Continuous use of condom catheter for urinary incontinence in a patient with neurogenic bladder gave rise to localized chronic oedema on the dorsum of the penis, mimicking keloid scar, and urethral fistula on the ventral surface. These unusual combined complications on the penile surfaces were due to pressure effect.

The urethral fistulas need for recovery the traditional surgical approach. Recently are available for surgeons substances and devices particularly compatible with biological tissue and very safe. We have taken advantage of this good opportunity to treat an old and multiple fistula of the urethra in the perineum. We are completely satisfied of this, alternative method that lead the patient to recover, avoiding in the same time the risks of another surgical operation too, that is difficult and complicated.

Strangulation of the penis caused by a coil of hair is uncommon. It can produce varying grades of injury that may result in a urethral fistula or even gangrene of the glans. Therefore it is important to be aware of this condition so that the strangulating hair can be removed and treatment instituted. In this article, one-stage repair of complicated cases of penile hypospadias when there is no penile skin available for safe reconstruction is described. An island of the most median hairless part of the scrotal skin around the median raphe was used as a skin flap based on its scrotal septal pedicle with an extended and free arc of rotation for repair of distant urethral defects. Seven paraplegic patients, presenting with urethro-cutaneous fistulae due to a loss 4 to 10 cm of urethral tissue, due to decubitus ulcer of the anterior perineum, were cured. Urethroplasty was performed by wrapping the urinary catheter in a resorbable polyglactin net, covered on top by a flap of a well vascularized musculocutaneous biceps femoralis flap. The follow-up of several years proved the reliability of this technique which can be perfectly applied to others kinds of urethroplasty.

Two cases of an ano-urethral fistula, being a low malformation, are reported herein. Both patients underwent a colostomy under the diagnosis of a recto-urethral fistula during the neonatal period. As a urethro-rectogram later revealed the fistula to be located between the anus and anterior urethra, revision of the urethral fistula and an anoplasty were performed through the perineal approach. It is very difficult to establish a correct diagnosis of an ano-urethral fistula at the initial stage of treatment, but we found the urethro-rectogram invaluable in this regard. Posterior Sagittal Anorectoplasty (PSARP) is a new technique which was pioneered by Pena and de Vries in 1982 for the repair of high anorectal malformations. It is based upon complete exposure of the anorectal region by means of a median sagittal incision that runs from the sacrum to the anal dimple cutting all muscle structures behind the rectum. An infant with the oculo-auriculo-vertebral (OAV) spectrum is described with the previously unreported association of rectal agenesis with recto-urethral fistula. The patient died at the age of 14 weeks from respiratory obstruction. Autopsy revealed the cause of death to be tracheal compression from a pulmonary artery sling. Previous reports of vascular rings within the OAV spectrum are reviewed, and it is concluded that these malformations are an uncommon, but clinically important, complication of the disorder.

Congenital rectourethral or anourethral fistulae without imperforate anus in males are rare, representing less than 1% of anorectal malformations. We report our experience with two males with "N type" urethral fistulae. One, a 5-year-old boy, presented with recurrent urinary tract infections (UTIs) and passage of urine per anus. We treated 33 patients 6 months to 19 years old with penile chordee without hypospadias from 1966 to 1990. In all cases the penile shaft was degloved, the urethra was widely mobilized and chordee was resected. The shaft skin then was closed, usually using Byars' flaps to shift some preputial skin ventrally. This method sufficed to straighten the penis in 10 patients. We report the reconstruction of the urethral fistula using a scrotal septocutaneous island flap. A scrotal septocutaneous island flap is supplied by vascular networks in the scrotal septum area from both perineal arteries and posterior scrotal arteries. Eight cases of closure of urethrocutaneous fistula and 15 cases of urethroplasty for hypospadias are reviewed. Most of the fistulas were present in the penoscrotal level. Fistula and hypospadias opening were closed by re-enforcement using a de-epithelialized scrotal skin flap. Thirty-four children, ten of them coming from another Surgical Center, underwent repair for urethral fistula after hypospadias reconstructive surgery, over a period of three years (1985-1987). After the first operation, there was no recurrence in 59% (20 cases). Currently, all fistulas have been closed, except in 3 cases. Closure by direct suture was used in 30 cases (88%).
