We report two cases of perivesical abscess associated with neurogenic bladders. One patient suffered from diabetic neurogenic bladder, and the other spinal cord injury. Both patients had an uninhibited contraction at the low capacity on cystometry. They complained of difficulty in micturition and presented with large lower abdominal tumors. Bladder trabeculation may occur as an early manifestation of neurogenic bladder following spinal cord injury. The prevalence of bladder trabeculation in relation to other urologic morbidity was assessed in this clinical study using a urologic flow sheet. Data were extracted from a retrospective chart review of 90 patients with spinal cord injury (71% men) regularly followed at a tertiary care center. Sixty-four patients with multiple sclerosis and associated symptoms of neurogenic bladder dysfunction underwent urodynamic evaluation. We have employed the alpha blocking drug Nicergoline in 14 patients with neuropathic bladder dysfunction of the upper motor neurone type. A clinical analysis was done on 303 patients diagnosed by urodynamic study as having an unstable bladder, which was characterized by uninhibited contraction of the detrusor without any overt neurogenic lesions. Transient urinary incontinence in an elderly person is often due to a potentially reversible factor that can be recognized and corrected by the primary care physician. We studied and treated prospectively 62 neurologically normal children with vesicoureteral reflux using urodynamic techniques to identify uninhibited bladder contractions with voluntary sphincteric obstruction (dyssynergia). All children received antibiotic prophylaxis. Anticholinergic drugs were used additionally to treat uninhibited bladder contractions. The effect of the new alpha-adrenoreceptor blocking agent prazosin on hypermotility of the urinary bladder was investigated, using controlled cystometry in neurological patients with normal bladder or with uninhibited neurogenic bladder. In short-term administration of prazosin, the bladder capacity was markedly increased and the uninhibited detrusor contraction was diminished. Six children with neurogenic disorders of micturition are stimulated by Katona's method. The evaluation of the clinical and cystometric results carried out every 4 weeks shows the contradictoriness of this method. The clinical results show an unequivocal improvement in micturition behavior, whereas checking the objective cystomanometric results only suggests an increasing tonicity of the detrusor and suppression of the uninhibited detrusor contractions. The urgency for carrying out cystomanometric measurements during Katona's electrostimulation therapy is pointed out.
