Unique presentation of urosymphyseal fistula and conservative management.

A urosymphyseal fistula is an abnormal connection between the lower urinary tract (typically bladder or proximal urethra) and pubic symphysis. This is an uncommon late complication of prostate cancer treatment, often occurring after pelvic radiation and urethral instrumentation. We report a 79-year-old man who, after external radiotherapy, salvage brachytherapy, and transurethral bladder neck incision, developed a urosymphyseal fistula with urinary tracking into the anterior thigh compartment. Given comorbidities, he was managed conservatively with urinary diversion, percutaneous drainage, and

Urostomy Complications: A Narrative Review for Emergency Clinicians.

BACKGROUND: A urostomy is a diverting measurement for urine outflow. These are necessary for several conditions, but several complications may occur following urostomy creation. OBJECTIVE: This narrative review provides emergency clinicians with a focused overview of urostomy complications. DISCUSSION: A urostomy is used in the management of several conditions, most commonly bladder cancer, but also trauma, congenital disorders, and neurogenic bladder. The most common type of diversion is a non-continent cutaneous diversion using the ileum as a conduit. There are a number of complications th

miR‑324‑5p in oncology: Molecular mechanisms and clinical outlook (Review).

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Research progress of SPP1 in cancer regulation: Tumorigenesis, metastasis, tumor microenvironment, immunity and therapeutic targets (Review).

OBJECTIVES: Psoriatic arthritis (PsA) has been defined as a systemic, chronic, inflammatory arthritis, usually seronegative for rheumatoid factor (RF), associated with cutaneous psoriasis. The exact prevalence of PsA is unknown and its estimation has been difficult, partly due to the lack of a widely accepted classification criteria. Agreed and validated criteria will facilitate comparison between centres and different countries in the areas of epidemiology, outcome studies and therapeutic trials. A number of classification criteria have been published by Moll & Wright (M & W), Bennett's, Vase

MAG3 SPECT/CT Detection of Vesicocolic Fistula in Genitourinary Tuberculosis.

Enterovesical fistula is a rare complication of genitourinary tuberculosis and may be difficult to diagnose, particularly in the setting of impaired renal function. We report a 41-year-old woman with chronic kidney disease and bilateral obstructive uropathy secondary to treated genitourinary tuberculosis. 99m Tc MAG3 renography demonstrated a nonfunctioning left kidney and preserved right renal function without obstruction. Delayed images revealed abnormal tracer propagation from the urinary bladder into bowel loops. SPECT/CT localized the tracer within the colon, confirming a vesicocolic fist

Villous adenoma of the urinary bladder with imaging findings suggestive of enterovesical fistula: A case report.

Villous adenomas of the urinary bladder are rare epithelial neoplasms with malignant transformation potential, exhibiting histological features similar to their gastrointestinal counterparts. Their low prevalence has hindered clinical characterization and differential diagnosis from other malignant bladder entities. We present the case of a 63-year-old woman with a history of cardiovascular and metabolic diseases who was admitted for acute urinary obstruction and hematuria. Imaging studies revealed a bladder mass and imaging findings suggestive of a possible enterovesical fistula, although no

Giant vaginal lithiasis in spinal cord injury related neurogenic lower urinary tract dysfunction: diagnostic pitfall with upper urinary tract deterioration.

INTRODUCTION: Vaginal lithiasis is exceptionally rare and may mimic vesical lithiasis on imaging, particularly in women with spinal cord injury (SCI) and neurogenic lower urinary tract dysfunction (nLUTD). Delayed recognition may cause recurrent infection, obstructive uropathy and upper urinary tract deterioration. CASE PRESENTATION: A 38-year-old woman with a T6 ASIA Impairment Scale (AIS) grade A SCI and nLUTD after epidural abscess drainage presented with continuous urinary leakage, recurrent febrile urinary tract infections and progressive renal impairment. Urine culture grew Pseudomonas

Conversion From Cutaneous Ureterostomy to Ileal Conduit in Frail Patients: A Descriptive Case Series.

BACKGROUND: Cutaneous ureterostomy is frequently selected for frail patients undergoing radical cystectomy due to its reduced operative complexity. However, long-term complications such as urinary tract infections (UTIs), stomal stenosis, and catheter dependence are common. Delayed conversion to ileal conduit has been proposed as a reconstructive strategy after clinical stabilization. CASE PRESENTATION: Five frail patients underwent radical cystectomy with cutaneous ureterostomy followed by delayed conversion to ileal conduit. Conversion occurred 10-36 months later due to recurrent complicati

Vesicocutaneous fistula secondary to invasive bladder squamous cell carcinoma: an unusual initial presentation.

