Histological Subtypes and Divergent Differentiation of Urothelial Carcinoma: Histology, Genomics, and Management Implications.

BACKGROUND AND OBJECTIVE: Histological subtypes and divergent differentiation are common in bladder cancer. Each subtype has distinct biology and clinical features. We aim to summarize pathology, molecular features, and outcomes of the most common bladder cancer subtypes to guide management. METHODS: We performed a narrative review of cohort studies, clinical trials, and population-based analyses assessing morphology, immunophenotype, genomic alterations, and outcomes following various localized and systemic therapies in patients with bladder cancer subtypes. KEY FINDINGS AND LIMITATIONS: Hi

Patient Trust and Medical Mistrust Toward Overactive Bladder Therapies: General Consumer Awareness and Perceptions.

INTRODUCTION: Overactive bladder (OAB) is a prevalent condition associated with impaired quality of life, yet many patients do not pursue treatment or minimally invasive therapies (MITs). Physician-patient concordance has been proposed as a factor influencing trust, although its role in OAB care is not well defined. We evaluated the association between physician gender concordance and patient trust, and examined the relationship of trust with health literacy, medical mistrust, and treatment preferences for OAB among broad US consumers. METHODS: In this survey-based study, Black and White wome

A Review of Minimally Invasive Therapies for Men with Overactive Bladder.

PURPOSE OF REVIEW: This manuscript aims to provide an overview of minimally invasive treatment for overactive bladder (OAB) in men, highlighting how concomitant bladder outlet obstruction in men can complicate management and lead to underutilization of these therapies in this patient population. RECENT FINDINGS: The 2024 AUA/SUFU guidelines now allow practitioners to offer minimally invasive therapy as an upfront treatment for OAB. Recent studies have closely examined the efficacy of several key procedures, including intradetrusor onabotulinumtoxin-A (BTX-A), sacral neuromodulation (SNM), and

The Role of Biomarkers in the Management of Muscle-Invasive Bladder Cancer.

Muscle-invasive bladder cancer (MIBC) management is evolving beyond traditional clinicopathologic staging to include integration of molecular biomarkers. DNA damage repair gene alterations predict cisplatin sensitivity, while fibroblast growth factor receptor mutations identify candidates for targeted therapy. Transcriptomic subtypes reveal tumor phenotypes with variable treatment responses. Most significantly, circulating tumor DNA (ctDNA) enables dynamic disease monitoring, with postoperative ctDNA detection identifying high-risk patients who benefit from adjuvant immunotherapy while ctDNA-n

Metastatic muscle invasive bladder cancer in pregnancy.

We present the case of a woman in her 30s presenting in the second trimester with frank haematuria and left flank discomfort. MRI revealed a T3b bladder tumour with suspected peritoneal invasion. Chemotherapy was contraindicated due to teratogenicity. A coordinated obstetrics-urology MDT was undertaken at 33 weeks, an elective caesarean section was performed immediately followed by a transurethral resection of bladder for tumour debulking and histological assessment. A preterm but healthy infant was delivered and subsequently discharged from NICU. Histology revealed grade 2 bladder adenocarcin

Prehabilitation and Patient Optimization for Cystectomy.

Prehabilitation is often defined as the process of enhancing or optimizing a patients' functional capacity preoperatively to improve tolerance of perioperative physiologic stressors. This is accomplished beginning with a standardized risk assessment using laboratory values, imaging, and validated questionnaires. Following this risk assessment, patients are referred to a team of multidisciplinary providers with a goal of optimizing nutritional status, exercise capacity, cessation of smoking, and enabling psychiatric preparedness for the stressors of radical cystectomy. Herein, we provide a crit

Open and Robotic Radical Cystectomy for Muscle-Invasive Bladder Cancer: Techniques, Outcomes, and Evidence.

This review examines the contemporary debate between open radical cystectomy and robotic-assisted radical cystectomy (RARC) for muscle-invasive bladder cancer. Drawing on randomized trials, including RAZOR, iROC, and MSKCC studies, we compare perioperative, oncologic, diversion-related, cost, and patient-reported outcomes. Current high-level evidence demonstrates oncologic noninferiority of RARC, with reduced blood loss but longer operative times and higher costs. Outcomes are strongly influenced by surgeon experience and institutional volume rather than approach alone. We highlight considerat

What Is the Proper Extent of Lymph Node Dissection During a Radical Cystectomy for Bladder Cancer?

