Identification of the Causative Pathogen in the 2023 Conjunctivitis Outbreak of Nepal Using Unbiased Metagenomic Next Generation Sequencing.

BACKGROUND: In mid-2023, Nepal experienced a significant outbreak of conjunctivitis, affecting over 60% of outpatients in eye hospitals and prompting school closures. The outbreak, peaking in August, predominantly impacted children and individuals with compromised immunity. Clinical manifestations included sudden-onset redness, foreign body sensation, watery discharge, and occasional lid swelling. Most cases exhibited acute haemorrhagic conjunctivitis, with management involving ocular lubricants, personal hygiene, and topical antibiotics. This case series from Himalaya Eye Hospital in Pokhara

Effects of fermented versus unfermented red cabbage on symptoms, immune response, inflammatory markers and the gut microbiome in young adults with allergic rhinoconjunctivitis: a randomised controlled trial protocol.

INTRODUCTION: Allergic rhinoconjunctivitis (ARC) is a highly prevalent immune-mediated condition associated with substantial symptom burden, impaired quality of life and increased healthcare use. Emerging evidence highlights the role of the gut microbiome in immune regulation and allergic disease. Fermented foods may contain live microbes (when unpasteurised or uncooked) and bioactive postbiotic metabolites that can modulate immune responses. Despite growing interest in dietary strategies targeting the microbiome, no randomised controlled trial has compared fermented versus unfermented red cab

A Rash Decision: Mycoplasma-Induced Mucositis in a Young Adult.

Mycoplasma pneumoniae-induced rash and mucositis (MIRM) is a distinct emerging clinical entity typically affecting adolescents and adults after a respiratory infection from Mycoplasma pneumoniae. Other known mucocutaneous complications include Stevens-Johnson syndrome, erythema multiforme, and toxic epidermal necrolysis. We present the case of a 24-year-old male patient who developed fever and cough followed by a rapidly evolving mucocutaneous syndrome characterized by oral mucositis, bilateral conjunctivitis, and vesiculobullous skin lesions affecting the palms, soles, and trunk with no evide

Comprehensive pharmacovigilance analysis of lebrikizumab: a real-world safety assessment based on the FDA Adverse Event Reporting System (FAERS) database.

BACKGROUND: Lebrikizumab is an interleukin-13 antagonist that has been approved for use in adults and pediatric patients aged 12 years and over who weigh at least 40 kg, for the treatment of moderate-to-severe atopic dermatitis (AD) that is inadequately controlled with topical prescription therapies, or for whom such therapies are not suitable. Due to the relatively short duration of existing clinical trials and the absence of real-world postmarketing evidence involving AD patients receiving lebrikizumab, it is crucial to comprehensively characterize its safety profile, including the potential

A mouse model of allergic conjunctivitis with mucosal immune-related gene and circRNA dysregulation.

Allergic conjunctivitis (AC) is an inflammatory ocular condition triggered by allergens like pollen. Although general allergic mechanisms are well characterized, the immune specificity of the ocular mucosa remains unclear. This study investigates immune-related mRNAs and circRNAs to uncover novel molecular pathways in AC pathogenesis. Using a ragweed pollen-induced murine AC model, conjunctival tissues were subjected to RNA sequencing. DESeq2-based differential expression analysis revealed dysregulated mRNAs and circRNAs, with significant enrichment in the IL-17 signaling pathway. Key IL-17-as

Mucus Fishing Syndrome: Case Series and a Narrative Review of Literature.

INTRODUCTION: Mucus fishing syndrome (MFS) is a self-induced mechanical ocular surface disorder characterized by compulsive removal of mucus strands from the conjunctival fornices. MFS is frequently underrecognized and misdiagnosed because of its poorly characterized pathophysiology and discreet behavioral trigger. We present a case series of MFS and review the current literature addressing its epidemiology, pathophysiology, psychiatric associations, and management. METHODS: We report two clinical cases and provide a comprehensive literature search across PubMed, MEDLINE (Ovid), and Google Sc

Non-Typeable Streptococcus pneumoniae and Pneumococcal Diseases: A Systematic Review and Meta-Analysis.

Non-typeable Streptococcus pneumoniae strains are not covered in the current pneumococcal conjugate vaccines (PCVs). As a result, their increasing prevalence in pneumococcal diseases following the introduction of PCVs poses a significant public health concern. This study provides a global summary of the epidemiology of non-typeable pneumococcal diseases by estimating their prevalence and antimicrobial resistance rates, and identifying the circulating sequence types. An extensive search was conducted in PubMed, Scopus and Web of Science for studies reporting pneumococcal diseases caused by non-

Gonococcal Dacryoadenitis: A Case Report.

INTRODUCTION: Neisseria gonorrhoeae (NG) causes sexually transmitted hyperacute bacterial conjunctivitis. Involvement of the lacrimal gland is exceedingly rare and has been previously reported in two case reports. CASE PRESENTATION: This report discusses a 71-year-old male who presented with a painful red left eye. There was notable swelling of the upper eyelid, which was confirmed on CT to be due to lacrimal gland inflammation. Clinical features of dacryoadenitis associated with scleritis were found on ophthalmologic assessment. Conjunctival swabs were sent for culture and returned positive

Reproducing an Edo Period Orbital Epithesis with Wood and Crystal.

In Japan, ophthalmic diseases, such as gonorrheal conjunctivitis, were widespread during the Edo period (1603-1868), leaving many patients without one or both eyes. Numerous records indicate that many professional denturists often crafted artificial eyes for such patients; however, no evidence of inserted prosthetic eyes has been found to date. The author aimed to recreate a wooden orbital epithesis using the same materials and methods as those used in the Edo period where boxwood was used for wooden plate dentures. The inner surface of the epithesis could be contaminated with mucus; therefore

Seasonal allergic rhinitis in children: diagnostic markers of extreme allergic phenotype formation.

BACKGROUND: Patients with severe allergic rhinitis may encounter an elevated risk of developing asthma and other associated allergic conditions. It is known that "allergic rhinitis" and "allergic rhinitis + asthma" are two clinically distinct phenotypes of allergic rhinitis. Consequently, the aim of our study was to establish the diagnostic criteria for the progression of the allergic phenotype in children with seasonal allergic rhinitis (SAR). METHODS: This cohort study included 47 patients aged between 6 to 17 years diagnosed with SAR, who were categorised into two groups: SAR/SAR with asso
