Effects of a supplement associating Lactobacillus strains and proanthocyanidin-rich plant extracts against recurrent uncomplicated, urinary tract infections: A prospective, controlled study.

PURPOSE: Recurrent cystitis, particularly common in women, substantially diminishes patient quality of life and represents a major clinical practice and public health burden. Increasing development of resistance to antibiotics has encouraged the search for alternative treatments. The benefits of a food supplement associating two Lactobacillus strains with proanthocyanidin A-rich cranberry and cinnamon extracts were evaluated in 80 women with a history of cystitis recurrence. MATERIALS AND METHODS: Post-inclusion recurrence frequency was compared between women taking the investigated supplemen

[2024 scientific breakthroughs in ambulatory general internal medicine].

In this article, we present eight studies published in the past two years that are likely to influence general practice in 2025. The key messages highlight the importance of physical activity in reducing cardiovascular risk, the effectiveness of aripiprazole for treatment-resistant depression, and the positive impact of guidelines on antibiotic use for acute cystitis. Furthermore, teleconsultation is more effective when supported by a pre-existing therapeutic relationship, extended leave reduces physician burnout, electronic cigarettes aid smoking cessation, and AI improves the management of e

Single-Dose Gentamicin vs Standard Care for Treatment of Acute Uncomplicated Cystitis in Premenopausal Women: A Randomized Trial.

BackgroundUrinary tract infections (UTI) are a disease of serious impact on every country in the world. Growing resistance has decreased the efficacy of many antimicrobial options. Aminoglycosides, like gentamicin, have long been used to treat gram-negative bacteria including UTIs.ObjectivesThe goal of this study was to compare single dose gentamicin to standard oral seven-day treatment in the emergency department for uncomplicated cases of acute uncomplicated cystitis in premenopausal women.MethodsThis was a randomized, open-label clinical trial of women at least 18 years of age, that were pr

The Role of Stratified Cumulative Antibiograms in the (Choice of Appropriate Antibiotics in Urinary Tract Infection) Management of Urinary Tract Infections.

Urinary tract infections (UTIs) are one of the most common bacterial diseases both in communities and in hospitalized patients, and at the same, time they are one of the most common indications for the use of antibiotics. UTI guidelines are generally available nationally or internationally, but they do not address all aspects of UTI treatment for different patient cohorts, age, gender, or comorbidities. The aim of the study was to point out the importance of stratified cumulative antibiograms at the level of individual health care facilities and the significant differences between epidemiologi

A bladder-blood immune barrier constituted by suburothelial perivascular macrophages restrains uropathogen dissemination.

Urinary tract infections (UTIs) predominantly occur in the bladder and can potentially progress into life-threatening sepsis if uropathogens spread unconstrainedly into the bloodstream. Here, we identified a subset of suburothelial perivascular macrophages (suPVMs) in the bladder that exerted a pivotal barrier function to prevent systemic bacterial dissemination during acute cystitis. During the initial phase of uropathogenic Escherichia coli (UPEC) infection, suPVMs actively captured UPEC invading the laminal propria and maintained the integrity of inflamed vessels. They subsequently underwen

Living With and Managing Uncomplicated Urinary Tract Infection: Mixed Methods Analysis of Patient Insights From Social Media.

BACKGROUND: Uncomplicated urinary tract infections (uUTIs) affect more than half of women in their lifetime and can impact on quality of life. We analyzed social media posts discussing uUTIs to gather insights into the patient experience, including aspects of their disease management journey and associated opinions and concerns. OBJECTIVE: This study aims to gather patient experience insights by analyzing social media posts that discussed uUTI. METHODS: A search string ("urinary tract infection" [UTI] or "bladder infection" or "cystitis" or "UTI" not "interstitial cystitis") was used to iden

New evidence for content validity of the German version of the Acute Cystitis Symptom Score: cognitive interview study among patients and experts.

PURPOSE: The Acute Cystitis Symptom Score (ACSS) is a clinically validated instrument to assess symptoms and quality of life in women with uncomplicated urinary tract infections (uUTIs). A previous study examining the content validity of the German version of the ACSS has shown some methodological limitations when rated against the criteria for content validity of the COnsensus-based Standards for the Selection of Health Measurement INstruments (COSMIN) initiative. Extending the existing evidence, the present study aimed to assess the content validity of the German version of the ACSS accordin

A Reminder App to Optimize Bladder Filling During Radiotherapy for Patients With Prostate Cancer (REFILL-PAC): Protocol for a Prospective Trial.

BACKGROUND: Many patients with nonmetastatic prostate cancer receive radiotherapy, which may be associated with acute cystitis, particularly if the volume of the urinary bladder is small. Three studies showed bladder volumes <200 ml or <180 ml to be associated with increased urinary toxicity. Therefore, it is important to maintain bladder volumes greater than 200 ml during as many radiation fractions as possible. Several studies investigated drinking protocols, where patients were asked to drink a certain amount of water prior to radiotherapy sessions. This may require considerable discipline

Efficacy of lactoferrin in preventing recurrent urinary tract infections in pregnant Egyptian women: a randomized controlled trial.

Источник пометил публикацию как отозванную

BACKGROUND: Prevention of a very prevalent problem such as urinary tract infections (UTIs), is of utmost importance, particularly during pregnancy, in order to limit the irrational use of antibiotics. Lactoferrin (Lf) has proven in vivo and in vitro antibacterial actions, especially against Escherichia coli, which is the main causative organism of UTIs. The study question is, "Does the administration of Lf to pregnant women with a history of RUTIs reduce the incidence of new episodes of UTIs during pregnancy?". METHODS: This was a randomized controlled study over 6 months that started from Fe

Inhibition of HPSE/SDC-2 axis-induced epithelial-mesenchymal transition for treating IC/BPS.

