding outcomes among SOT recipients receiving TXA remains limited. METHODS: A retrospective review was performed of 29,240 consecutive primary TJA patients who all received intraoperative intravenous TXA between June 2011 and March 2025. Patien

[From sudden hearing loss to an inflammatory systemic disease].

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides are a group of rare autoimmune disorders characterized by necrotizing inflammation of small- to medium-sized blood vessels. They typically involve the upper and lower respiratory tracts and frequently the kidneys. However, they may also present with otologic manifestations, which can occasionally occur as an early or even the sole clinical symptom. The following case report outlines the course of the disease in a patient in whom bilateral hearing loss ultimately emerged as the leading manifestation of ANCA-associated vasculiti

Prenatal ultrasound diagnosis of sirenomelia: A case report.

Sirenomelia is an extremely rare and almost invariably lethal congenital malformation characterized by variable fusion of the lower limbs and severe associated visceral anomalies. We report a case prenatally diagnosed at 20 weeks of gestation in a 25-year-old nondiabetic, nonconsanguineous woman referred for suspected polymalformative fetal syndrome. Prenatal ultrasound demonstrated severe oligohydramnios, bilateral small hyperechoic kidneys suggestive of renal dysplasia, fused lower limbs, absent external genitalia, and a single umbilical artery, findings strongly supporting the diagnosis of

Beyond Chronic Rhinosinusitis: Multisystem Granulomatosis With Polyangiitis Presenting With Cavitary Pulmonary Nodules and Sinonasal Destruction.

Granulomatosis with polyangiitis (GPA) is a rare necrotizing vasculitis of small- to medium-sized vessels characterized by granulomatous inflammation, most commonly involving the upper respiratory tract, lungs, and kidneys. Early manifestations are frequently non-specific and may resemble chronic allergic or infectious rhinosinusitis, delaying recognition until destructive sinonasal, pulmonary, renal, neurologic, or cutaneous involvement emerges. Cutaneous vasculitis and cavitary pulmonary nodules further complicate the diagnostic process by raising concern for infection or malignancy.  We rep

Unexpected aggressiveness in VHL-associated clear cell renal cell carcinoma: insights from two patients highlighting biological heterogeneity.

Von Hippel-Lindau (VHL) disease is a hereditary cancer syndrome predisposing to renal cell carcinoma (RCC), typically considered more indolent than sporadic RCC. The '3 cm rule' has guided nephron-sparing interventions, as tumours smaller than 3 cm rarely metastasise. However, accumulating evidence suggests this paradigm is not universal, with some VHL-associated RCCs demonstrating aggressive biological behaviour despite their small size. We report two cases of aggressive clear cell RCC (ccRCC) in patients with suspected VHL. The first case involved a middle-aged male presenting with bilateral

Early-onset kidney failure in a girl with autosomal dominant tubulointerstitial kidney disease due to a de novo UMOD variant.

Autosomal dominant tubulointerstitial kidney disease (ADTKD) is characterized by renal tubular and interstitial abnormalities and slow progressive loss of kidney function. Patients with ADTKD rarely progress to kidney failure in early childhood. A 7-year-old Japanese girl was admitted to the hospital due to afebrile seizures and was later diagnosed with Panayiotopoulos syndrome. Blood examinations showed that she had a serum creatinine level of 1.57 mg/dL (Cr-eGFR 28 mL/min/1.73 m²), consistent with chronic kidney disease stage 4. Ultrasonography showed bilateral small to normal-sized kidneys,

Severe Chronic Kidney Disease Presenting as Asymptomatic Normocytic Anemia in a Child.

Anemia is a common finding in pediatric practice and is most often attributed to nutritional deficiency; however, it may also represent the earliest manifestation of underlying systemic disease. We describe the case of a six-year-old girl who presented for a routine well-child visit and was found to have severe normocytic, normochromic anemia despite appearing clinically asymptomatic. Subsequent laboratory evaluation revealed profound renal dysfunction with metabolic acidosis and electrolyte abnormalities. Renal imaging demonstrated bilaterally small kidneys without hydronephrosis, consistent

Role of Lipid Signaling by Platelet-Activating Factor Receptor in Tubular Epithelial Cells in Acute Kidney Injury-to-CKD Transition.

