Preliminary experience with minimally invasive transvaginal single-port laparoscopic vesicovaginal fistula repair: report of 10 cases.

BACKGROUND AND PURPOSE: As a common complication following genitourinary trauma, the methods and approaches for repairing vesicovaginal fistulas (VVF) remain controversial. Minimally invasive techniques, such as laparoscopy, are gaining widespread attention. This study summarizes our surgical experiences and postoperative management regarding transvaginal single-port laparoscopic repair of vesicovaginal fistulas. PATIENTS AND METHODS: We retrospectively analyzed the clinical data of 10 transvaginal single-port laparoscopic vesicovaginal fistula repairs performed in the Department of Urology a

Comparison of sterile normal saline and 1% povidone-iodine for vaginal preparation in vesicovaginal fistula: a randomised trial.

BACKGROUND: Vaginal preparation before transvaginal gynecological surgeries improves the success rate. However, there is no consensus on which agent is superior for transvaginal repair of vesicovaginal fistula (VVF). We aimed to compare irritation symptoms and antimicrobial function between sterile normal saline (SS) and 1% povidone-iodine (PI) for vaginal preparation in surgical repair of VVF. METHODS: This randomised, controlled trial was conducted on patients with VVF underwent transvaginal repair. Preoperative vaginal irrigation was randomly performed with either SS or 1% PI. The patients

A model to predict failure of surgical treatment of vesicovaginal fistulas.

INTRODUCTION: Vesicovaginal fistula (VVF) is one of the most important diseases related to quality of life in women's diseases and postoperative complications. We aimed to identify preoperative predictive factors of fistula repair failure and to develop a predictive model for this disorder. PATIENTS AND METHODS: The data from patients who underwent VVF repair from January 2010 and December 2020 were recorded and analyzed. The therapeutic results were assessed after a follow-up of at least six months. A successful VVF repair was defined as: closed VVF as noted on visual inspection of both the

A complication of emergency caesarean section: delayed diagnosis of vesicouterine fistula with associated necrotising soft tissue infection and small bowel obstruction.

Vesicouterine fistula (VUF) is a rare complication following caesarean sections, often presenting with the classic triad of Youssef syndrome (urinary incontinence with cyclic haematuria (menouria) and amenorrhoea) but sometimes without typical symptoms. We report a case of a woman in her early 30s who developed VUF after an emergency lower segment caesarean section, complicated by necrotising soft tissue infection (NSTI) of the left forearm. One week post-surgery, she presented with NSTI symptoms and haematuria, which were managed with broad-spectrum antibiotics and debridement. During her hos

A Foreign Body as a Gynaecological and Sexological Issue-Case Study and Literature Review.

Introduction: The most commonly encountered foreign body in the vagina is a tampon, which is routinely removed during gynaecological examinations. While these cases are common and well-known in medical practice, there are also situations involving much more unusual foreign bodies. This article focuses on these rare and often surprising cases, which can pose diagnostic and therapeutic challenges. Highlighting this topic aims to draw attention to the variety of situations gynaecologists may encounter in their daily professional practice. Case Study: A 62-year-old woman was admitted to the Gynaec

Robotic Vesico-Vaginal Fistula Repair.

Vesico-vaginal fistulae require complex reconstructions and robotic assistance can be beneficial in obtaining successful outcomes. In this video, we demonstrate two of our techniques of vesicle-vaginal fistula repair, one with transverse cystotomy and other with vertical cystotomy, along with salient steps of the procedure. The indication, preparation, instrumentation, and surgical steps are also discussed along with a review of available literature.

Urologic Fistulas in Czech Women With Gynaecologic Malignancies.

INTRODUCTION: In developed countries, urologic fistulas arise mainly from malignancies, radiotherapy, or surgical trauma. Hysterectomy and radiation therapy are both critical components of the treatment of women with cancers. Urologic fistulas significantly reduce the quality of life of cancer patients, and may result in delays or even refusal of adjuvant treatment by these patients, thereby negatively impacting both short- and long-term cancer survival. MATERIALS AND METHODS: A 10-year retrospective study of urologic fistulas associated with gynaecologic malignancies at the University hospit

Pyocystis Causing Abdominal Wall Abscess and Necrotising Fasciitis.

Pyocystis is a rare but known complication of the defunctioned bladder. We report a case of a female patient with long-term bilateral nephrostomies who was initially thought to have cystitis but later found to have pyocystis causing a vesicocutaneous fistula and an anterior abdominal wall abscess. It progressed to necrotising fasciitis, necessitating emergency drainage and debridement. This yielded 400 ml of pus, and she was found to have a defect in the anterior rectus sheath leading straight to the bladder. This report highlights the importance of maintaining a high index of suspicion for py

Quantifying the impact of bladder complications following gynecological cancer treatment: systematic review and meta-regression.

OBJECTIVE: Gynecological cancer treatments, including radiotherapy (RT) and chemotherapy, leads to various bladder complications. The anatomical proximity of the treatment site to the urinary bladder primarily explains the complications following RT, while chemotherapy contribute to bladder dysfunction through systemic mechanisms. This study systematically reviews the nature, extent, and prevalence of bladder complications among women treated for these malignancies, underscoring the influence of treatment modalities on bladder function. METHODS: A comprehensive search of databases including E

Complicated appendicitis with bladder perforation treated with laparoscopic surgery: case report.

Misdiagnosis and delay in the treatment of acute appendicitis increases the risk of complications secondary to visceral perforation. Appendicular rupture into the bladder is an uncommon complication, but possibly to happen; treatment generally consists of a laparotomy with appendectomy and repair of the bladder wall. The laparoscopic approach is a viable option in cases of complicated appendicitis, even in complex and infrequent clinical scenarios. The aim of this paper is to describe a case of complicated appendicitis with bladder perforation, treated with a laparoscopic right hemicolectomy,

Adaptations for Urogynecologic Surgery in Limited-Resource Settings: How to Do More with Less.

INTRODUCTION AND HYPOTHESIS: Pelvic floor disorders affect up to 50% of women in limited-resource settings (LRS) but are severely under-treated. Historically, attention has focused on urogenital fistulae, but pelvic organ prolapse (POP) is emerging as a growing issue, especially for women engaged in manual labor. Women in these regions often endure their conditions in silence owing to social stigma and mental health impacts, compounded by health care access barriers. Delivery of urogynecological services in LRS requires adaptable surgical models and skills because of limited tools and equipmen

Fetal Presence in the Bladder Detected on Ultrasound: A Case Report.

Vesicouterine fistula is a rare kind of fistula with significant complications. In this report, we present a pregnant 39-year-old woman (gestational age: 20 weeks) who had symptoms of hematuria and abdominal pain. In the pelvic examination, she had some fetal tissue exiting from the cervix, leading to misdiagnosis of preterm labor. Further ultrasonographic examination showed that the fetus's body was detached from the head and moved through the fistula to the bladder. This case highlights the importance of assessing the bladder in women with a history of cesarean deliveries and hematuria, even
