Microneedle-based drug delivery for inflammatory and immune-mediated ocular diseases: current applications and future perspectives.

Inflammatory and immune-mediated ocular diseases encompass a spectrum of conditions that arise from dysregulated immune-inflammatory responses marked by a chronic clinical course, a propensity for relapse, and a tendency to cause significant ocular tissue damage, which can lead to profound impairment of visual function. Dysregulation of the balance between immune cell subpopulations is central to the pathogenesis of many of these diseases. At present, non-specific treatments such as glucocorticoids and immunosuppressants form the cornerstone of therapy for inflammatory and immune-mediated ocul

Nano formulations of quercetin for ocular delivery.

Quercetin (QUE), a bioactive flavonoid with potent antioxidant, anti‑inflammatory, and neuroprotective properties, has emerged as a promising therapeutic candidate for age‑related macular degeneration, diabetic retinopathy, cataracts, glaucoma, dry eye disease, and allergic conjunctivitis. Its therapeutic effects are primarily mediated through modulation of key molecular pathways, including direct scavenging of reactive oxygen species (ROS), activation of Nrf2‑dependent antioxidant responses, inhibition of NF‑κB and MAPK signaling, and downregulation of pro‑inflammatory cytokines and angiogeni

Presumed herpes simplex virus keratouveitis in a 2-year-old child: a case report.

BACKGROUND: Ocular herpes simplex virus (HSV) disease is uncommon in very young children and may be difficult to recognize because history and examination are limited and early manifestations can resemble bacterial conjunctivitis. Pediatric disease may involve multiple corneal layers, extend into the anterior chamber, recur, and threaten visual development. We describe presumed HSV keratouveitis in a 2-year-old child. CASE PRESENTATION: A 2-year-old girl presented with a two-month history of right-eye pain, photophobia, and tearing after an episode diagnosed elsewhere as HSV blepharoconjuncti

Ocular Immune Toxicity in Cancer Immunotherapy, From Immune Privilege Breakdown to Treatment Rechallenge: A Review.

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy but can disrupt ocular immune privilege and produce heterogeneous immune-related adverse events (Eye-irAEs). This structured narrative review with evidence mapping integrates clinical, epidemiologic, mechanistic, pharmacovigilance, and management data to define translational pathways from checkpoint blockade to ocular toxicity and treatment decisions. In a real-world cohort of 41,020 ICI-treated patients, ophthalmic irAEs occurred in 4.18% within 1 year, including dry eye disease in 2.00%, conjunctivitis in 0.87%, blepharitis

Emerging RNA-Based Therapeutic Strategies for Inflammatory Ophthalmic Disorders of the Anterior Segment: Mechanistic Insights and Translational Potential.

Anterior segment ocular inflammatory disorders, including conjunctivitis, keratitis, scleritis, dry eye disease, and anterior uveitis, represent a major cause of visual impairment worldwide. Conventional pharmacological therapies such as antihistamines, corticosteroids, and immunomodulators primarily provide symptomatic relief but are limited by their adverse effects, poor long-term efficacy, and lack of disease-modifying potential. In this review, RNA-based therapeutics are discussed in detail, emerging as promising and targeted approaches for modulating the molecular mechanisms underlying oc

Individualising biologic therapy for paediatric severe asthma: A comparative review of all five approved agents, real-world evidence, and a biomarker-guided clinical decision algorithm.

Severe asthma affects 5-10% of children with asthma yet accounts for disproportionate morbidity and healthcare utilisation. The recent expansion of biologic therapies-targeting IgE, IL-5, IL-4/13, TSLP, and IL-5 receptor α-has transformed management but introduced complexity into clinical decision-making. This review provides a comprehensive, comparative analysis of currently approved biologic agents for paediatric severe asthma, with emphasis on efficacy, safety, biomarker-guided selection, and practical implementation. We searched PubMed/MEDLINE (2015-2026) for randomised controlled trials,
