A Prospective Observational Real-World Study to Assess the Effectiveness and Safety of Single-Inhaler Triple Therapy with Glycopyrrolate/Formoterol Fumarate/Budesonide in Patients with Uncontrolled Asthma.

OBJECTIVES: Individuals with asthma remain inadequately controlled despite treatment with inhaled corticosteroid and long-acting β2-agonist (ICS/LABA) therapy. Single-inhaler triple therapy (SITT) combining ICS, LABA, and long-acting muscarinic antagonist (LAMA) offers a practical treatment escalation option. This study aims to assess the real-world effectiveness, safety, and adherence of SITT with budesonide/glycopyrrolate/formoterol fumarate in adults with uncontrolled asthma. MATERIALS AND METHODS: This multicentric, prospective, observational study was conducted across five centers in Ind

A quasi-experimental study evaluating the effectiveness of a male-specific tuberculosis screening and diagnosis intervention in Uganda (IGNITE).

BACKGROUND: Despite men having a higher burden of TB, they are frequently missed by TB services due to social, behavioural, and health system factors that limit healthcare access. In Uganda, where there are no gender-specific guidelines for TB, we implemented and evaluated a gender-specific intervention to find and treat men with TB. METHODS: We conducted a quasi-experimental study comparing TB notifications before (January-June 2023) and after (July-December 2023) the intervention at both intervention (Gombe and Mityana hospitals) and control (Luwero and Kiboga hospitals) sites. The interven

Antimicrobial activity of twelve plant-derived compounds against fish-associated non-tuberculous mycobacteria.

Nontuberculous mycobacteria (NTM) are important pathogens of fish, causing chronic infections that are difficult to control in aquaculture. In this study, we evaluated the in vitro antimicrobial activity of twelve plant-derived compounds against six fish-associated NTM strains using a broth microdilution assay. Minimum inhibitory concentrations (MICs) ranged from 0.25 to >32 µg/mL. Our results demonstrated that cinnamaldehyde exhibited the strongest antimycobacterial effect, followed by sesamol and farnesol, while several other phytochemicals showed moderate activity and salicylic acid was ina

The Potential Role of Gut Microbiota in Combating Mycobacterium tuberculosis Infection.

Tuberculosis (TB) is a chronic infectious disease caused by Mycobacterium tuberculosis (M.TB) infection. Although there have been significant advances in the treatment of M.TB infection, it still faces significant challenges. The gut microbiota is a highly complex ecosystem that plays an important role in regulating metabolism, immune regulation, and maintaining healthy balance in the human body. More and more studies have found that while gut microbiota regulates the immune regulation process of pulmonary TB through the "gut-lung axis," infection with M.TB can also affect the microbial stabil

Not All Extrapulmonary Tuberculosis Is Equal: Site-Specific Mortality in a National Indian Cohort.

Among 777,512 Indian adults with extrapulmonary tuberculosis, in-treatment mortality varied over tenfold by site, highest for tuberculous meningitis (adjusted odds ratio 12.48 vs. lymph node TB), miliary (5.17), and pericardial TB (3.86). Associations of HIV, diabetes, alcohol, and tobacco use with mortality varied across sites. Risk-stratified, comorbidity-aware care is warranted.

Disseminated MDR tuberculosis manifesting as Pott's disease with tuberculous septic ankle arthritis: an uncommon sequential dual-site involvement.

Disseminated osteoarticular TB is uncommon, and the occurrence of multidrug-resistant (MDR) strains impacting many skeletal locations in an immuno-competent individual is very rare. A 25-year-old male from a TB-endemic rural region is presented, having a three-month progression of worsening low back pain, subsequently accompanied by lower limb weakness. GeneXpert from both sites detected Mycobacterium tuberculosis that was resistant to rifampicin. The line probe assay detected resistance to rifampicin, high-level resistance to isoniazid, and resistance to ethambutol. After surgical procedures

Psychological violence, subjective well-being, and burnout among healthcare workers: The roles of sleep disturbance and perceived occupational infection risk.

BACKGROUND: Psychological violence (PCV), which includes verbal abuse, bullying, intimidation, sexual harassment and ethnic discrimination, is the most frequently reported form of workplace violence in health care and a well-documented correlate of burnout. Healthcare workers (HCWs) in infectious disease services face this psychosocial hazard alongside two further, structurally persistent conditions: impaired recovery through disturbed sleep, and the appraised risk of acquiring infection at work. How these demands combine with workers' personal psychological resources is not well understood.

The perceived impact of extreme weather events on care for people with HIV or tuberculosis and HIV in Zimbabwe: a survey-based multicentre cross-sectional observational study and formative evaluation.

BACKGROUND: The growing threat to health systems posed by extreme weather events (EWEs) disproportionately affects people living with HIV and tuberculosis disease in high-burden, low-resource settings. In Zimbabwe, for example, the 2023-24 El Niño-induced drought intensified food insecurity and reduced access to clinics and medication for such vulnerable populations. We aimed to conduct a formative evaluation to assess the perceived impact of climate change and EWEs on HIV and tuberculosis determinants and service delivery and identify potential climate-adaptive health-service strategies. Nano-NucleOTAC-mediated Z-DNA transformation disrupts mycobacterium tuberculosis biofilms and activates antibacterial immunity.