Interstitial cystitis/bladder pain syndrome (IC/BPS) plagues patients and clinicians with its unclear etiology and pathogenesis, and ineffective treatments. Destruction of epithelial tissue and proliferation of interstitial tissue are typical pathological features of IC/BPS, in which epithelial-mesenchymal transition (EMT) may play an important role. Both the increased urination frequency observed in mice with acute cystitis induced by cyclophosphamide (CYP) and the disruption of the anti-leakage barrier in urothelial cells induced by LPS are associated with the occurrence of EMT. The expressi

Urinary Tract Infection Due to Raoultella planticola in a Healthy Adult: A Report of a Rare Case.

A 56-year-old immunocompetent woman presented with recurrent urinary tract infections (UTIs) over a two-year period, during which Raoultella planticola was uniquely identified in her urine culture. Recurrent UTIs are generally defined as two or more infections within six months, or three or more within one year, and often prompt further investigation to identify potential risk factors. This case report highlights the rarity of R. planticola as a causative agent of UTIs and underscores the importance of comprehensive microbiological evaluation in patients with recurrent infections. The patient

Are long-lasting nonspecific symptoms related to vitamin D deficiency among older adults living in nursing homes?

BACKGROUND: Confusion, restlessness, and fatigue are common among older adults living in nursing homes. These nonspecific symptoms are often treated with antibiotics since they are frequently misinterpreted as urinary tract infections. Therefore, it is crucial to investigate other potential causes of these nonspecific symptoms. Impaired cognitive function in older adults may be associated with vitamin D deficiency and could lead to nonspecific symptoms. Currently, it is unknown whether there is a correlation between nonspecific symptoms, often misinterpreted as acute cystitis, and vitamin D de

Understanding patient and healthcare provider perspectives of uncomplicated urinary tract infection: patient journey, disease management, and the impact of antimicrobial resistance.

Uncomplicated urinary tract infections (uUTIs/acute cystitis) carry a substantial physical and psychological burden that negatively impacts patient quality-of-life, particularly for those who experience recurrent infection. A disconnect can exist between patients and healthcare professionals (HCPs), leading to poor patient-HCP communication, suboptimal treatment, and feelings of frustration and anxiety for many patients. The views of four patient authors with recurrent UTI or chronic uUTI and two HCP authors managing patients with this disease are presented in this Patient Perspectives article

Remote versus in-person consultations for acute cystitis: antibiotic treatment and clinical outcomes, a retrospective cohort study.

BACKGROUND: The COVID-19 pandemic brought a surge of remote consultations in Norwegian primary care with unknown implications for antibiotic treatment and outcomes of acute cystitis. AIM: To examine whether there were differences in antibiotic treatment or repeat contacts for acute cystitis between remote and in-person consultations. DESIGN & SETTING: A retrospective cohort study was undertaken. For the 4-year period from 2018-2021, we linked individual registry data on all contacts for cystitis by women aged ≥16 years in general practice and out-of-hours (OOH) services in Norway with regist

Updates to Recurrent Uncomplicated Urinary Tract Infections in Women: AUA/CUA/SUFU Guideline (2025).

PURPOSE: Our perceptions of recurrent UTI (rUTI) have evolved due to additional insights into rUTI pathophysiology, an appreciation for the adverse effects of repetitive antimicrobials ("collateral damage"), rising rates of bacterial antimicrobial resistance, and better reporting of the natural history of localized cystitis and rUTI. This document seeks to guide the evaluation and management of patients with rUTIs to prevent inappropriate antibiotic use, decrease the risk of antibiotic resistance, reduce adverse effects of antibiotics, provide guidance on strategies for rUTI prevention, and im

Multiplex Polymerase Chain Reaction vs Standard Urine Culture in Women Presenting with Symptoms of Acute Cystitis.

INTRODUCTION AND HYPOTHESIS: Prior studies suggest that acute cystitis diagnosis using standard urine culture (SUC) may be suboptimal compared to multiplex polymerase chain reaction (mPCR). mPCR is hypothesized to deliver faster and more accurate results. METHODS: This was a prospective, paired, within-subject study of female patients ≥ 60 years presenting with symptoms of acute cystitis. For all patients, urine samples were collected and SUC and mPCR results using the Vikor Urine-ID™ platform were obtained. Initial treatment was based on the results of the first available test, either SUC or

Efficacy and in vitro activity of gepotidacin against bacterial uropathogens, including drug-resistant phenotypes, in females with uncomplicated urinary tract infections: results from two global, pivotal, phase 3 trials (EAGLE-2 and EAGLE-3).

Two recent Phase 3 trials demonstrated the efficacy of gepotidacin compared with nitrofurantoin to treat uncomplicated urinary tract infections (uUTIs) in females. Pretreatment urine specimens were obtained from all participants. Based on pooled trial data (treatment groups combined), central laboratory culture results identified 1,421 (45%) participants with ≥1 baseline qualifying (≥105 CFU/mL) uropathogen (i.e., pooled microbiological Intent-to-Treat population). Escherichia coli, Klebsiella pneumoniae, and Proteus mirabilis were the most common qualifying uropathogens. Among 1,159 E. coli i
