Straining to Pass Urine in Men with Lower Urinary Tract Symptoms.

Straining to void is widely regarded as a clinical indicator of bladder outlet obstruction (BOO) in men with lower urinary tract symptoms (LUTS). However, the physiological significance of this behaviour remains poorly understood. This review examines the role of straining during the voiding cycle and evaluates current evidence regarding its relationship with BOO. Straining represents a voluntary increase in intra-abdominal pressure that may occur at different stages of micturition. Studies comparing patient-reported straining with urodynamic findings demonstrate poor correlation, and the prev

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

Postvoided Residual/Bladder Emptying: A Mini Review.

Postvoid residual (PVR) urine volume is widely used in the evaluation of patients with lower urinary tract symptoms (LUTS) and benign prostatic obstruction (BPO), yet its clinical role remains debated. This mini review critically appraises current evidence on the definition, pathophysiology, measurement, and clinical utility of PVR. PVR reflects impaired bladder emptying and is influenced by multiple mechanisms, including bladder outlet obstruction, detrusor underactivity, and altered sensory function. However, its diagnostic accuracy for identifying specific conditions is limited by high inte

Seeking a Credible Basis for Prostatic Obstruction as the Reversible Cause of Other Lower Urinary Tract Dysfunctions.

Lower urinary tract symptoms (LUTS) and various lower urinary tract dysfunctions (LUTDs) often coexist because their prevalence increases with ageing. In men, benign prostatic obstruction (BPO) is often proposed as a cause of other "secondary" LUTDs. However, causality of BPO in LUTDs, other than voiding LUTS and acute urinary retention, is probably indirect, if present at all. Indirect mechanisms complicate the inference of causality and may reduce the chance of reversibility. BPO is not a unifying explanation for LUTD, but one possible aetiological factor among many age-related, metabolic, n

Intractable Diarrhea due to Enterovesical Fistula Caused by Recurrent Urinary Tract Infection with Bladder Outlet Obstruction: A Rare Clinical Presentation of "Urinary Diarrhea".

An enterovesical fistula (EVF) is an abnormal connection between the intestinal tract and urinary bladder, typically developing with symptoms of pneumaturia and fecaluria. We herein present the case of an 81-year-old man with 10-day watery diarrhea. After ineffective empiric antidiarrheal treatment, computed tomography and cystography revealed EVF and gas in the bladder lumen and within its wall. His diarrhea improved after urethral catheter placement. EVF can occasionally cause diarrhea in a delicate pressure balance between the bladder and intestine. Clinicians should be aware of this unusua

Urothelial Piezo1 channel contributes to BOO-induced inflammation, EMT, and bladder fibrosis via activation of NLRP3 inflammasome.

BACKGROUND: Elevated intravesical pressure has been implicated as a key trigger in the onset and progression of bladder outlet obstruction (BOO)-induced inflammation and fibrosis. Mechanosensitive ion channel Piezo1 is abundantly expressed in urothelium. However, it remains unclear whether Piezo1 in the bladder epithelium is necessary for BOO-induced fibrosis. This study aimed to elucidate the role of urothelial Piezo1 in BOO-induced bladder fibrosis. METHODS: BOO was induced in female mice via partial urethral ligation. Urothelium-specific Piezo1 conditional knockout mice and Piezo1-knockout

Hydrostatic pressure regulates glutamine metabolism to promote bladder fibroblast activation via the Piezo1/YAP1/GLS1 axis.

BACKGROUND: Bladder outlet obstruction (BOO), initiated by abnormal mechanical stress, leads to progressive bladder fibrosis and functional decompensation. Abnormal mechanical stress plays a key role in the pathogenesis of various diseases, including fibrosis. However, the mechanisms through which BOO-induced abnormal mechanical stress drives bladder fibrosis remain poorly understood. Recent studies have highlighted that Yes-associated protein 1 (YAP1) serves as a critical integrator of mechanical signals and metabolic alterations. Abnormal mechanical stress can promote disease progression by
