"I Just Tested So That I Know My Health Status": HIV Testing Motivations and Decision-Making Among Adolescents in Tanzania.

Tanzanian adolescents are vulnerable to HIV and national policy supports HIV testing services (HTS), yet HTS is underutilized by adolescents. This qualitative study explores factors that influence the attitudes and decision-making of Tanzanian adolescents regarding HTS uptake. The VITAA clinical trial tested a school-clinic partnership that offered health check-ups for Tanzanian adolescents with optional HTS. Semistructured interviews (n = 35) were conducted with adolescents (mean age 16) from the VITAA cohort. Thematic qualitative analysis was informed by the Health Belief Model. Most adolesc

An Adapted Intervention Mapping Approach to Scaling Digital HIV Care Through Single-Session Design.

Digital health interventions (DHIs) for HIV prevention have demonstrated feasibility, acceptability, and, to a lesser extent, efficacy, but few have achieved widespread dissemination. We used intervention mapping and single-session design principles to adapt Keep It Up! (KIU!), a CDC-designated best-evidence HIV prevention program originally delivered over 6 months, into a single-session intervention, Keep It Brief! A 13-member expert and community advisory board guided six adaptation steps, prioritizing behavioral outcomes, objectives, and KIU!'s core theoretical determinants. The resulting i

Global spread and evolutionary trajectories of HIV-1 CRF01_AE: Insights from phylogenetic analysis.

HIV-1 circulating recombinant form (CRF) 01_AE has driven major HIV epidemics across Southeast and East Asia for decades, yet the mechanisms underlying its long-term regional spread remain poorly resolved. Using near-complete genome sequences and Bayesian phylogeographic reconstruction, we traced the evolutionary history of CRF01_AE from its origin in Central Africa to its expansion across Asia. Our results uncover a decades-long transmission corridor linking Africa, Thailand, and China, in which CRF01_AE was introduced into Thailand in the early 1980s and subsequently disseminated across Sout

Acción PrEP: A Multi-Method Pilot Design to Assess the Feasibility of Community Co-Designed Implementation Strategies to Advance PrEP Uptake Among Latines in the U.S. South.

INTRODUCTION: Despite systemic inequities in pre-exposure prophylaxis (PrEP) uptake among Latines (a gender-inclusive term for people of Latin American descent, used to promote inclusivity and language justice), clinic-level implementation strategies addressing health equity-related PrEP barriers are underutilized. This multi-method study assessed the feasibility and acceptability of community-co-designed, equity-focused strategies to improve PrEP uptake among Latines in the Southern United States. METHODS: Facilitated by a community-based organization (CBO)/academic partnership, a workgroup

Assessing Fidelity to HIV Index Case Testing Protocols in Malawian Clinical Settings: Results From a Cluster Randomized Controlled Trial.

INTRODUCTION: HIV index case testing is an evidence-based practice in which health care workers (HCWs) counsel people living with HIV (index clients) to offer HIV testing services to their sexual partners and biological children (contacts). For index case testing to be effective, it is important that index- and contact-client counselling protocols are implemented as intended (fidelity). We evaluated whether a digital training implementation strategy improved HCWs' fidelity to these counselling protocols. METHODS: We conducted a cluster-randomized controlled trial at health facilities (cluster

Spatial Heterogeneity of Digital HIV/AIDS Stigma in China: Cross-Sectional Ecological Study.

BACKGROUND: HIV/AIDS stigma remains a major barrier to public health intervention and social inclusion. In China, short-video platforms, such as Douyin, have become important spaces where people living with HIV/AIDS share daily-life narratives and receive public responses. However, little is known about how public responses to these narratives vary geographically, or how regional sociodemographic and epidemiological contexts are associated with digital stigma and support. OBJECTIVE: This study aimed to examine the thematic structure, spatial heterogeneity, and macro-level correlates of public

Impact of the war in Ukraine on HIV-1 subtype distribution and antiretroviral resistance in Slovakia, 2022.

The study analyses the prevalence of HIV-1 subtypes and antiretroviral resistance among newly registered HIV-positive individuals in Slovakia in 2022, with a particular focus on the impact of migration caused by the war in Ukraine. A total of 198 cases were confirmed or reconfirmed HIV-positive of whom 109 (55.1%) citizens were from Ukraine, 80 (40.4%) were from Slovakia, and 9 (4.5%) were from other countries. HIV subtyping and resistance testing were performed in 99 patients (50.0% of all newly registered cases). Among 99 analysed patients, HIV-1 subtype B was detected in 51 (51.5%) and non-

Self-care practices of people living with HIV and hypertension or diabetes comorbidities in low-and-middle income countries: A systematic review.