Radical cystectomy with pelvic lymph node dissection remains the standard of care for muscle-invasive and high-risk nonmuscle-invasive bladder cancer. Lymphatic mapping studies demonstrate that primary drainage traverses the true pelvis, with most metastatic nodes located below the aortic bifurcation. Two randomized trials failed to show a survival benefit from extended dissection, though both were limited by enrolment of lower-risk populations and underpowered to detect true treatment effects. A risk-adapted approach is recommended, reserving extended dissection for higher-risk patients while

Trimodal Therapy for Muscle Invasive Bladder Cancer: An Update on Current Practices and Future Directions.

Bladder preservation utilizing maximal transurethral resection of the bladder tumor, radio sensitizing chemotherapy, and targeted pelvic radiation (known as Trimodal Therapy (TMT) is a validated treatment alternative for muscle invasive bladder cancer (MIBC) for appropriately selected and counseled patients. Long term results of the original studies that described TMT and continuing advancements in radiation techniques and chemo/immunotherapy regimens support its place in international treatment guidelines as more than a second line therapy for cystectomy ineligible patients. This review will

The Role of Surgery in Locally Advanced and Oligometastatic Bladder Cancer.

The role of surgery in the multimodal management of locally advanced and oligometastatic bladder cancer remains critical but incompletely defined. While current evidence is predominantly derived from retrospective series and institutional experiences, both metastasectomy and consolidative lymphadenectomy have demonstrated potential survival benefits in carefully selected patients following systemic therapy. However, interpretation of these data is hampered by the absence of standardized definitions, small and heterogeneous patient cohorts, and a lack of prospective, randomized trials. Ongoing

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

Surrogacy of intermediate clinical endpoints for overall survival in BCG-naïve high-risk non-muscle-invasive bladder cancer.

INTRODUCTION: Recent trials in BCG-naïve high-risk non-muscle-invasive bladder cancer (HR-NMIBC) have added immunotherapy to BCG, often using event-free survival (EFS) as the primary endpoint. However, neither EFS nor progression-free survival (PFS) has been validated as a surrogate for overall survival (OS) in this setting. METHODS: We retrospectively analyzed data from 18 international centers, including patients with BCG-naïve HR-NMIBC treated between 2001 and 2025. Adequate BCG exposure (International Bladder Cancer Group criteria) was the treatment contrast for its effects on intermediat

Mechanisms of microbiome-immune interactions in bladder cancer and targeted regulatory strategies.

Bladder cancer (BC), a highly prevalent malignancy of the urinary tract, imposes a considerable clinical burden because of its high recurrence rates and a substantial risk of disease progression. Accumulating evidence suggests that microbiome dysbiosis is associated with the initiation and progression of BC through potentially bidirectional interactions. Recent advances in high-throughput sequencing technologies have facilitated a more comprehensive characterization of the urinary microbiome, thereby revealing its potential involvement in the pathogenesis of BC. This review summarizes the prop

Neoadjuvant Therapy in Muscle-Invasive Bladder Cancer: Current Evidence, Evolving Strategies, and Future Directions.

Neoadjuvant therapy is now a cornerstone of muscle-invasive bladder cancer management, evolving from traditional cisplatin-based chemotherapy to novel immunotherapy and antibody-drug conjugate combinations. While dose-dense methotrexate, vinblastine, doxorubicin, and cisplatin remains the cytotoxic standard, the phase III NIAGARA and KEYNOTE 905 trials have established new benchmarks for cisplatin-eligible and ineligible populations, respectively. Furthermore, the integration of molecular subtyping, DNA damage repair biomarkers, and liquid biopsies (circulating tumor DNA) is facilitating a shi

Adjuvant Therapy in Patients after Cystectomy.

Adjuvant cisplatin-based chemotherapy is standard of care for patients with muscle-invasive bladder cancer who have received no prior neoadjuvant therapy, are cisplatin-eligible, and have at least pT3 or lymph node-positive disease at the time of radical cystectomy. Carboplatin is inferior to cisplatin in bladder cancer and has not been extensively studied in the adjuvant setting. Adjuvant nivolumab and adjuvant pembrolizumab are recommended by National Comprehensive Cancer Network (NCCN) guidelines for patients with high-risk pathology on radical cystectomy.
