Sacral rootlet rhizotomy at the conus medullaris for hypertonic neurogenic bladder.

Three patients with spastic neurogenic bladder underwent sacral root stimulation and appropriate section of motor rootlets to the detrusor at the levels of the conus medullaris. Compared to the results obtained with rhizotomies at the sacral levels in a previous series of patients, urinary frequency and infection were inhibited for longer periods of time, but some uninhibited bladder activity recurred in all three patients. Analysis of anatomical and electrophysiological data serves to emphasize the complexity and asymmetry of efferent supply to detrusor muscle. The authors conclude that the r

The anticholinergic effects of dicyclomine hydrochloride in uninhibited neurogenic bladder dysfunction.

A preliminary study to assess objectively the effects of dicyclomine upon uncontrolled detrusor contractions and bladder capacity was undertaken in 14 patients with a cystometric diagnosis of uninhibited neurogenic bladder. Response was measured by cystometry performed 1, 2 and 4 hours after a 20 mg. oral dose. Of the 14 patients 11 had significant blockade of uninhibited contractions and a concomitant increase in bladder capacity. Similar but less dramatic changes were noted in the remaining 3 patients. Dicyclomine seems to have effective anticholinergic properties. No remarkable side effects

Overactivity of the integral storage and voiding reflexes in the juvenile urinary incontinence syndrome.

The physiologic importance of 12 integral storage and voiding reflexes has not been generally recognized and appreciated by clinicians. Pathologic overactivity of reflexes No. 9 and No. 10, the urethrodetrusor facilitatory reflexes, appears to be a very common factor in the etiology of the juvenile incontinence syndrome. Mild but urodynamically significant obstructive lesions of the distal urethra are a common predisposing factor to chronic urethritis and are potentially remediable by simple surgical procedures. Cystometric as well as symptomatic improvement is commonly observed postoperativel

Vesico-ureteral reflux and uninhibited neurogenic bladder.

Ureteral reflux in female patients who also have latent uninhibited neurogenic bladder occurs synchronously with uninhibited contraction, is blocked by buscopan and is aggravated by besacholine. Intrinsic neuromuscular antireflux mechanism of the vesicoureteral junction (VUJ) appears to be affected by unknown ways with uninhibited contraction. Urethrovesical reflux observed synchronously with uninhibited contraction seems to play an additional role in the genesis of urinary infection. A different look is required in the management of these refluxes, in that concomitant use of the anticholinerg

Micturition urodynamic flow studies in children.

Voiding abnormalities are encountered frequently in pediatric patients. Symptoms of daytime incontinence, frequency and nocturnal enuresis in any combination may indicate underlying neurophysiologic detrusor imbalance. Incomplete evaluation of these symptoms can result in inappropriate medical therapy or even ineffective operations. Within the preceding 7 months 34 children with hard-core voiding abnormalities were evaluated with urodynamic techniques. Several categories of abnormal voiding patterns were identified, including the hyperactive external sphincter, uninhibited pediatric neurogenic

The treatment of the uninhibited bladder with dicyclomine.

Symptomatic treatment of the uninhibited bladder has presented a challenge because of the lack of an effective, well tolerated smooth muscle relaxant for the bladder that can be used during an extended interval. In a preliminary study oral dicyclomine produced resolution or significant improvement of symptoms in 24 of 27 patients and an increase in bladder capacity by an average of 137 plus or minus 26 ml. (91 plus or minus 22 per cent) after 8 weeks of therapy. Additional controlled trials definitely are warranted.

Detrusor hyperreflexia. A survey on its etiology and treatment.

Detrusor hyperreflexia (DH) is a frequently occurring condition. The symptomatology is characterized by frequency, urgency and urge incontinence. DH is defined as involuntary, uninhibited detrusor contractions. The physiology and pathophysiology of the micturition reflex is reviewed. The balance between cerebral stimulation and supraspinal inhibition is discussed. DH is caused by disturbances in this balance. Whereas increased afferent impulses to the central nervous system due to local disorders in bladder and/or urethra may produce DH, a neurological disorder affecting the inhibitory nervous

Management of the uninhibited bladder by selective sacral neurectomy.

