Beyond the Central Nervous System: Neisseria meningitidis as an Unusual Cause of Urogenital Infection.

Neisseria meningitidis is a well-known cause of invasive diseases such as meningitis and septicemia, but is increasingly recognized as an emerging urogenital pathogen. We report the case of a 36-year-old man who presented to a tertiary hospital in Riyadh, Saudi Arabia, with persistent dysuria and purulent urethral discharge. Despite initial treatment for nonspecific urethritis and subsequent management for chronic prostatitis with levofloxacin and azithromycin, his symptoms persisted. Although standard multiplex polymerase chain reaction (PCR) assays for common sexually transmitted infections

Testing for 'threads' and leucocyte esterase in first-void urine to exclude the diagnosis of non-specific urethritis in asymptomatic men.

Recent evidence suggests that asymptomatic nonspecific urethritis (NSU), which is not routinely tested for, is a clinically significant pathology. The aim of this pilot study was to determine if testing for urinary threads, leucocyte esterase (LE) or both in asymptomatic men is a good screening tool for NSU. Of the126 asymptomatic men, 8% met microscopic criteria for the diagnosis of NSU. The positive predictive value for NSU was 71% (95% confidence interval (CI): 29.3-95.5%) and the negative predictive value was 96% (95% CI: 92.8-99.5%). The absence of threads and negative LE makes urethritis

The urethral smear as a tool in diagnosing adenovirus-induced urethritis.

Adenovirus is a recognised cause of non-gonococcal urethritis, and is not uncommonly associated with extragenital signs and symptoms. This case report describes a patient with symptoms of conjunctivitis, meatitis and urethritis. The urethral smear revealed almost exclusively monocytes microscopically, raising the suspicion of a viral aetiology. Results confirmed the presence of adenovirus in both the eyes and urethra. Despite waning reliance on the urethral smear in sexual health clinics, it can still be an important diagnostic tool in assessing the aetiology of non-specific urethritis. Findin

Sexually transmitted infections and male circumcision: a systematic review and meta-analysis.

The claim that circumcision reduces the risk of sexually transmitted infections has been repeated so frequently that many believe it is true. A systematic review and meta-analyses were performed on studies of genital discharge syndrome versus genital ulcerative disease, genital discharge syndrome, nonspecific urethritis, gonorrhea, chlamydia, genital ulcerative disease, chancroid, syphilis, herpes simplex virus, human papillomavirus, and contracting a sexually transmitted infection of any type. Chlamydia, gonorrhea, genital herpes, and human papillomavirus are not significantly impacted by cir

Sexual health and the Rugby World Cup 2011: a cross-sectional study of sexual health clinics in New Zealand.

BACKGROUND: We aimed to describe the characteristics of sexual health clinic (SHC) attendance and sexually transmissible infection (STI) diagnoses during the Rugby World Cup (RWC) in New Zealand in 2011. METHODS: SHC attendance and STI diagnoses around the time of the RWC were compared with the 5 preceding years. A cross-sectional survey conducted at SHCs in four New Zealand cities collected information from attendees having RWC-related sex. RESULTS: Although there was no statistically significant increase in clinic attendance or STI diagnoses during the RWC compared with previous years, in

Potential impact of human papillomavirus vaccines on public STD clinic workloads and on opportunities to diagnose and treat other sexually transmitted diseases.

BACKGROUND: Eradicating genital warts through HPV immunization could decrease STD clinic utilization but may result in missed opportunities to diagnose other STDs. OBJECTIVES: To define the proportion of STD clinic visits attributable to HPV and to describe the prevalence of other STD diagnoses among visits for HPV-related presenting concerns. STUDY DESIGN: Cross-sectional analysis of medical records (1994-2004) from a single STD clinic. Prevalences of STDs were calculated for male and female patients with and without HPV-related presenting concerns. RESULTS: Of the 66,537 visits included i

Microbiological features of persistent nonspecific urethritis in men.

