Prognostic Factors of Transvaginal Repair for Vesicovaginal Fistulas: A 5-Year Single-Center Study.

INTRODUCTION AND HYPOTHESIS: Vesicovaginal fistula (VVF) causes significant physical and psychosocial morbidity. Transvaginal repair, although generally safer than the abdominal route, lacks guideline-defined predictors of surgical success. However, recent trials have suggested that certain factors may influence surgical success. METHODS: This retrospective study was conducted in a single-center hospital in Jakarta, Indonesia, between January 2020 and December 2024. Fistulas were classified using the Goh and Waaldijk systems. Data on patient demographics were collected preoperatively, and dat

Aortoesophageal fistula caused by Mycobacterium bovis following intravesical Bacillus Calmette-Guérin therapy: a case report and literature review.

BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) therapy is an established treatment for non-muscle-invasive bladder cancer, involving direct instillation of a solution containing attenuated Mycobacterium bovis into the bladder. Although generally well-tolerated, rare vascular complications, including mycotic aneurysms and aortoenteric fistulas, have been reported. CASE PRESENTATION: We report the case of a 76-year-old Japanese man who developed an aortoesophageal fistula three years after completing intravesical BCG therapy for superficial bladder carcinoma. The patient initially prese

Improving obstetric fistula care in Africa: A scoping review of key components of repair programs.

Obstetric fistula is a preventable childbirth injury caused by prolonged labor without timely care, primarily affecting women in sub-Saharan Africa, resulting in incontinence, stigma, and social and economic consequences. While surgical repair is essential, post-repair support and community reintegration are critical for recovery. The purpose of this scoping review was to assess key elements of obstetric fistula repair programs in Africa, including outreach and program delivery. A search of four databases and relevant websites was conducted to identify primary research published between 2014 a

Consensus Statements on the Definition of Surgical Success Following Obstetric Urinary Pelvic Floor Fistula Repair: An IUGA-ICS Proposal.

INTRODUCTION: Standardized definitions for surgical success after obstetric urinary pelvic floor fistula (UPF) repair are lacking. This study aimed to establish a consensus among fistula surgeons on defining surgical success for UPF repair caused by obstetric (childbirth-related) injuries. MATERIALS AND METHODS: A working group was initiated by the International Urogynecology Association (IUGA) in collaboration with the International Continence Society (ICS). Following a systematic literature review, statements were developed, and a Delphi method was applied to reach a consensus on each state

Iatrogenic vesicocolic fistula after gynecologic surgery: a case report.

INTRODUCTION AND IMPORTANCE: We present a unique case of a vesicocolic fistula (VCF) that occurred postoperatively after ovarian cyst surgery, caused by the migration of an intraoperatively forgotten gauze into the urinary bladder. This represents a very rare etiology of VCFs. CASE PRESENTATION: A patient who had undergone surgery for a giant left ovarian cyst 5 months prior presented with recurrent urinary tract infections and an ultrasound image suggestive of a bladder stone. Cystoscopy, contrast-enhanced computed tomography, and colonoscopy confirmed a VCF with an intravesical foreign body

Principles and Techniques of Vesicovaginal Fistula Repair.

INTRODUCTION AND HYPOTHESIS: Vesicovaginal fistula (VVF) are anomalous communications between the bladder and vagina, often arising as a complication of obstetric injury or gynecologic surgery. Patients typically present with continuous urinary leakage. Successful repair hinges on adherence to core surgical principles of adequate exposure, tension-free multilayer closure, and well-vascularized tissue interposition when needed. METHODS: This video article demonstrates the application of these principles across three cases performed via transvaginal, robotic, and open abdominal approaches. The

Characteristics Associated with Failure of Obstetric Fistula Surgery.

INTRODUCTION AND HYPOTHESIS: Quality of life can significantly improve following successful surgical repair of obstetric fistula; however, these surgeries may be technically challenging. There are limited consensus data regarding optimal fistula closure techniques and perioperative management. The objective of this study was to identify clinical and anatomical characteristics associated with failure of obstetric vesicovaginal/urethrovaginal fistulas (VVaF/UVaF) closure. MATERIALS AND METHODS: In this retrospective cohort study, medical records of patients following VVaF/UVaF surgery at a sing

Surgical management of anterior sacral meningoceles: a case series and systematic review.

PURPOSE: Anterior sacral meningoceles (ASMs) are rare forms of spinal dysraphism caused by anterior herniation of meninges through a sacral defect. Surgical management is the treatment of choice, yet the optimal approach remains debated. This study compares outcomes of three main surgical approaches-anterior transabdominal, posterior transsacral, posterior sagittal (PSARP-type)-focusing on the efficacy and safety of the PSARP-type. METHODS: A retrospective single-center analysis was conducted on eight patients treated for ASM between 2008 and 2025. Demographic, clinical, radiological, functio

Step-by-step combined surgical approach to successfully repair complex and challenging vesico-vaginal fistulas: Insights from a case series.

INTRODUCTION: Several surgical approaches are available for the repair of complex vesico-vaginal fistulas; however, robust clinical evidence and standardization of techniques are lacking. Complex vesico-vaginal fistula is defined as either a recurrent fistula following primary surgical repair or a fistula related to prior pelvic surgery and/or radiotherapy. The aim of this study was to describe a step-by-step combined vaginal and laparoscopic technique and to assess its feasibility, safety, and efficacy. MATERIAL AND METHODS: A retrospective study was performed between 2016 and 2023, involvin

An unusual cause of a vesical mass: Small bowel diverticulum presenting with a bladder fistula: A case report and review of the literature.

RATIONALE: Small intestinal diverticulum is rare (<5% incidence) and often asymptomatic. Its presentation as a bladder mass mimicking tumor, leading to vesico-enteric fistula, is exceptionally uncommon. This case highlights the diagnostic challenge and potential for misdiagnosis. PATIENT CONCERNS: A 35-year-old woman presented with a complex clinical course spanning 4 surgical interventions over 20 months. Initial symptoms included urinary frequency, urgency, dysuria, and persistent right lower quadrant abdominal pain. Recurrent "bladder tumors" were noted on serial cystoscopies, and the pati
