When Lupus Hits the Spine: A Case of Extensive Transverse Myelitis.

PRESENTATION: The patient presented with acute urinary retention, fever (39°C), and rapidly progressive bilateral lower limb weakness progressing to flaccid paraplegia (0/5 MRC) with a T6 sensory level. DIAGNOSIS: MRI spine demonstrated longitudinally extensive T2-hyperintense intramedullary lesions from T6 to the conus. CSF analysis showed lymphocytic pleocytosis and elevated protein; autoimmune serology confirmed active systemic lupus erythematosus (SLE). SLE-associated longitudinally extensive transverse myelitis (LETM) was diagnosed following exclusion of other differential diagnoses. TR

[Case Report: Guillain-Barré Syndrome with Difficult-to-manage Pain and Persistent Urinary Retention].

Guillain-Barré Syndrome (GBS) is an acute inflammatory polyradiculoneuropathy that affects the peripheral nervous system, predominantly impairing motor function. Pain, both somatic and neuropathic, is reported in 89% of cases and is refractory to first-line analgesics in most of these. We present the case of a 75-year-old woman with an acute presentation of areflexic flaccid tetraparesis compatible with GBS. She received treatment with intravenous immunoglobulin (IVIg), which improved the motor component, but she experienced pain refractory to nonsteroidal anti-inflammatory drugs and pregabali

Severe Acute Transverse Myelitis With a Rapid Progression in an Infant.

Acute transverse myelitis (ATM) is a rare inflammatory disorder of the spinal cord, often triggered by infections, systemic diseases, or occurring without a known cause (idiopathic). It is a diagnosis of exclusion, characterized by motor, sensory, and autonomic dysfunction, with spinal cord inflammation confirmed through cerebrospinal fluid analysis or MRI. While most children experience a favorable recovery, severe cases can lead to lasting neurological deficits. This report presents the case of a 10-month-old male who presented with severe para-infectious ATM, likely triggered by Campylobact

A Rare Case of Acute Aortic Occlusion Mimicking Cauda Equina Syndrome in a Patient With Inflammatory Bowel Disease.

Acute aortic occlusion (AAO) is a rare and life-threatening condition, mostly secondary to acute thrombosis or embolism. It usually presents as bilateral lower limb ischemia; however, in rare cases, spinal cord infarction might coexist, mimicking cauda equina syndrome. We present a rare case of AAO by saddle embolism of a thoracic aortic mural thrombus. A 48-year-old male patient with a past medical history of ulcerative colitis and smoking was admitted with urinary and fecal incontinence, flaccid paraplegia, bilateral lower limb anesthesia, peripheral cyanosis, and pulseless lower extremities

What do X-ray images of the bladder during video urodynamics show us in patients with spinal cord injury?

STUDY DESIGN: Retrospective anonymized cohort study. OBJECTIVES: To study X-ray images of video urodynamics (VUD) in patients with spinal cord injury (SCI). SETTING: Single-center study. METHODS: X-ray images during VUD were categorized. Relation with the American Spinal Injury Association Impairment Scale (AIS), time since and level of SCI, cystometric data, method of bladder management, findings of flexible cystoscopy, and renal ultrasound were evaluated. Changes over time were studied. RESULTS: In 231 consecutive patients, VUD was done at a mean of 8.5 years after SCI. X3-ray bladder ap

A 66-Year-Old Woman with Achondroplasia and Quadriparesis with an Ovarian Cyst Erroneously Diagnosed as Urinary Retention by an Automated Bladder Scan.

BACKGROUND Automated bladder scanning has become a principal tool in the assessment and management of chronically debilitated patients residing in skilled nursing facilities, hospices, and acute inpatient settings. To a large extent, the bladder scan, generally performed by nursing staff, has replaced physical examination while addressing the differential diagnoses of anuria or voiding disturbances that require consideration of urinary catheterization. Health care providers can quickly master this easily performed technique, and currently, due to confidence in the bladder scan finding, physica

Botulinum toxin in low urinary tract disorders - over 30 years of practice (Review).

Botulinum toxin is a substance produced by Clostridium Botulinum and is responsible for human botulism. This substance is a poison, a neurotoxin, but used in limited quantities it can be a cure for some diseases. It is well connected to a large variety of medical applications. The mechanism of action relies on blocking the acetylcholine at the neuromuscular junction, which blocks the transmission of the nervous impulse with secondary flaccid paralysis. In urology, its role in idiopathic overactive bladder and neurogenic bladder is well known. We performed a thorough review using PubMed and oth

[Acute flaccid paralysis related to enterovirus with complete resolution of the neurological symptoms. A pediatric case report].

