Modified Top-Down Approach to Reduce Voiding Cystourethrograms: Diagnostic Accuracy of Late DMSA Scintigraphy and Renal Ultrasound for Predicting Vesicoureteral Reflux in Children > 2 Years. A Retrospective Audit.

OBJECTIVE: The Indian Society of Pediatric Nephrology (ISPN) guidelines recommend only renal ultrasound for children > 2 years with febrile urinary tract infection (UTI), but its accuracy in predicting vesicoureteral reflux (VUR) is limited. We audited a modified top-down approach (MTDA) using late-phase Tc99m-dimercaptosuccinic acid (DMSA) scintigraphy to select patients for undergoing voiding cystourethrography (VCUG). METHODS: This is a retrospective audit of consecutive children > 2 years with febrile UTI who completed the index test (late-phase DMSA for the presence/absence of cortical s

Pseudomonas aeruginosa Exacerbates Bladder Cancer Progression by Activating Cancer-Driven Immunosuppression.

Pseudomonas aeruginosa infection is still a serious problem among immunocompromised patients who have advanced urinary tract malignancies, yet the impact of P. aeruginosa on disease progression remains poorly defined. In this study, we show that urinary tract infections caused by P. aeruginosa exacerbated bladder cancer progression in murine models, primarily by inducing an immunosuppressive tumor microenvironment. Mechanistically, P. aeruginosa activated Toll-like receptor 5 (TLR5) on bladder cancer cells, triggering phosphorylation of ERK1/2 and subsequent secretion of the chemokine CCL20. T

B cells disrupt tertiary lymphoid structure formation and suppress anti-tumor immunity.

Tertiary lymphoid structures (TLSs) promote antigen-specific anti-tumor immunity, but the regulators of TLSs homeostasis in cancer remain unclear. Using single-cell RNA-sequencing and spatial transcriptomics, we identify an IGLL5+ B cell subset in bladder cancer (BCa). In genetically engineered and humanized mouse models, these IGLL5+ B cells disrupt TLS's integrity and impair immunotherapy responses. Mechanistically, IGLL5+ B cells bind high endothelial venules (HEVs) via IGLL5-LTβR ligand-receptor interactions, with IGLL5 inducing a conformational change in LTβR that inhibits non-canonical N

Tumor-homing cerium nanozyme platform enables coordinated drug release and STING activation for bladder cancer treatment.

Non-muscle-invasive bladder cancer (NMIBC) carries 70% incidence yet remains plagued by high recurrence because single-modality chemotherapy or immunotherapy is crippled by rapid urinary clearance, poor tumor specificity, and narrow therapeutic windows. To overcome these challenges, we develop a homologous-targeted cerium nanozyme (HMZCO/DS@CM) that co-loads doxorubicin and the STING agonist MSA-2 in Mn/Zr-co-doped CeO₂ cores camouflaged with tumor-cell membranes. We discover that Mn/Zr substitution accelerates Ce³⁺/Ce⁴⁺ redox cycling, sustaining peroxidase- and SOD-like activities to deplete

Identification of a Muscle-Invasive Bladder Carcinoma Molecular Subtype of Poor Responders to Neoadjuvant Chemotherapy and High Expression of Targetable Biomarkers.

Neoadjuvant chemotherapy (NACT) is the standard treatment for muscle-invasive bladder carcinoma (MIBC), but its efficacy varies significantly among patients. The aim of this study is the identification of biomarkers and biological processes related to the response to neoadjuvant chemotherapy (NACT) in muscle-invasive bladder carcinoma (MIBC). Fifty-eight transurethral resection (TURBT) samples and thirty cystectomy samples from NACT non-responders were analyzed using mass spectrometry. Samples were classified with sparse k-means and consensus clustering. Protein networks were built using proba

Association between metabolic syndrome and overactive bladder in American adults: A cross-sectional study from NHANES 2005-2018.

The purpose of this study was to investigate the relationship between metabolic syndrome (MetS) and overactive bladder (OAB). This research involved a cohort of 11,078 individuals enrolled from the National Health and Nutrition Examination Survey (NHANES) between 2005 and 2018. We applied restricted cubic spline analysis and weighted multivariable logistic regression to assess the association between MetS and OAB, supplemented by subgroup analyses to reinforce these findings. After full multivariable adjustment, MetS was associated with OAB (odds ratio [OR] = 1.30; 95% confidence interval [CI]

Chemotherapy resistance of urinary bladder cancer mediated by a Notch and Wnt co-regulatory module of stemness.

