Duplication of the urethra is a rare congenital anomaly. While it is true that in some instances reconstructive surgery is not necessary, those cases that require repair pose a significant challenge to the urologist. Our experience with 3 cases illustrates the potential complexity of this deformity. A 6-month-old boy with high anal atresia was operated upon via a sacro-perineal approach. Any surgeon performing hypospadias repair must be prepared to manage the surgical complications. From January 1976 to December 1982, 176 cases of hypospadias treated with a number of different operations were critically analyzed for complications. There were no complications in 143 cases (81 per cent). However, there were 44 surgical complications in the remaining 33 patients (19 per cent), which included stricture formation (11), urethral fistula (11), severe skin edema (2), subcutaneous hematoma (2), persistent chordee (2), scrotal abscess (1) and bladder calculus (1). There were some problems in International Classification of Anorectal Anomalies. Recto-Urethral Fistula: The level of recto-urethral Fistule and that rectal pouch are different from case to case. Hypertrophied genital folds: this has no value in the diagnosis of low type. Recently the Japan Study Group of Anorectal Anomalies has proposed clinical score, which is based on four parameters, rectal sensation, continence, constipation and soiling. But this score has also some problems. We have proposed a new clinical assessment.

A reliable two stage method of closure of proximal urethral fistulas after hypospadias repair is described. It has been successful in five patients, four of whom had recurrent fistulas after failure of simpler methods of closure.

The University of Missouri-Columbia protocol for localised cancer of the prostate calls for pelvic node dissection, 10000 cGy at the periphery of the prostate from 125I and 4000 cGy in 20 fractions to the whole pelvis using supervoltage X-ray therapy. In spite of the resemblance of this ano-urethral fistula to an "H-type" ano-urethral fistula, which is thought to result from the failure of the division of the cloaca in the embryological period, three abnormalities seen in this case were thought to be the result of a serial event which occurred in the more distal part, the anal and the genital folds, during the embryological period.

Rectoprostatic fistula is a rare complication after transurethral resection of the prostate or prostatectomy for benign and malignant neoplasms of the prostate. Repair of these fistulas is difficult, especially when previous treatment includes radiation therapy to the prostate. Various operative approaches have been described to close these inaccessible fistulas. Because of their location near the outlet of the pelvis, access to or exposure of these fistulas is quite limited. These fistulas can be easily exposed and repaired through the posterior wall of the rectum (transsphincteric approach).

Urethral fistula formation after urethroplasty for hypospadias is a frequent occurrence. Repeated failures can occur even after multiple attempts at repair. A surgical procedure is described using the double-breasted principle to treat these resistant cases. Twenty-four patients were operated on using this technique and have been followed up from 6 to 27 months. There were 3 failures in the earlier cases but these patients were cured by a repeat of the procedure.

Two fistulae were created in each of 26 adult male New Zealand rabbits. In six, no closure was carried out. In 20 rabbits subsequent closure was carried out using, alternately, a macroscopic and microscopic technique. We were then able to evaluate a total of 26 repaired fistulae using several parameters. Patency of the fistulae was present after 3 months in all six rabbits in which no closure was carried out. Comparison of the two techniques showed nine recurrences or persistent fistulae in the ten repaired by standard macroscopic techniques, and one fistulae still present after closure of 16

A case of extensive granuloma inguinale simulating squamous cell carcinoma is described. There was past history of urethritis leading to a urethral fistula. The ulcer healed almost completely within 19 days of receiving streptomycin injections. The patient had associated scabies and presumably also had latent syphillis (His VDRL was reactive in 1:8 dilution). The patient belonged to Madhya Pradesh.