Since the wild poliovirus no longer circulates, the number of cases of acute flaccid paralysis decreased. However, cases related to non-polio enteroviruses and neurotrope viruses continue to occur. We present a nine-year-old patient with meningitis and myelitis with motor involvement in the lower limbs and neurogenic bladder associated with enterovirus, with complete resolution of the neurological symptoms following the administration of hyperimmune gammaglobulin.

Atypical teratoid/rhabdoid tumor of lumbar spine in a toddler child.

BACKGROUND: Atypical teratoid/rhabdoid tumor (AT/RT) is a rare pediatric central nervous system malignancy with poor outcome. AT/RT is infrequently located in the spinal cord. CLINICAL PRESENTATION: A 16-month-old boy presented with progressive urinary retention and weakness of the lower extremities. Magnetic resonance imaging of the spine revealed an intradural extramedullary mass occupying the spinal canal at the level of T10-L3. The tumor was successfully resected by using neuroendoscopy. Histopathology demonstrated rhabdoid cells with eccentric nuclei and eosinophilic cytoplasmic hyaline

Continent Catheterizable Vesicostomy: An Alternative Surgical Modality for Pediatric Patients With Large Bladder Capacity.

OBJECTIVE: To present a modified technique and early outcomes of a continent catheterizable vesicostomy in pediatric patients with either flaccid neurogenic bladder or intractable voiding dysfunction and large capacity bladder. METHODS: Six patients underwent the procedure from October 2014 to December 2015. A 4-cm Pfannenstiel incision was made, avoiding intraperitoneal dissection. After adequate mobilization, a 2-cm vertical flap at the dome of the bladder was identified and tubularized over a 12Fr catheter with 4-0 vicryl suture. The tubularized flap was then intussuscepted into the bladde

Botulinum Toxin Type a as a Therapeutic Agent against Headache and Related Disorders.

Botulinum neurotoxin A (BoNT/A) is a toxin produced by the naturally-occurring Clostridium botulinum that causes botulism. The potential of BoNT/A as a useful medical intervention was discovered by scientists developing a vaccine to protect against botulism. They found that, when injected into a muscle, BoNT/A causes a flaccid paralysis. Following this discovery, BoNT/A has been used for many years in the treatment of conditions of pathological muscle hyperactivity, like dystonias and spasticities. In parallel, the toxin has become a "glamour" drug due to its power to ward off facial wrinkles,

Applications of Botulinum toxin in urogynaecology.

Botulinum toxin (BTX) is a neurotoxin produced by bacterium clostridium. It is the most poisonous naturally occurring substance known to mankind. The neurotoxin binds to the peripheral cholinergic terminals and inhibits acetylcholine release at that junction leading to flaccid paralysis. This process appears to offer an attractive therapeutic option, filling the void between anticholinergics and surgery in cases of neurogenic and idiopathic detrusor overactivity, detrusor sphincter dyssynergia (DSD), interstitial cystitis and pelvic pain. This article reviews the application of Botulinum toxin

Transient ventriculoperitoneal shunt malfunction after chronic constipation: case report and review of literature.

BACKGROUND: Significant constipation in patients with shunt-dependent hydrocephalus may often be enough to bring a subclinical shunt malfunction to clinical attention or even to be the cause of temporary distal peritoneal shunt malfunction. The treatment of the constipation may address the symptomatic shunt dysfunction so as to avoid operative intervention. The pathogenesis of distal shunt malfunction and its management in such a situation are discussed, and the relevant literature is briefly reviewed. CASE REPORT: A 16-year-old young boy had shunt-dependent hydrocephalus secondary to prematu

[Neuropathy with non-alcoholic thiamine deficiency: two cases of bladder disorders].

INTRODUCTION: Neuropathy with non-alcoholic thiamine deficiency is reported in the literature, but bladder disorders are rarely detailed. CASE REPORTS: We report two cases of bladder disorders in neuropathy with thiamine deficiency. One patient presented with a flaccid bladder and impaired sensation; the postvoid residual volume was raised. The other patient had reduced bladder capacity, with detrusor hyperreflexia and detrusor-sphincter dyssynergia. In both cases, the bladder disorders disappeared with thiamine supplementation. CONCLUSION: Bladder symptoms may be heterogeneous in nonalcohol