The authors report the treatment of incontinence due to uninhibited bladder contractions by selective sacral neurectomy in nine patients, four without evidence of neurological disease. A detailed and objective analysis of bladder and urethral function, together with quantitation of clinical features, was made before and after operation. Seven patients were either cured or greatly improved. The overall increase in bladder capacity and reduction of uninhibited activity were statistically significant. The resting urethral sphincter pressure was unchanged, but the contractility of the voluntary ex

Uninhibited neurogenic bladder in adults.

The uninhibited neurogenic bladder is a common entity. It is characterized by urge incontinence, frequency, enuresis, and recurrent urinary tract infections. The diagnosis can only be made by a thorough urologic investigation, including cystometrography. A sound treatment program involves the use of anticholinergic medication to abolish uninhibited contractions, frequent voiding to keep intravesical pressure low, and antibiotic treatment when indicated for concurrent infection.

Plasma catecholamines during paroxysmal neurogenic hypertension in quadriplegic man.

Blood pressure, heart rate, and plasma catecholamine levels were measured in 16 quadriplegic subjects with physiologically complete cervical spinal cord transections above the level of the sympathetic outflow, and in 15 normal subjects (controls). In the quadriplegics the average resting blood pressure was 107/59 (mean, 75) mmHg, heart rate was 65 beats/min, and plasma norepinephrine (NE) and epinephrine (E) levels were 0.05 and 0.005 ng/ml, respectively. In the controls average resting blood pressure was 117/79 (mean, 92) mmHg, heart rate was 61 beats/min, and resting plasma NE and E levels w

Oxybutynin in bladder spasm, neurogenic bladder, and enuresis.

Oxybutynin chloride (Ditropan), a tertiary amine possessing anticholinergic and papaverine-like, direct muscular antispasmodic effects, has been used in controlled clinical studies in patients with neurovesical reflex activity, uninhibited bladders, enuresis, and primary muscle spasm. The cystometrically documented, synergistic, anticholinergic, and muscle relaxant activity of oxybutynin observed in these studies indicates that the drug can be highly effective in the management of reflex neurovesical dysfunction, enuresis, and bladder spasm.

Lioresal in the treatment of neurogenic bladder dysfunction.

Lioresal is a very effective compound in treatment of the neurogenic uninhibited bladder due to spinal cord lesions. It does not decrease, however, bladder spasms due to cerebral lesions. In detrusor-sphincter externus dyssynergia as frequently encountered in discreet spinal lesions Lioresal restores the normal reciprocal innervation of bladder and sphincter externus again. This effect is demonstrated electromyographically. Lioresal is of no effect on the normal bladder. Bladder capacity and micturition reflex before and after Lioresal therapy are unchanged.

Urge incontinence in women.

A survey has been presented of the author's personal methods to treat urge incontinence in women caused by neurogenic disturbances or by urethritis. Denervation of the bladder by unilateral or bilateral resection of the inferior hypogastric plexus is used for neurogenic disturbances with uninhibited bladder contractions or hypertonic bladders and in cases of interstitial cystitis, if a preoperative blockade with local anaesthesia has given a favourable result. Urethral diverticula are extirpated. Urethritis is treated with careful dilatation of the uretha and massage in combination with local

Urodynamic analysis of differential sacral nerve blocks and sacral neurectomy.

Urodynamic analysis in 17 patients after long-acting anaesthetic root blocks of S2-S4 has shown that it is possible to increase bladder capacity and reduce uninhibited contractions without significant adverse effects on sphincter function. Satisfactory voiding dynamics are observed after blocks. This has provided a physiological rationale for selective sacral neurectomy in eight patients. Initial improvement after operation has been noted in seven cases.
