Extraperitoneal Rupture of a Bladder Diverticulum and the Role of Multidetector Computed Tomography Cystography.

Nontraumatic rupture of the bladder is less widely recognized than traumatic rupture, with a challenging early diagnosis due to high variability in clinical presentations. We report a case of extraperitoneal rupture of a bladder diverticulum in a patient with diabetes mellitus who presented with paralytic ileus. Despite conservative management, the patient developed sepsis requiring surgical treatment. Urinary tract infection and bladder outlet obstruction were considered to be potential mechanisms of the rupture. Multidetector computed tomography cystography should be used as the first-line m

Non-traumatic bladder rupture showing less than 10 Hounsfield units of ascites.

AIM: Bladder ruptures are commonly misdiagnosed as gastrointestinal perforations or intestinal ischemia. If a diagnosis is made preoperatively, conservative treatment is a safe and effective option. We evaluated the validity of using the attenuation value of ascites, measured by non-contrast computed tomography (CT), to identify patients with bladder ruptures. METHODS: A retrospective search of our hospital database identified 7 patients with confirmed bladder ruptures between 2007 and 2013. We also enrolled 18 patients with gastrointestinal perforations and 10 patients with intestinal ischem

Asymptomatic ureteral rupture secondary to chronic urinary retention from massive prostatic enlargement.

Non-traumatic ureteral rupture has been reported more frequently, resulting from increased intraluminal pressures from distal urinary tract obstruction. We report the case of a 77-year-old man presenting with chronic urinary retention secondary to massive prostatic enlargement through acute kidney injury. Ultrasound scan detected a shallow left perinephric fluid collection with a possible bladder mass, demonstrated on flexible cystoscopy to be a massive median lobe of prostate. Computed tomography confirmed extravasation of urine from the left proximal ureter. In the absence of specific sympto

Spontaneous bladder perforation unrelated to trauma or surgery.

Urinary bladder ruptures (UBR) typically result from either blunt or penetrating trauma, or from iatrogenic surgical injuries. Patients typically present with symptoms including lower abdominal pain, haematuria, dysuria and anuria. Here, we report on a rare case of spontaneous bladder perforation. A 60-year-old, Caucasian woman initially presented with lower abdominal pain and diarrhoea, and was subsequently found to have an elevated serum creatinine level. A CT cystogram revealed a leak from the bladder. The patient consented to exploratory laparotomy and repair of the non-traumatic bladder p

A Case of Post-radiotherapy Urethral Stricture with Spontaneous Bladder Rupture, Mimicking Obstructive Uropathy due to Cancer Metastasis.

Non-traumatic, spontaneous urinary bladder rupture is a rare complication of urethral stricture. Furthermore, its symptoms are often nonspecific, and misdiagnosis is common. The authors experienced a case of urethral stricture with spontaneous bladder rupture and bilateral hydronephrosis, mimicking obstructive uropathy attributed to cancer metastasis. A 55-year-old woman was admitted with abdominal pain and distension, oliguria, and an elevated serum creatinine level. She had undergone radical hysterectomy for uterine cervical cancer and received post-operative concurrent chemoradiation therap

Percutaneous transhepatic gall bladder drainage: a better initial therapeutic choice for patients with gall bladder perforation in the emergency department.

OBJECTIVES: To investigate clinical features and outcomes in patients with acute cholecystitis with gall bladder perforation receiving open cholecystectomy or percutaneous transhepatic gall bladder drainage in the emergency department. METHODS: From 1996 through 2005, 33 patients with non-traumatic gall bladder perforation, among 585 patients with acute cholecystitis, were enrolled. Patients were divided into two groups: open cholecystectomy in 16 patients and percutaneous transhepatic gall bladder drainage in 17 patients. Medical records, including demographic data, past history of systemic

Primary malignant melanoma of the bladder.

AIM: The aim of the present study was to elucidate the role of high temperature requirement A2 (HtrA2) in germ cell loss in the heat-stressed testis. METHODS: We examined the expression of HtrA2, caspase-9 activity and proteolytic activity of HtrA2 in the rat testis, and their in vivo responses to experimental cryptorchid treatment. RESULTS: Northern analysis revealed the expression of HtrA2 mRNA peaked at days 1 and 7 after cryptorchid treatment. While expression of HtrA2 mRNA was seen in the spermatogonium, spermatocytes and some spermatids in normal adult rat testis, experimental cryptorc

Spontaneous hemorrhage of a spinal ependymoma: a rare cause of acute cauda equina syndrome: a case report.