KEY POINTS: Platelet-activating factor receptor contributed to renal tubular epithelial cells G2/M arrest through suppressing mouse double minute 2-mediated p53 ubiquitin degradation in AKI-to-CKD transition. Phosphatidylethanolamine (18:0/18:1) served as a novel endogenous ligand of platelet-activating factor receptor, inducing the downstream signaling. WAY-639497, a small-molecule platelet-activating factor receptor antagonist identified by virtual screening, mitigated renal tubular epithelial cells G2/M arrest and AKI-to-CKD transition. BACKGROUND: AKI represents a critical clinical compli

[Update renal cell carcinoma : Guideline update on small renal masses].

Small renal masses (kidney tumors < 4 cm) are benign in up to 30% of cases and often exhibit slow growth, yet they are still frequently primarily managed by surgery. A tumor biopsy should be performed before initiating active surveillance. As up to 8% of renal tumors could have a hereditary component, risk factors such as a diagnosis before the age of 47 years old, bilateral or multifocal lesions and syndrome-associated extrarenal manifestations, should be systematically assessed and clarified by a genetic evaluation. In patients with clear cell renal cell carcinoma and intermediate-high or hi

Multifocal Low-Grade Oncocytic Tumor of the Kidney Associated With Germline TSC1 Alteration: Case Report and Literature Review.

Low-grade oncocytic tumor (LOT) of the kidney is a rare, indolent neoplasm within the spectrum of eosinophilic renal tumors. It typically presents as a small, solitary mass and is associated with molecular alterations in TSC1, TSC2, or MTOR. We describe a 72-year-old male with a history of pancreatic neuroendocrine tumor, gastrointestinal stromal tumor, and prostate cancer who presented with multiple bilateral renal masses. Partial nephrectomy revealed two distinct tumors: one consistent with LOT and the other with a sclerosing angiomyolipoma (AML). The LOT consisted of oncocytic cells with ro

Case Report: Successful management of bilateral ureteral obstruction following repair of iatrogenic urethral ligation and transection during a canine cryptorchidectomy.

An 8-month-old male English Bulldog was presented to an academic referral center following iatrogenic urethral transection during an elective unilateral cryptorchidectomy. A contrast cystourethrogram demonstrated contrast leakage from the pre-prostatic urethra. An exploratory laparotomy confirmed ligation and transection of the pre-prostatic urethra, and a urethral anastomosis was performed. Postoperatively, the patient developed progressive azotemia, and abdominal imaging revealed bilateral hydroureter and hydronephrosis consistent with bilateral ureteral obstruction. No apparent cause of mec

Congenital Anomalies of the kidney and urinary tract: pediatric origins and adult outcomes.

Congenital anomalies of the kidney and urinary tract (CAKUT) are a heterogeneous group of structural defects that arise from disordered nephrogenesis. CAKUT accounts for 20%-30% of all prenatal anomalies, with a pooled birth prevalence of 3.83 per 1000. The pathogenesis reflects disrupted ureteric bud-metanephric mesenchyme signalling and early transcriptional programs. Genetic disorders underlie approximately one-fifth of cases, with monogenic causes (notably HNF1β and PAX2) and copy number variants being particularly prevalent in syndromic presentations. The broad phenotypic expression under

N-acetylcysteine reduces serum creatinine, blood urea nitrogen, symmetric dimethylarginine and urine protein to creatinine ratio in cats with chronic kidney disease: a double-blind, placebo-controlled clinical trial.