We report a rare presentation of vesicocutaneous fistula associated with invasive squamous cell carcinoma. An elderly man presented with urinary leakage from an anterior abdominal wall wound and features of sepsis, preceded by progressive suprapubic swelling that spontaneously ruptured. CT imaging demonstrated a fistulous tract between the bladder and abdominal wall with features of locally advanced malignancy and appearances suspicious for metastatic disease. Histopathology confirmed squamous carcinoma; however, due to limited sampling, it was not possible to definitively distinguish between

Robot-Assisted Upper Pole Hemi-nephroureterectomy for Vaginal Ectopic Ureter.

A duplex kidney with vaginal ectopic ureter insertion is a rare congenital anomaly frequently presenting as refractory urinary incontinence in females and often misdiagnosed as neurogenic bladder or vesicovaginal fistula. The authors describe the case of a 41-year-old female with progressive relapsing-remitting multiple sclerosis (EDSS 6.5), wheelchair dependent, bilateral hip arthroplasties, persistent vaginal leakage, recurrent multidrug-resistant Escherichia coli urinary tract infections, and initial misattribution to neurogenic bladder leading to intermittent self-catheterization. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

N-cadherin mechanosensing in ovarian follicles controls oocyte maturation and ovulation.

The cell adhesion molecule N-cadherin (CDH2) is a membrane component of adherens junctions which regulates tissue morphogenesis and architecture. In the follicles of mammalian ovaries, N-cadherin adherens junctions are present between granulosa cells, cumulus cells, and at the interface of cumulus cell transzonal projections and the oocyte. We demonstrate a mechanosensory role of N-cadherin integrating tissue structure and hormonal regulation of follicular morphogenic events including expansion of the cumulus-oocyte complex (COC) matrix, oocyte maturation, and ovulation. Two small molecule N-c

Ruxolitinib alleviates DSS-induced acute ulcerative colitis by inhibiting STAT1 phosphorylation and reducing MDSC infiltration.

OBJECTIVE: This study aimed to evaluate the therapeutic effects of Ruxolitinib, a JAK1/2 inhibitor, on DSS-induced acute colitis in mice, with a focus on its impact on disease activity, inflammatory responses, modulation of myeloid-derived suppressor cells (MDSCs), and regulation of the JAK/STAT1 signaling pathway. METHODS: Acute UC was induced in C57BL/6 mice by administering a 2.5% DSS solution. Mice were randomly assigned to three groups: the blank group (no DSS), the model group (DSS only), and the Ruxolitinib-treated group (DSS +30 mg/kg Ruxolitinib by gavage for 14 consecutive days). Bo

Buccal Mucosal Graft as a Second Layer in the Vaginal Repair of Vesicovaginal Fistulas.

OBJECTIVE: Vesicovaginal fistula (VVF) is a pathological communication between the urinary bladder and the vagina. The most common cause of VVF is hysterectomy, while less common causes include obstetric trauma and pelvic surgery. Most cases require surgical intervention. Although various techniques have been described for the management of VVF, none has been considered the gold standard of management. In this study, we describe our technique using buccal mucosal graft (BMG) as a second layer in the repair of VVF through a vaginal approach. METHODS: A total of 10 patients underwent surgery be

Medical Gaze Sans Bioethics: Revisiting Enslaved Black Women's Medical Bondage in Behind the Sheet.

The birth of modern gynecology in the USA is preceded by experimental exploitations of Black women's bodies in the mid-nineteenth century, entailing a long-drawn extraction of "reproductive knowledge" from enslaved patients. Charly Evon Simpson's Behind the Sheet (2019) stages the history of medical bondage of Black enslaved women in antebellum South, reconstructing the events that led to the surgical innovation for vesico-vaginal fistula. Scrutinizing Simpson's dramatization of the event, this paper prompts inquiries into the interplay of power and consent between the physician and the enslav

Robotic Laparoscopy and Endoscopy Cooperative Surgery for Primary Appendiceal Mucinous Carcinoma Masquerading as Bladder Cancer: A Case Report.

Bladder invasion by appendiceal cancer resulting in a vesico-appendiceal fistula is an uncommon occurrence. Both radical tumor removal and functional preservation of the bladder are desirable in the surgical treatment of this disease, and there are few reports on detailed surgical methods. Here, we describe a case of primary appendiceal mucinous carcinoma with bladder invasion treated with robotic laparoscopy and endoscopy cooperative surgery (RECS). A woman in her 60s was initially considered to be bladder tumor and underwent transurethral resection. However, the tumor rapidly regrew, and she

[Functional disorders of the lower urinary tract following urogynecologic and abdominal surgery].

Bladder dysfunction is divided into storage and emptying disorders, which can also be the result of surgical interventions in the small pelvis, either individually or in combination. Neuroirritants from alloplastic implants are often associated with urge complaints and pelvic pain. Removal of the irritant agent carries the risk of incalculable collateral damage and recurrence of the symptoms that initially led to surgery. Conservative measures, on the other hand, are often lengthy, multimodal, and yet frustrating. Iatrogenic denervation of the lower urinary tract-mainly due to damage to the pe