The biofilms of Mycobacterium Tuberculosis (Mtb) use extracellular DNA (eDNA) as a protective structural scaffold that shields resident bacteria from host immunity and antibiotics, promoting persistent infection. An FDA-approved DNase can partially hydrolyze eDNA but has limited affinity and nucleolytic activity against Z-form DNA (Z-DNA). After confirming the abundance of Z-DNA in Mtb biofilms, we develop Nano-NucleOTAC, a nano-nucleolysis targeting chimera that combines an eDNA-binding ligand (lactoferrin), a DNA intercalator (chloroquine) and DNase. Nano-NucleOTAC anchors to Z-DNA in Mtb H3

Prognostic utility of immune biomarkers for identifying individuals at risk of unfavourable treatment outcomes in pulmonary tuberculosis.

INTRODUCTION: Tuberculosis remains a major global health challenge, with many patients expe- riencing unfavourable treatment outcomes despite standard therapy. Host immune biomarkers, including cytokines, chemokines, and matrix metalloproteinases (MMPs), have shown promise in predicting disease severity and treatment response, but require validation across independent cohorts. METHODS: We validated previously identified immune biomarkers using baseline plasma samples from a nested case-control cohort within the RePORT India consortium (cases: n=74; controls: n=161). Disseminated Tuberculosis With Ocular Involvement After 12 Years of Anti-TNF-α Therapy.

BACKGROUND: Ocular tuberculosis (TB) is an uncommon yet potentially vision-threatening manifestation of Mycobacterium tuberculosis, particularly difficult to diagnose in immunocompromised individuals. The increasing use of anti-tumor necrosis factor-α (anti-TNF-α) agents has markedly heightened the risk of TB reactivation, often with disseminated or extrapulmonary involvement despite negative baseline screening. CASE PRESENTATION: We describe a 52-year-old woman with rheumatoid arthritis receiving long-term adalimumab and methotrexate who developed disseminated TB with pulmonary, splenic, and

A Whole-blood Transcriptional Signature Associated With Obstructive Post-tuberculosis Lung Disease.

BACKGROUND: Post-tuberculosis (TB) lung disease (PTLD) affects ∼50% of persons with pulmonary TB (PTB). We recently discovered whole blood transcriptional signatures associated with PTLD. We examined whether a minimum gene signature predicts PTLD as a clinically useful biomarker. METHODS: We prospectively enrolled 301 treatment naive adults with newly diagnosed PTB (cohort A). We collected whole blood at 0 and 6-month visits, isolated RNA, and measured a modified MTB host response (HR) signature (mHR) based on expression of DUSP3, GBP5, and TMBIM6. We recorded spirometry at 6 (n = 216) and 12

18F-FDG PET/CT and post-treatment lung function in adolescent tuberculosis.

BACKGROUND: Data on post-tuberculosis outcomes in adolescents are scarce. Although fluorine-18 fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) has been used in adult tuberculosis research, little is known about the association between PET/CT findings and functional outcomes in general. METHODS: Early FDG PET/CT images of 50 adolescents (31 female, median age 17.2 years) with confirmed pulmonary tuberculosis were read visually and quantitative data of disease burden (total lung volume, cavity volume, total hard volume and total glycolytic activity (TGA))

Catalysis and allostery in bacterial ATP phosphoribosyltransferases.

ATP phosphoribosyltransferase (ATPPRT) catalyses the first reaction of L-histidine biosynthesis: the condensation of ATP and 5-phospho-α-d-ribose 1-pyrophosphate to form N1-(5-phospho-β-d-ribosyl)-ATP with the release of inorganic pyrophosphate. L-Histidine allosterically inhibits ATPPRT as a mechanism to regulate this energy-intensive pathway in histidine-synthesising organisms, such as bacteria, archaea and plants. Absent in humans and essential in many disease-causing bacteria, such as Mycobacterium tuberculosis and Acinetobacter baumannii, ATPPRT is a promising drug target for novel antibi

Birth and clinical outcomes of pregnant participants with advanced HIV disease.

INTRODUCTION: Clinical trials for the evaluation of novel diagnostics and treatments for HIV-associated opportunistic infections have historically excluded pregnant persons. Little information is available regarding the clinical outcomes and pregnancy outcomes of pregnant persons with advanced HIV disease while taking opportunistic infection prophylaxis for tuberculosis (TB) and cryptococcosis. In this study, we describe characteristics at enrollment, clinical outcomes, and birth outcomes of pregnant people with advanced HIV in Uganda. Objective rest-activity rhythm amplitude and lung function in US adults: A cross-sectional analysis of the 2011-2012 National Health and Nutrition Examination Survey.

This cross-sectional analysis of 4,150 US adults from the 2011-2012 NHANES examined associations between wrist-accelerometry-derived rest-activity rhythm metrics and respiratory outcomes spanning spirometry, exhaled nitric oxide, and self-reported respiratory symptoms and diagnoses. Relative amplitude (RA), interdaily stability, intradaily variability, M10, L5, and cosinor parameters were derived from hourly monitor-independent movement summary (MIMS) values. The co-primary outcomes were FEV1 and FVC z-scores (GLI 2012); secondary outcomes included FEV1/FVC, FeNO, and latent-class-derived resp
