We report a rare case of a 25-year-old man with paraplegia and hemodialysis dependency due to traumatic spinal cord injury and renal infarction. Ischial and severe sacral pressure ulcers complicated by an urethrocutaneous fistula developed in the patient. A multidisciplinary approach was implemented, including percutaneous cystostomy, negative pressure wound therapy, and musculocutaneous flap surgery. This strategy successfully preserved urethral integrity, a significant achievement in such complex cases. After complete ulcer closure and meticulous follow-up, the patient successfully underwent

BACKGROUND: Metoidioplasty is a genital-masculinization surgery for transmen with gender dysphoria that creates a small penis from a hormonally enlarged clitoris, and while standing urination is currently facilitated by urethroplasty using buccal (Belgrade method) or labia-minora (Ring method) mucosal grafts combined with a vaginal flap, these techniques involve buccal wounds and are associated with high urethral-fistula rates.
AIM: This study aims to describe our vaginal-mucosal graft metoidioplasty technique and report early outcomes from a case series of seven patients, based on the hypoth

INTRODUCTION: Urethrocutaneous fistula (UCF) is a common complication after hypospadias repair with an incidence of 5-10%. Several techniques are described for its repair: small UCFs are frequently corrected by isolation, excision, and closure with apposition of a protective second layer. In 2008 Malone described the PATIO technique: the fistula tract is turned inside out in the urethral lumen preventing contact with passing urine without direct urethral sutures.
OBJECTIVE: Aim of our study is to present our outcomes using a modified version of the PATIO technique, with a more reproducible is

PURPOSE: This study aimed to report the outcomes of hypospadias repair performed at a tertiary-level hospital during 2018 to 2021.
METHODS: A retrospective chart review of 119 patients was performed.
RESULTS: The most frequent coronal hypospadias cases were distal. However, the most common scrotal hypospadias cases were proximal. The average age at the time of the first surgery was 2.4 years (standard deviation, ±2.3 years). The tubularised incised plate technique was performed for 57 of these 119 patients. Urethral fistula was the most common complication associated with distal and proximal

We report on a case of a two-year-old male dog, breed chow-chow, who suffered from urethral fistula as a result of ureterolithiasis. The urethral defect was identified intraoperatively with methylene blue. An autologous regenerative approach was combined with surgical closure of the defect, due to the well-known healing issues of the urethral wall in such conditions. A part of abdominal fat tissue was dissected to produce microfragmented adipose tissue containing mesenchymal stem cells, which was combined with platelet-rich plasma. The final product was applied in the area around the urethral

BACKGROUND: Ileoanal pouch is a demanding procedure with many potential technical complications including bladder or ureteral injury, while inflammation or stricture of the anastomosis or anal transition zone may lead to the formation of strictures and fistulae, including to the adjacent urethra. Pouch urinary tract fistulae are rare. We aimed to describe the presentation, diagnostic workup, and management of patients with pouch urinary at our center.
METHODS: Our prospectively maintained pouch registry was queried using diagnostic codes and natural language processing free-text searches to i

Pelvic fracture (PF) is a rare emergency, which led to pelvic fracture urethral injury (PFUI) in 1.6% to 25% of cases. Urethral injury assessment requires a thorough analysis of the initial injury history and imaging that combine cystourethrography acutely, repeat urethral imaging as well as adjunctive use of MRI in the follow-up period. A more complex surgical approach to the delayed reconstruction of PFUI may be predicted by the urethral gap length (cysto-urethrography) or a lower pubo-urethral angle (MRI). Delayed urethroplasty is the treatment of choice, performed once the patient has reco

We describe the first case of a recto-urethral fistula following an MRI-guided transurethral prostate ablation procedure (TULSA). The patient experienced urine per rectum six weeks after the procedure. A voiding cystourethrogram confirmed the presence of a recto-urethral fistula, which was managed with a urethral catheter and a suprapubic tube. Patient was then asymptomatic, with spontaneous healing of the fistula and catheters removed after six weeks. Not previously reported following TULSA, rectourethral fistula is a rare but known complication with other focal therapy modalities. Awareness

PURPOSE: Genital gender-affirming surgery in transmasculine patients encompasses both metoidioplasty and phalloplasty. Some patients opt to undergo staged phalloplasty by metoidioplasty first (SPMF). The aim of this study was to evaluate whether SPMF is associated with less surgical complications compared with immediate phalloplasty (IP).
METHODS: Our institutional database was retrospectively evaluated to identify transmasculine patients who underwent SPMF between 2006 and 2020. These patients were matched based on the type of flap (radial forearm free flap vs. anterolateral thigh flap) and

Background: Worldwide, many different surgical procedures and techniques are being used for masculinizing genital gender affirming surgery (gGAS) in transgender and gender diverse (TGD) individuals. Studies reporting on masculinizing gGAS measure and report different outcomes, hindering comparison and thus evidenced-based treatment decision making. This study aims to systematically assess reporting heterogeneity in masculinizing gGAS studies to date. Methods: A systematic review in compliance with the Cochrane Handbook for Systematic Reviews of Interventions was conducted. A multi-database lit

BACKGROUND: The association of Hirschsprung's disease with anorectal malformation (ARM) is both diagnostically and surgically challenging, in addition to it being a rarity. Symptoms attributable to post-repair of ARM may mask the underlying Hirschsprung's disease (HD) and become worse after closure of stoma. We aim to highlight this rare finding, the challenges associated with the management and the consequent multiple surgeries the patient underwent.
CASE PRESENTATION: We report the case of a seven-year-old boy who had a diverting Devine descending colostomy for high ARM and was followed up

Fetal lower urinary tract obstruction is a rare condition commonly associated with significant perinatal morbidity and mortality, particularly when detected in the second trimester. The concurrent presentation of a recto-urethral fistula with membranous urethral atresia and sacrococcygeal teratoma has not been reported. We describe a case of extrinsic fetal lower urinary tract obstruction related to a recto-urethral fistula, diagnosed at 33 weeks gestation and managed by serial vesicocentesis until 38 weeks. This case, notable for its absence of hypoplasia despite late diagnosis, resulted in a

BACKGROUND/PURPOSE: The most frequent complication following hypospadias correction is fistula formation. Recently, fibrin glue has been employed as a 2nd layer covering the urethroplasty resulting in a reduction in the incidence of fistulas. Platelet-rich fibrin (PRF) is an autologous growth factor-rich source. Although the authors recommend the use of additional layers in hypospadias correction, there remains a controversy. In a trial to address this issue, our research was conducted to compare the accessibility of urethroplasty coverage using PRF to traditional local flaps, aiming to preven