BACKGROUND AND PURPOSE: The role of opportunistic microorganisms in urethritis in men is controversial. The aim of this study was to determine the changes in urethral microflora in the presence of persistent nonspecific urethritis (PNU) in men by in vitro detection of decomplementary activity (DCA) phenotypes of urethral isolates and comparison of the isolates with those from patients with or without PNU. METHODS: The study included 18 healthy men and 24 men with PNU. Culture specimens were spread on various selective media. Bacterial DCA was tested by measuring the decrease in complement act

Clinical patterns of sexually transmitted diseases, associated sociodemographic characteristics, and sexual practices in the Farwaniya region of Kuwait.

BACKGROUND: Sexually transmitted diseases (STDs) are common worldwide, but there are few studies available on STDs from the Gulf countries, including Kuwait. Our goal was to determine the clinical patterns, sociodemographic factors, and sexual practices of patients with STDs who attended a government hospital in the Farwaniya region of Kuwait. METHODS: All patients seen over a 1-year period (July 2003 to June 2004), who presented with signs and symptoms suggestive of STDs, were included. Sociodemographic details and clinical findings were recorded. The diagnosis of each STD was based mainly o

Nonspecific urethritis and reactive arthritis.

Nongonococcal urethritis (NGU) is a common sexually transmitted infection most often caused by Chlamydiae and Mycoplasmae. A few other organisms, as well as some nonsexual factors, also contribute to its etiology. NGU can result in considerable physical and psychological morbidity. In a few cases, it can lead to complications like reactive arthritis, and pelvic inflammatory disease, with their attendant morbidities. Correct diagnosis and treatment is therefore imperative in proper management of these cases. While earlier diagnostic support for these infections was limited and expensive, the ad

Lack of association between long-term illness and infectious intestinal disease in New Zealand.

AIMS: To investigate whether the increase in notified cases of infectious intestinal disease in New Zealand from 1988 to 2001 has resulted in a concurrent increase in associated secondary illness cases. METHODS: National surveillance system data were compared to hospital discharge data. RESULTS: No statistically significant correlation between the number of cases of campylobacteriosis and Guillain-Barre syndrome (GBS) was found. There was no statistically significant correlation between the number of cases of campylobacteriosis, salmonellosis, shigellosis, and any of the categories of reacti

The Italian STD 'Hot-Line'. Making people aware.

BACKGROUND: To increase the accessibility of qualified and anonymous information on sexually transmitted diseases (STDs) in Italy, a national telephone hotline was initiated in February 1997. GOAL: The goal was to determine the profile of callers to the national STD hotline, assess their concerns, identify their sources for the hotline telephone number, and to compare callers' disease interests with diagnoses made at STD clinics. STUDY DESIGN: The survey analyzed 3577 calls received from February 1997 to December 1999. Hotline operators addressed callers' questions, asked 6 short-answer ques

[Characteristics of changes in microbiocenosis in patients with chronic nonspecific urethritis].

The species composition and the complex of biological characteristics of microflora in the front section of urethra in healthy males and in patients with chronic nonspecific urethritis. The study revealed that in patients with chronic nonspecific nongonococcal urethritis changes in the microbiocenosis of the urethra were observed. These changes were manifested by a decrease in the number of species, the appearance of Grain-negative enterobacterial flora and an increase in the persistence potential of symbiotic bacteria. These disturbances are regarded as the manifestation of urogenital dysbios

[Analysis of the etiologic structure of urinary tract infection and antibiotic-resistance of its pathogens].

The main pathogens of inflammatory diseases of the kidneys and upper urinary tracts in inpatients of an urological unit were gramnegative organisms of the family Enterobacteriaceae while the pathogens of the infection of the lower urinary tracts (nonspecific urethritis) and male genitalia were grampositive cocci. Pseudomonas aeruginosa, Enterobacter agglomerans and Proteus spp. (indole positive) were the chief causative agents of the hospital infections. The analysis of the materials revealed a tendency towards an increase in the microflora resistance to the most widely used antibiotics: amino
