P-Selectin Glycoprotein Ligand (PSGL)-1 Expression on CD4+ T Cells in Patients with HIV: Insights from an Observational Study.

In the context of HIV infection, PSGL-1 expression and impact on CD4+ T cells remain largely unexplored. Thus, to address these critical gaps in knowledge, blood was sampled from HIV-negative controls and both ART-treated and ART-naïve individuals and stratified according to CD4+ T cell counts. PSGL-1 expression on CD4+ T cells and CD4+ T cell-platelet aggregates, along with PD-1, Bcl-2, and Caspase-3, were assessed with flow cytometry. Levels of IL-12, IFN-γ, LPS, and β-glucan were determined with ELISA. Spearman's correlation test was used to determine the correlation between PSGL-1 expressi

Mpox: Exploring Epidemiology, Disease Outcomes, and Preventative Vaccination Among People with HIV During the Ongoing Outbreaks.

Human mpox, caused by the mpox virus, is a reemerging viral zoonosis that has gained global attention due to recent Clade IIb outbreaks outside of Africa, as well as ongoing Clade Ia and Ib outbreaks in the Democratic Republic of Congo (DRC) and surrounding regions. Since the start of these outbreaks in 2022, approximately 160,000 people have been affected across more than 100 countries. People with human immunodeficiency virus (HIV; hereafter referred to as PWH) have been disproportionately affected, accounting for approximately 50% of all cases. Mpox is typically a self-limiting illness caus

Mpox in People Living with HIV: Clinical Challenges, Preventive Strategies and Public Health Implications.

Monkeypox virus (MPXV) re-emerged in 2022 with a global outbreak that affected more than 100,000 individuals worldwide. People living with HIV (PLWH) accounted for a substantial proportion of cases, raising concerns about disease presentation, management, and outcomes in this population. Evidence indicates that PLWH with advanced or uncontrolled HIV infection experienced more severe mpox, with higher hospitalization rates, more complications, and longer disease courses. In contrast, individuals with well-controlled HIV generally had outcomes similar to those without HIV. Access to timely diagn

Accessing HIV Care to Irregular Migrants in Israel, 2019-2024.

In Israel, irregular migrants (IMs) living with human immunodeficiency virus (HIV-1) that have no access to regular health insurance are provided with HIV-1-related health coverage under a public-private partnership (PPP) program initiated by the Ministry of Health in 2014. Here we characterized IMs referred to the PPP in 2019-2024 and used a linear mixed-effects model to follow up their CD4 and HIV-1 viral load (VL) counts for a median period of 16 months. Subtypes, resistance mutations and phylogenetic relationships were determined in all cases with viral failure and in selected cases with a

Defining and Predicting HIV Immunological Non-Response: A Multi-Definition Analysis from an Indonesian Cohort.

Immunological non-response (INR) to antiretroviral therapy (ART) is a critical concern for PLHIV, characterized by inadequate CD4+ T-cell recovery despite virological suppression. This retrospective study analyzed medical records of virologically suppressed adult PLHIV on ART (2004-2024) at two hospitals in Surabaya, Indonesia, using four operational categories to identify clinical and demographic determinants of INR. Patients were classified as immunological responders (IRs) or non-responders (INRs) based on four definitions: INR1 (CD4+ gain < 100 cells/mm3), INR2 (CD4+ < 350 cells/mm3), INR3

Diagnostic Accuracy of Next-Generation Sequencing: Prevalence of HIV-1 Drug Resistance and Associated Factors Among Adults on Integrase Inhibitors with Virologic Failure.

Emerging evidence indicates a high rate (>10%) of drug resistance (DR) associated with integrase strand transfer inhibitors (INSTIs) in developed countries, although there is limited information on DR during INSTI treatment in Uganda. With the increased use of INSTIs as standard first-line treatment, monitoring for DR using next-generation sequencing (NGS) has become essential. NGS can detect the lower-frequency variants that may be missed by traditional Sanger sequencing (SS). This study evaluates the diagnostic accuracy of next-generation sequencing (NGS) compared to Sanger sequencing for de

CAR-T Cell Therapy for HIV Cure: Current Challenges, Advances and Future Directions.

Antiretroviral therapy (ART) effectively suppresses HIV replication but fails to eradicate latent reservoirs, leading to viral rebound after interruption. Chimeric antigen receptor (CAR) T-cell therapy offers a potential strategy to achieve durable remission. A systematic PubMed search (July 2020-June 2025) identified 253 studies on CAR-T therapy in HIV; 74 met inclusion criteria and were qualitatively analyzed. Preclinical data showed that CAR-T cells can recognize and eliminate infected cells, reach viral reservoirs, and persist long term, particularly when derived from hematopoietic stem ce

Impact of an analytical treatment interruption on partners and family members of trial participants in Durban, South Africa: a qualitative study.

BACKGROUND: HIV cure-related research often include analytical treatment interruptions (ATIs), which are monitored pauses in antiretroviral therapy (ART) to determine whether interventions can stimulate viral control without ART. While ATIs have been conducted in high-income countries, scale-up in low-income HIV high-burden countries in sub-Saharan Africa raise unique ethical, social, and practical challenges. HIV cure-related trials focus on participants without considering the experiences of partners and family members. Here, we explore the perspectives of partners and family members, includ

Global burden and forecast of infectious diseases attributable to drug use: evidence from GBD 2021.

BACKGROUND: Drug use is a significant risk factor for infectious diseases, yet a comprehensive global assessment of this burden-encompassing trends, inequalities, and forecasts-is lacking, hindering progress toward World Health Organization (WHO)'s 2030 elimination targets. This study provides the first such analysis, leveraging the Global Burden of Disease (GBD) 2021 study to quantify the specific burden of infectious diseases attributable to drug use. METHODS: Using data from 204 countries and territories in the GBD 2021 study, we analyzed deaths and disability-adjusted life years (DALYs) f

Transcription of HIV-1 is heterogenous among authentic latent CD4+ T cell clones.

Antiretroviral therapy suppresses HIV-1 infection but fails to eliminate a reservoir of intact latent proviruses that reside primarily in CD4+ T cells. The lack of precise understanding of the latent compartment has made it challenging to develop curative strategies for HIV-1 infection. Here we report on the properties of CD4+ T cell clones carrying intact latent proviruses, expanded in vitro from single cells obtained from the reservoir of people living with HIV-1. The latent proviruses in the clones were integrated into ZNF genes, nongenic satellite, and centromeric regions, frequently assoc

A case of HIV encephalopathy with HIV cerebrospinal fluid escape, followed by second-generation integrase inhibitor resistance.

The management of advanced HIV can be complicated by immune reconstitution inflammatory syndrome (IRIS) and cerebrospinal fluid (CSF) viral escape. We report a novel case of symptomatic CSF escape driven by a rare bictegravir resistance mutation, emerging after a severe unmasking IRIS event. A 61-year-old male patient with advanced HIV (clusters of differentiations 4+ T-cell count 31 cells/μl) initiated antiretroviral therapy with bictegravir/emtricitabine/tenofovir alafenamide. A total of 1 week later, he developed varicella-zoster virus-associated acute retinal necrosis, consistent with IRIS
