Levator ani muscle enthesis-related pain in patients with bladder pain syndrome: A retrospective cross-sectional study.

OBJECTIVE: Interstitial cystitis/bladder pain syndrome is a chronic pelvic pain condition with heterogenous mechanisms. Levator ani muscle (LAM) enthesis-related pain has been proposed as a musculoskeletal contributor to bladder pain symptoms. This study investigated whether LAM enthesis-related pain is independent of cystoscopic glomerulation in patients presenting with bladder pain symptoms. METHODS: This single-center retrospective study included 231 patients with painful bladder symptoms who underwent cystoscopy between January 2019 and September 2024. Patients were stratified according t

Chronic High-Dose Ketamine Fails to Induce a Cystitis-Like Phenotype in Mice Despite High Urinary Metabolite Levels.

AIMS: Ketamine‑induced cystitis (KIC) is a severe complication of chronic ketamine misuse, yet the underlying mechanisms remain poorly understood. Established rodent models have demonstrated variable changes in voiding behavior and bladder histology, without exploring the sensory mechanisms underlying altered voiding and pain at all. This study aimed to establish a murine model of chronic ketamine-induced cystitis to investigate changes in pain behavior and sensory signaling. METHODS: Male and female C57BL/6 J mice received intraperitoneal ketamine (100 mg/kg, n = 20) or saline (n = 20) for 1

Intestinal-Type Villous Adenoma of the Urinary Bladder Presenting as an Asymptomatic Diffuse Plaque-Like Trigonal Lesion: A Rare Case With Multimodal Clinicopathologic Correlation.

Villous adenoma of the urinary bladder is a rare, benign glandular neoplasm with intestinal differentiation. This neoplasm is frequently associated with chronic urinary irritation and intestinal metaplasia of the urothelium. Fewer than 50 cases have been documented, and limited information is available regarding its clinicoradiologic spectrum. A 51-year-old man with severe anemia was found to have multiple broad-based exophytic lesions of the bladder on ultrasound. There was no hematuria, mucosuria, or other lower urinary tract symptoms. A minimally enhancing, plaque-like lesion measuring appr

Bladder Urothelial Carcinoma Diagnosed by Perineal Cutaneous Biopsy: A Case Report.

Bladder cancer is the most common malignancy of the urinary tract, and urothelial carcinoma represents its most frequent histological subtype in Brazil. Diagnosis is conventionally established by direct biopsy of the urothelial mucosa obtained through cystoscopy with transurethral resection of the bladder tumor, which also allows sampling of the detrusor muscle for adequate staging. We report the case of a 71-year-old hypertensive man without a smoking history who initially presented with perineal induration, difficulty in defecation, and intermittent diarrhea, and was first investigated for s

Phenotyping Interstitial Cystitis/Bladder Pain Syndrome: Clinical and Mechanistic Evidence, Candidate Biomarkers, and Implications for Management.

Interstitial cystitis and bladder pain syndrome are clinically overlapping conditions that should be distinguished according to Hunner lesion status. IC with Hunner lesions is a distinct inflammatory bladder disease characterized by urothelial denudation, lymphoplasmacytic infiltration, adaptive immune activation, and progressive structural impairment. The intensity and distribution of these inflammatory and structural abnormalities may vary among patients with IC, but the underlying pathological pattern is comparatively coherent. BPS without Hunner lesions is substantially more heterogeneous.

Intravesical Allogeneic Platelet Concentrate for Refractory BK Virus-Associated Haemorrhagic Cystitis After Allogeneic Haematopoietic Stem Cell Transplantation.

Background/Objectives: BK virus-associated haemorrhagic cystitis (BKV-HC) is a severe complication of allogeneic haematopoietic stem cell transplantation (allo-HSCT) for which no standard treatment exists. Intravesical platelet-rich plasma has been proposed, but recipients are profoundly thrombocytopenic, and an autologous product cannot be obtained. We report the outcome of an intravesical allogeneic platelet concentrate (allo-PC) prepared from irradiated single-donor apheresis platelets in patients refractory to all available treatment. Methods: Single-centre retrospective series of 14 conse

How do I manage candiduria: practical algorithms and remaining conundrums in 2026.

