The incontinent elderly face many difficulties from a problem that can have a multiplicity of causes. Etiologic factors of incontinence may be medical, psychologic or social. The cortical inhibitory stimuli that prevent involuntary voiding tend to decline with age. When carefully measured, some degree of uninhibited neurogenic bladder is often found in the elderly. Nonetheless, incontinence does not necessarily result. Precipitating factors, often emotional, determine the outcome.

The influence on the cystometrogram of repeated filling and varying filling rates was studied in two groups of neurological patients, one with normal and one with uninhibited neurogenic bladder. A technique which permits an even bladder filling and a continuous recording of the intravesical pressure was used. The present paper reports a family settled in the northern part of Norway, with a hereditary neurological disorder consisting clinically of spastic paraplegia associated with neurogenic bladder disturbances and syndactylia. We treated 63 patients with urinary symptoms associated with uninhibited detrusor contractions on cystometry by prolonged bladder distension. All patients had received drug therapy previously without success. Fourteen patients whose symptoms relapsed after a previous distension or who failed to obtain any improvement have had repeat distension. Postoperative cystometry was done in 36 cases. The effect of Flavoxate on hyperactive detrusor contraction was studied in 37 patients, including 11 patients in whom the drug was administered intravenously during the urodynamic study. Voiding abnormalities are encountered frequently in pediatric patients. Symptoms of daytime incontinence, frequency and nocturnal enuresis in any combination may indicate underlying neurophysiologic detrusor imbalance.