STUDY DESIGN: An interesting case of spontaneous bleeding from an ependymoma of the filum terminale is presented. OBJECTIVES: To document a rare case of rapid neurologic deterioration as a consequence of spinal tumoral hemorrhage and to highlight important issues regarding tumor histologic subtype and the impact of anticoagulation that have emerged from an illuminating review of the literature. SUMMARY OF BACKGROUND DATA: To our knowledge, there have been only 8 reported cases of hemorrhage from an ependymoma of the filum terminale or conus medullaris causing acute cauda equina syndrome. [Spontaneous bladder perforation due to eosinophilic cystitis: a case report].

We report a case of eosinophilic cystitis the onset of which was characterized by acute peritonitis secondary to a spontaneous intraperitoneal rupture of the vesical cupula. The patient was treated with urgent partial cystectomy in another hospital and 3 months later he underwent endoscopic diathermic coagulation of a residual inflammatory lesion at our institution. After an 18 month endoscopic follow-up no further signs of recurrent eosinophilic cystitis have been pointed out. The non-traumatic bladder perforation and the absence of any other bladder pathology might indicate that eosinophilic

Spontaneous intraperitoneal rupture of the bladder.

Spontaneous intraperitoneal rupture of the bladder (also called unsuspected, idiopathic, or non-traumatic rupture of the bladder) is a well defined but rare condition which has not been universally recognized in the past. We are reporting four cases and emphasizing its clinical picture so it might be better recognized and diagnosed early on in the process, and so that its management be planned appropriately in order to reduce its morbidity and its mortality. A review of the literature is provided to help achieve this end. Spontaneous intraperitoneal vesical rupture is almost always a surprise

Recurrent spontaneous bladder rupture. A case report.

Spontaneous (non-traumatic) intraperitoneal rupture of the bladder in a 37-year-old pregnant woman was repaired at laparotomy. Nine months later she again presented with an intraperitoneal bladder rupture during pregnancy and fatal sepsis. Histological examination of the resected bladder wall showed acute ulcerative and necrotising cystitis. All patients with apparently spontaneous bladder rupture should undergo full urological evaluation to identify possible disease which might lead to recurrent rupture.

[Spontaneous rupture of the urinary bladder].

A case of non-traumatic rupture of the urinary bladder is reported. The patient, a 52-year-old woman, was admitted to our clinic with the chief complaints of dysuria and gross hematuria after hemorrhoidectomy. Ultrasonography and CT scan showed a huge mass in Retzius' cavity. Then, rupture of the bladder was diagnosed through subsequent cystoscopy and cystography. During surgery, we recognized a large defect in the anterior wall of the urinary bladder associated with reddish-yellow fluid in the pelvic cavity. Histological findings showed no malignant change in the bladder. Various factors were

Nontraumatic spontaneous rupture of the urinary bladder.

Rupture of the urinary bladder without a history of recent trauma or instrumentation is rare; it is usually associated with bladder disease or outlet obstruction. The patient is usually seen as an emergency case and has an atypical clinical picture. The four patients in this report were initially seen and treated by a general surgeon; in only two was the diagnosis established preoperatively and confirmed by retrograde cystography. Awareness on the part of the consulting surgeon that bladder rupture is possible in a predisposed patient may lead to a correct preoperative diagnosis. In this serie

[CT findings in acute spinal epidural hematoma caused by a ruptured cavernous angioma].

A case of acute spinal epidural hematoma due to the rupture of cavernous angioma is reported. A 68-year-old man was admitted to our hospital with a complaint of hematoemesis. After the successful treatment of bleeding from a gastric ulcer by using endoscopical method, he noticed severe motor weakness in his lower extremities. Complete paraparesis of his lower limbs, total sensory loss below the level of fifth thoracic vertebrae, and bladder disturbance were revealed on neurological examination. A metrizamide myelogram showed complete block at the level of fourth thoracic vertebrae. A computed

Non-traumatic intra-hepatic rupture of the biliary tree. Report of a case.

Non-traumatic extra-hepatic rupture (NTEHR) of the bile system into free peritoneal cavity is a well-known condition. Usually the gall-bladder is the site of perforation, but non-traumatic perforation of the common bile duct and the hepatic duct occurs. For the hepatic duct it is extremely rare and has so far been reported in only 7 cases in adults (Christensen, Baekgaard & Blichert-Toft, 1977; MacWilliams, 1912; Reich, 1942). A review of the literature reveals, that non-traumatic intra-hepatic rupture (NTIHR) in the biliary tree with perforation into free peritoneal cavity has apparently neve

Idiopathic rupture of the bladder.

Non-traumatic spontaneous rupture of the otherwise normal bladder is a rare occurrence. The literature is reviewed and 4 new cases are reported. Experimental data are presented and a theory is reviewed regarding the frequent intraperitoneal location of the rupture. The frequent paucity of symptoms compared to the high mortality rate is stressed.