BACKGROUND: People living with HIV in low-and middle-income countries (LMICs) face a growing burden of hypertension and diabetes comorbidities. Although self-care is essential, limited evidence exists on how patients manage these conditions concurrently within resource-constrained settings. We examined self-care practices among people living with HIV and hypertension or diabetes comorbidities in LMICs and identified treatment burdens related to these practices. METHODS: We searched Medline, EMBASE, PsycINFO, Global Health, Scopus, Web of Science, CINAHL, WHO Global Index Medicus, African Jour

Faith-based organisations in providing HIV services within low- and middle-income countries (LMICs): a systematic review.

BACKGROUND: To end the acquired immune deficiency syndrome (AIDS) epidemic by 2030, the Joint United Nations Programme on HIV/AIDS (UNAIDS) proposes that 95% of people living with HIV should be diagnosed, 95% of those with a diagnosis should be on treatment, and 95% of those on treatment should achieve viral suppression. To attain these targets, UNAIDS estimates that community-based HIV services should increase from 5% to 30%. Within low-and-middle-income countries (LMICs), faith-based organisations (FBOs) often possess broad societal influence and extensive reach. Thus, many major AIDS organi

Optimizing pneumococcal prevention in South African older adults: Cost-effectiveness of PCV20 versus PPSV23 in the context of HIV-associated risk.

Pneumococcal disease remains a major cause of morbidity and mortality among older adults in South Africa, particularly among people living with HIV. Evidence on the cost-effectiveness of Prevnar 20 (PCV20) vs. Pneumovax 23 (PPSV23) vaccine in this setting remains limited. This study was conducted to evaluate the cost-effectiveness of PCV20 compared with PPSV23 vaccine among adults aged ≥65y in South Africa, and to assess the impact of HIV stratification on economic outcomes. A static decision-tree model was developed from the South African public healthcare payer perspective. The analysis inco

Cost-effectiveness of concurrently available oral and long-acting injectable pre-exposure prophylaxis for preventing HIV infections among men who have sex with men in eastern China: A modelling study.

BACKGROUND: China faces a growing HIV burden among men who have sex with men (MSM), yet uptake of pre-exposure prophylaxis (PrEP) remains low. Long-acting injectable cabotegravir (CAB-LA) may improve prevention effectiveness, but its economic value in China remains uncertain. METHODS: We developed a risk-stratified dynamic compartmental model incorporating HIV natural history, diagnosis, ART, and both transmitted and acquired drug resistance. Model parameters were calibrated to published surveillance data in Zhejiang and Jiangsu Province from 2009 to 2019. Assuming zero PrEP coverage in 2025,

Exploring the impact of Chinese medicine on CD4 cell counts in people living with HIV by mixed-effects model: a retrospective cohort study based on propensity score matching.

BACKGROUND: CD4 cells are the targets of human immunodeficiency virus (HIV). If left untreated, HIV enters CD4 cells and replicates, ultimately leading to the death of the infected cells. The loss of these cells impairs immune function. As Chinese medicine (CM) can be used to treat people living with HIV (PLHIV), it is important to explore the long-term effects of CM on the CD4 cell count of PLHIV. METHODS: An open-label study was conducted using data from conventional medical records. PLHIV who met the criteria and were treated with CM (CM group) and not treated with CM (non-CM group) were p

Evaluating Fc-mediated antibody functions against SARS-CoV-2 in PLHIV: reduced FcγRIIIa-dependent activity and ADCC despite preserved neutralising antibodies.

BACKGROUND: Antibody-dependent cellular cytotoxicity (ADCC) is a critical Fc-mediated immune mechanism contributing to antiviral defence. However, its functionality in people living with HIV (PLHIV) following SARS-CoV-2 exposure remains poorly understood. OBJECTIVE: This study evaluates the anti-Spike ADCC antibody levels, functional ADCC activity, and neutralising antibody responses in HIV-negative and HIV-positive individuals. METHODOLOGY: The study included 30 samples per group. Anti-Spike ADCC-binding antibodies were measured using FcγRIIIa-binding assays, while functional ADCC activity

The selected few: Mechanisms of HIV-1 reservoir persistence.

Combination antiretroviral therapy durably suppresses HIV-1 replication yet leaves untouched a small, highly specialized population of infected cells that sustains lifelong viral persistence and remains the principal obstacle to cure. In this review, we synthesize emerging insights into viral and host mechanisms that maintain this extremely durable cellular reservoir, with emphasis on proviral transcriptional regulation, interactions between immune effectors and infected cells, cell-intrinsic programs governing survival and proliferation, and microenvironmental cues that confer competitive fit
