OBJECTIVES: A urethral caruncle is a common benign vulvar lesion in postmenopausal women. We describe a case of malignant melanoma that originally presented as a caruncle.
METHODS: A 73-year-old female presented to the urogynecology office for prolapse. Initially, she was diagnosed with a urethral caruncle that was noted to be enlarging on a subsequent exam. A biopsy was taken, which returned with poorly differentiated malignant melanoma.

The urethral caruncle is the most common lesion arising from the posterior lip of the urethral meatus in women; however, various benign and malignant tumours may mimic this condition. We report a case of a 74-year-old postmenopausal woman with stage III cystocele and a non-circumferential urethral caruncle presenting with bulge, voiding difficulty, occasional bleeding, and meatal discomfort. A 61y/o woman who presented with a dark, bleeding mass at the urethral meatus, accompanied by dysuria and significant weight loss. Initial diagnosis by a GP suggested a urethral caruncle. Primary urethral melanoma is an exceptionally rare malignancy, comprising a small fraction of melanomas and malignant tumors affecting the female urethra. Urethral caruncles are seen almost exclusively in women. When these lesions are diagnosed in a male patient, he is likely to undergo unnecessary radical treatment mistaking a urethral caruncle for a malignant urethral neoplasm. Hence, a proper histopathological diagnosis is mandatory before deciding the final treatment. We report a case of fleshy masses in the penile urethra of a young male patient. Simple excision was performed and the histopathology revealed a urethral caruncle of the angiomatous variety.