BACKGROUND: Candiduria is frequently encountered in hospitalized patients and encompasses a wide spectrum of clinical scenarios, ranging from asymptomatic colonization or sample contamination to genuine urinary tract infections and, in rarer cases, invasive candidiasis. Despite its high prevalence, especially in urological and intensive care settings, its clinical relevance is often uncertain, leading to diagnostic uncertainty and inconsistent management, even among infectious disease specialists. OBJECTIVES: This review aims to provide a structured approach to the evaluation and management o

Screening Bacteriuria Using the Sysmex UF-5000 during Intravesical Bacillus Calmette-Guérin Therapy for Bladder Cancer.

Intravesical Bacillus Calmette-Guérin (BCG) therapy for bladder cancer requires screening for urinary tract infection during treatment, but conventional urinalysis cannot reliably distinguish bacteriuria from BCG-induced inflammatory cystitis. This study evaluated the ability of the Sysmex UF-5000 urine flow cytometer to detect significant bacteriuria and predict Gram stainability during intravesical BCG. This retrospective study included 145 patients who underwent intravesical BCG therapy after transurethral resection of bladder tumors (TURBT). Urine flow cytometry and cultures were performed

Evaluation of nanofibrillated cellulose as a urethral bulking agent in female dogs with acquired urethral sphincter mechanism incompetence: a prospective clinical study.

Urethral bulking agent (UBA) implantation is a minimally invasive, effective and safe alternative treatment for refractory urethral sphincter mechanism incompetence (USMI). However, repeated implantations are frequently needed to maintain continence. Therefore, a prospective interventional clinical study with eight client-owned neutered female dogs was designed to evaluate the clinical efficacy and 12-month outcome of nanofibrillated cellulose (NFC), a novel biomaterial as a UBA. Concurrent causes of urinary incontinence were excluded with clinical and neurological assessment, blood and urine

Single-Incision Plus One Robot-Assisted Abdominoperineal Resection Using the Hugo RAS System Through a Planned Stoma Site: A Novel Reduced-Port Robotic Surgery.

BACKGROUND: Reduced-port surgery has been developed to minimize abdominal wall trauma while maintaining the benefits of minimally invasive surgery. However, reduced-port robotic rectal surgery remains technically challenging due to interference of external arms. The modular design of the Hugo RAS system (Medtronic, Minneapolis, MN, USA) provides greater flexibility in arm positioning to minimize external arm interference without compromising instrument maneuverability1-4. METHODS: In abdominoperineal resection (APR), the incision for a permanent colostomy is inevitable. We report the first ca

[MALACOPLAKIA OF THE URINARY BLADDER: A CASE REPORT WITH DIAGNOSTIC DIFFICULTY].

We herein report a rare case of malacoplakia of the urinary bladder that demonstrated significant diagnostic difficulty. The patient was a 65-year-old woman with a history of breast cancer, dermatomyositis, steroid-induced diabetes mellitus, and cutaneous cryptococcosis. She presented with repeated episodes of dysuria and hematuria since the age of 62. Computed tomography (CT) revealed progressive bladder wall thickening, and she was therefore diagnosed with cystitis by her primary care physician and treated with antibiotics. Her symptoms worsened at 63 years of age, and further bladder wall t

Role of the urobiome in urinary tract infections: current knowledge and research prospects.

Urinary tract infections (UTIs), particularly cystitis, are among the most common bacterial infections worldwide, with a disproportionate burden on adult female individuals, who often have recurrence. The discovery of the urobiome, a low microbial biomass community in the urinary tract, has transformed our understanding of UTI pathogenesis and infection susceptibility. Experimental and omics approaches provide insights into the taxonomic and functional complexity of the urobiome, highlighting the importance of microbial metabolites, ecological interactions, and host-microbe dynamics. Deciphering Polyomavirus-Mediated Carcinogenesis in the Genitourinary Tract.

Human polyomaviruses, particularly BK polyomavirus (BKPyV) and JC polyomavirus (JCPyV), establish persistent infections within the genitourinary tract and undergo reactivation under conditions of immunosuppression. Although their clinical relevance is well established in transplant-associated nephropathy and haemorrhagic cystitis, accumulating molecular, pathological, and epidemiological evidence suggests that these viruses may also contribute to genitourinary carcinogenesis. This review examines the emerging role of BKPyV and JCPyV in urothelial, upper urinary tract, renal, and prostate malig