BACKGROUND: Oxidative stress is considered a significant contributing factor of chronic kidney disease (CKD). To date, there is a paucity of clinical data in the literature regarding the effect of N-Acetylcysteine (NAC) in cats with naturally developing CKD. The aim of the study is to evaluate whether the addition of NAC in the treatment of cats with acute exacerbations of CKD could improve kidney function biomarkers over the use of intravenous fluid therapy alone. METHODS: A total of 50 client-owned cats were included in the study. The inclusion criteria comprised cats previously diagnosed w

Coexistence of PYCR1-related cutis laxa and bilateral grade V vesicoureteral reflux with reflux nephropathy: A case report.

PYCR1-related cutis laxa (autosomal recessive cutis laxa type IIB) is a rare inherited connective tissue disorder characterized by loose, inelastic skin, and variable multisystem involvement. While systemic and neurological manifestations have been well described in the literature, data on renal and urinary tract findings remain sparse. We report a 6-year-old girl with genetically confirmed PYCR1-related cutis laxa who presented with recurrent febrile urinary tract infections. Workup revealed bilateral grade V vesicoureteral reflux alongside a small, atrophic left kidney with cortical defects

A case of successful surgical management of colonic duplication with pygomelia in a 2-month-old dog.

A 2-month-old crossbreed female dog was presented for investigation of multiple congenital malformations, including pygomelia and duplication of the anus. Computed tomography revealed a duplicated colon, a duplicated pelvis with associated supernumerary hind limbs, a perineal hernia and abnormalities of the left kidney and ureter. During surgical exploration, a third ovary with its associated uterine horn was also identified. Surgical management consisted of excision of the duplicated colon and anus, disarticulation of the supernumerary hind limbs and correction of the perineal hernia. Postope

A feline case of suspected partial persistent urogenital sinus successfully managed by urethral plication alone.

CASE SUMMARY: A 5-month-old intact female Russian Blue cat presented with continuous urinary incontinence and intermittent haematuria. Retrograde vaginourethrography and contrast-enhanced CT revealed absence of distinct urethral and vaginal structures, with a single tubular lumen extending from the bladder to the vulva. Cystoscopy and exploratory laparotomy confirmed the absence of the uterine body and vagina, while bilateral uterine horns were present and both ureters inserted normally into the urinary bladder. These findings suggested partial persistent urogenital sinus (PPUS) rather than a

Minimally invasive treatment of recurrent varicose veins with thrombotic complications using EVLA: A case report.

We present a clinical case of a 59-year-old male who complained of varicose veins and a burning sensation in the right lower limb. His medical history included bilateral great saphenous vein (GSV) removal (2003), type 2 diabetes mellitus, stage 4 chronic kidney disease, gout, and arterial hypertension. Duplex ultrasound revealed varicothrombophlebitis of the small saphenous vein (SSV) and a floating thrombus in the popliteal vein. Initial treatment consisted of oral anticoagulation with rivaroxaban, followed by EVLA of the SSV and ultrasound-guided sclerotherapy of tributaries. After anticoagu

A Small Mass Causing a Big Problem: Severe Aortic Regurgitation Induced by a Papillary Fibroelastoma.

Papillary fibroelastoma (PFE) is a benign primary cardiac tumor arising from the endocardial surface, most commonly affecting the valvular endocardium. Although often incidental, PFEs may lead to serious complications, including systemic embolization, valvular obstruction, or regurgitation. Aortic valve fibroelastoma resulting in severe aortic insufficiency (AI) may be rare. A 77-year-old woman with a history of hypertension and chronic kidney disease presented with progressive dyspnea, orthopnea, and bilateral lower extremity edema. Postobstructive diuresis and its biochemical characteristics in children undergoing pyeloplasty for unilateral pelviureteric junction obstruction - a prospective observational study.

BACKGROUND: The characteristics of postobstructive diuresis (POD) following relief of unilateral ureteric obstruction (UUO) has been studied in animals, but there is scarcity of literature on translation of these findings in human pediatric patients with unilateral pelviureteric junction obstruction (PUJO) following pyeloplasty. OBJECTIVE: The primary objective was to assess the biochemical characteristics of the POD from the operated kidney. Our secondary objective was it's clinical implications and identifying predisposing factors. STUDY DESIGN: A prospective observational study was conduc
