OBJECTIVES: To describe functional outcomes and identify risk factors for artificial urinary sphincter (AUS) failure and need for urinary diversion (UD) in patients with iatrogenic devastated bladder outlet (DBO). Our hypothesis was that pelvic radiation or ablative therapy (RT/AT) increases the risk of complications compared to surgery alone.
METHODS: Multicenter, retrospective study of patients with complications from previous treatments, subsequent AUS failure and unsalvageable DBO, ultimately requiring UD between 2008 and 2020. Demographics, comorbidities, prior treatments, reconstructive

Background Primary bladder neck obstruction (PBNO) is a functional obstruction at the bladder neck causing lower urinary tract symptoms (LUTS) in young males. Treatment options include medical therapy with alpha-blockers and surgical intervention, such as bladder neck incision (BNI). This study compares the clinical and urodynamic outcomes of these modalities. Methodology A retrospective analysis was conducted on 67 males (<40 years) with PBNO diagnosed via video urodynamics. Patients were divided into medical (n = 39) and surgical (n = 28) groups. Symptom scores (International Prostate Sympto

INTRODUCTION/AIMS OF STUDY: This original work done in patients with Benign Prostatic Obstruction aims to find out the impact of duration of medication therapy on the outcomes of TURP. Medical management of BPO is recommended as the first line of treatment and surgery is usually reserved for those who have failure of medical management and/or complications of BPO (refractory retention, vesical calculus, recurrent UTI or hematuria). However, there is some pointing evidence that delay in the surgery may affect the overall outcomes of transurethral resection of prostate (TURP) [1, 2] Presently, t

Invasive urodynamics (iUDS) should be selectively performed in male patients with lower urinary tract symptoms before invasive treatment for benign prostatic obstruction. Recent evidence highlights the relevance of iUDS, particularly in patients with mixed symptomatology or significant comorbidities. Parameters such as the maximum flow rate, Bladder Outlet Obstruction Index, Bladder Contractility Index play a crucial role in predicting surgical outcomes, which emphasizes the importance of meticulous patient selection to maximize therapeutic efficacy.

AIMS: We examined the role of subtypes of adenosine receptors in bladder dysfunction and changes in the adenosine receptor expression in the bladder using male rats with partial bladder outlet obstruction (BOO).
METHODS: In Sprague-Dawley rats (male 8-weeks old), BOO was produced by a partial ligation of the urethra along a metal rod of a 1.2 mm outer diameter. Control rats underwent sham operation. Awake cystometrograms (CMG) were first recorded during saline instillation, and then an adenosine A1 receptor agonist (CCPA, 4.1 μM), an adenosine A2A antagonist (ZM241385, 15 μM), or inosine (1 m

Background Primary bladder neck obstruction (PBNO) in females is a rare condition with a tedious diagnostic process. The existing literature on bladder neck incision (BNI) to treat PBNO lacks a precise description of the surgical technique. Objective In this study, we provide a diagnostic protocol to streamline the evaluation of PBNO in females. We evaluate the outcomes of a precise surgical technique of BNI aimed at reducing the rates of known complications such as vesicovaginal fistula (VVF) and urinary incontinence. Methods This single-center prospective observational study, conducted over

INTRODUCTION: Bladder outlet obstruction (BOO) due to benign prostatic hyperplasia (BPH) is common in aging men and can be treated with holmium laser enucleation of the prostate (HoLEP). However, diminished bladder contractility (DC) is also highly prevalent (9%-48%) and can be clinically indistinguishable from BOO without urodynamics (UDS). While HoLEP effectively treats BPH/BOO, clinical outcomes data for patients with DC are limited and mixed. We aim to compare the prevalence and risk factors of catheter dependence among patients with and without DC after HoLEP.
METHODS: A retrospective co

An increasing number of procedures are available for treatment of bladder outlet obstruction. Minimally invasive surgical treatments include prostatic urethral lift (UroLift), water vapour therapy (Rezūm), the temporary implanted nitinol device, and transperineal laser ablation. Longer patient waiting lists, wider patient choice, patient comorbidities, and interest in conducting procedures in outpatient settings have placed a focus on options other than a general anaesthetic (GA). Non-GA options for BOO include various instillations or injections of local anaesthetic and methods for sedation.

Bladder outlet obstruction (BOO) leads to bladder structural and functional changes, including elevated detrusor contractile pressure, detrusor hypertrophy, fibrosis, and impaired contractility. Currently, no effective therapeutic strategy for BOO-induced bladder dysfunction. This study investigated whether low-intensity extracorporeal shock wave therapy (LiESWT) could improve bladder function by promoting detrusor contractility, urothelial proliferation, angiogenesis, and neurogenesis in a rat model of partial BOO (PBOO). Male Sprague-Dawley rats were divided into sham and PBOO groups for 4 o