Despite breakthroughs in molecularly targeted and immune therapies, the prognosis for patients with urinary bladder cancer (UBC) has remained unsatisfactory over the past few decades. Understanding the molecular underpinnings of UBC treatment refractoriness is crucial for identifying novel therapeutic targets and strategies. Cancer stemness plays a pivotal role in the oncogenesis and treatment resistance of UBC, while the underlying molecular regulatory mechanisms are poorly understood. We identified isoform 1 of ASPM (ASPM-i1) as the most upregulated stemness-associated factor in tumorigenic

U-shaped and linear associations of ketogenic diet with nocturia and overactive bladder: mediation roles of frailty and platelet-to-HDL-C ratio and the influence of physical activity.

BACKGROUND: Ketogenic diet (KD), characterized by low carbohydrate and high fat intake, has become an increasingly popular strategy for weight management and metabolic improvement in recent years. However, its potential influence on lower urinary tract symptoms (LUTS), particularly overactive bladder (OAB) and nocturia, remains unclear. This study aimed to investigate the associations between the ketogenic diet ratio (KDR) and OAB or nocturia, and to explore the mediation roles of the frailty index (FI) and platelet-to-HDL-C ratio (PHR). METHODS: We analyzed data from 22,249 adults in the Nat

Real-world outcomes of bladder-sparing strategies for BCG-unresponsive non-muscle-invasive bladder cancer: a multicenter study.

INTRODUCTION: Patients with BCG-unresponsive non-muscle-invasive bladder cancer (NMIBC) face a high risk of disease progression. While radical cystectomy (RC) remains the recommended standard of care, many patients are unfit for or unwilling to undergo radical surgery, leading to bladder-sparing strategies spreading in real-world practice. This study aims to describe the oncological outcomes of patients diagnosed with BCG-unresponsive NMIBC treated with gemcitabine/docetaxel (Gem/Doce), electromotive drug administration of mitomycin C (EMDA/MMC), further BCG, or upfront RC. METHODS: We include

Effects of a Cranberry, Probiotic, and Vitamin C Supplement on Recurrent Urinary Tract Infections in Children-A Cohort Study.

Recurrent urinary tract infections (UTIs) are common in children and may lead to long-term renal complications, especially in those with vesicoureteral reflux (VUR). Concerns about antibiotic resistance and side effects of prolonged antibiotic use underscore the need for safe, non-antibiotic alternatives. This prospective, self-controlled observational study evaluated a standardized combination of cranberry extract (36 mg proanthocyanidins daily), probiotics (Lactobacillus rhamnosus and Bifidobacterium longum), and vitamin C (250 mg daily) in 39 children aged 3-16 years, including a subgroup w

rBCG-LTAK63 Outperforms BCG in Bladder Cancer Immunotherapy: Dendritic and T Cell Coordination Drives Superior Tumor Control in a Mouse Model.

Bacillus Calmette-Guérin (BCG) remains the standard treatment for non-muscle invasive bladder cancer (NMIBC), yet approximately 30% of patients fail to respond. To enhance therapeutic efficacy, we developed rBCG-LTAK63, a recombinant BCG strain, as a novel immunotherapeutic candidate. In vitro, rBCG outperformed BCG in MB49 cell/splenocyte co-cultures by enhancing T cell activation and improving spheroid growth control. In vivo, rBCG demonstrated superior antitumor efficacy, significantly reducing the growth of subcutaneously implanted MB49 tumor cells. Immune profiling revealed that rBCG uniq

Alcohol Use Disorder versus Adverse In-Hospital Outcomes after Radical Cystectomy and Ileal Conduit Urinary Diversion.

BACKGROUND: Radical cystectomy (RC) is the gold standard treatment for muscle-invasive bladder cancer (BCa). Alcohol use disorder (AUD) may predispose to worse adverse in-hospital outcomes after RC and ileal conduit urinary diversion. The current study addressed this knowledge gap. PATIENTS AND METHODS: Descriptive analyses, propensity score matching (PSM), and multivariable logistic and Poisson regression models were used to address patients with BCa treated with RC and ileal conduit within National Inpatient Sample (2006-2019). RESULTS: Of 15,018 RC and ileal conduit patients, 320 (2.1%) h
