INTRODUCTION: Podocytopathies such as minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS) remain therapeutic challenges in adults. Although corticosteroids and rituximab (RTX), a chimeric anti-CD20 monoclonal antibody, are effective in most patients, up to 10% show resistance or relapse despite B-cell depletion. Obinutuzumab (OBZ), a humanized type II anti-CD20 monoclonal antibody, achieves deeper and more sustained B-cell depletion and may overcome RTX inadequate response.

C3 glomerulopathy is an ultra-rare kidney disease driven by dysregulation of the alternative complement pathway fluid phase C3 convertase. We report the case of a previously healthy 13-year-old girl who presented with concurrent nephrotic and nephritic syndrome and low complement C3. Her initial kidney biopsy surprisingly showed mesangioproliferative glomerulonephritis with dominating IgA deposits resulting in a diagnosis of IgA nephropathy. Despite standard immunosuppressive treatment with prednisone and mycophenolic acid plus angiotensin-converting enzyme inhibitors, she achieved only partia

BACKGROUND: Diabetic kidney disease (DKD) remains a leading cause of end-stage renal disease (ESRD), with persistent proteinuria contributing to renal and cardiovascular complications. Despite dual therapy with renin-angiotensin-aldosterone system (RAAS) blockers and sodium-glucose co-transporter-2 inhibitors (SGLT2is), residual proteinuria persists in a substantial subset of patients. Finerenone, a novel nonsteroidal mineralocorticoid receptor antagonist, has shown promise in reducing albuminuria and slowing chronic kidney disease progression. This study evaluated the additive effect of finer

PURPOSE: Haematuria and proteinuria may point to kidney diseases, but may also be found incidentally. Among schoolchildren, many studies suggest their prevalence to around 1%. In neonates, the frequency and persistence of haematuria and/or proteinuria in the general population have so far not been investigated systematically.
METHODS: In the course of the prospective KUNO-Kids Health Study, urine samples were collected and analysed by dipstick in asymptomatic neonates on days three to five after birth. Those with positive findings underwent a maximum of two follow-ups until 16 weeks of age an

Infection-related glomerulonephritis (IRGN) in adults often arises in individuals with diabetes mellitus and can present with severe acute kidney injury (AKI). Because AKI impairs renal drug clearance, sulfonylureas (SU) may accumulate and precipitate profound hypoglycaemia. A 67 year-old man with type 2 diabetes mellitus was found unconscious and presented with severe hypoglycaemia (28 mg/dL) and marked AKI (serum creatinine 10.89 mg/dL) accompanied by oliguria and volume overload. Laboratory findings showed heavy proteinuria, haematuria, low complement C3, and elevated anti-streptolysin O ti

We read with great interest the article by Kitisin et al. published in the Journal of Critical Care. In our letter, we focus on raising a key methodological concern: when "persistent AKI" is defined as "lasting more than 48 hours from AKI onset," and serum creatinine and urine output are only specified as "at least twice daily" assessments, the heterogeneity in monitoring intensity among different patients may influence outcome determination. Variable sampling frequencies (potentially related to disease severity or influenced by clinicians' awareness of uACR results) alter the capture of AKI o

The prognostic value of donor-derived cell-free DNA (dd-cfDNA) for long-term kidney allograft outcomes after indication biopsy remains incompletely defined. In this prospective single-center cohort, 106 kidney transplant recipients with 108 indication biopsies were assessed for dd-cfDNA at biopsy and at 7, 30, and 90 days thereafter. dd-cfDNA was analyzed as a continuous, threshold-based, and longitudinal time-dependent variable. Clinical endpoints included ≥30% eGFR decline within 2 years, indication for re-biopsy, and graft failure. Persistent elevation of dd-cfDNA (≥0.5% at 90 days) occurre

Chronic kidney disease (CKD), defined per the Kidney Disease: Improving Global Outcomes (KDIGO) guidelines by persistent (≥3 months) abnormalities of kidney structure or function, including reduced estimated glomerular filtration rate (eGFR) and/or albuminuria, affects 10%-15% of adults worldwide and is a powerful, independent risk factor for cardiovascular diseases, including heart failure (HF). In CKD (stages G1-G3), particularly when accompanied by albuminuria (KDIGO stages A2-A3), even mild reductions in eGFR are associated with subclinical cardiac abnormalities-left ventricular hypertroph

BACKGROUND: Immunoglobulin A nephropathy (IgAN) is not only the most common primary glomerular disease but also a chronic inflammatory condition associated with increased cardiometabolic risk through the cardiorenal axis. Persistent proteinuria and progressive renal dysfunction are linked to adverse cardiovascular and metabolic outcomes and may complicate the delivery of long-term lifestyle and nutritional risk-modifying strategies. Telitacicept, a dual BAFF/APRIL inhibitor, has emerged as a targeted immunomodulatory therapy for IgAN, yet the clinical evidence remains heterogeneous.

Kidney complications, referred to as nephropathy, develop early in sickle cell disease (SCD). In addition to its known morbidity, abundant data show that chronic kidney disease (CKD) is associated with an increased mortality risk in SCD. Increasing evidence suggests that the natural history of SCD nephropathy is progressive. Initial glomerular hyperfiltration without albuminuria appears to precede increasingly severe albuminuria, kidney function decline, and eventual kidney failure. Based on the current understanding of the natural history of SCD nephropathy, intervention before the developmen

BACKGROUND: Immunoglobulin A nephropathy (IgAN) is the most common primary glomerulonephritis worldwide. A significant proportion of patients, approximately 20–40%, progress to end-stage renal disease (ESRD) within 10 to 20 years. Telitacicept, a dual inhibitor of BAFF and APRIL, is now recommended in clinical guidelines for the treatment of high-risk IgAN. However, the optimal timing for initiating this therapy remains undefined. METHODS: This retrospective study included 19 patients with high-risk IgAN (persistent proteinuria of at least 1 g/d despite at least 3 months of optimised therapy,

Cardiac myxomas are rare in children and can mimic other conditions. We report a 14-year-old boy initially diagnosed with acute poststreptococcal glomerulonephritis based on hematuria, mild proteinuria, and elevated anti-streptolysin-O titers. However, worsening dyspnea, edema, and signs of heart failure prompted further evaluation. Echocardiography revealed a large left atrial mass with impaired biventricular function and tricuspid regurgitation. Surgical excision confirmed a myxoma. The patient's renal and systemic symptoms were attributed to low cardiac output and venous congestion, not tru

Steroid-resistant nephrotic syndrome (SRNS) in children carries a poor prognosis, and recent discoveries implicate anti-nephrin autoantibodies (ANABs) as mediators of podocyte injury. Lipoprotein apheresis, originally developed for familial hypercholesterolemia, has been explored as rescue therapy in refractory SRNS, but its immunologic effects remain uncertain. This case demonstrates a decline in ANAB titers accompanied by clinical improvement following lipoprotein apheresis. We report a 7-year-old male of Afro-Caribbean descent with biopsy findings consistent with minimal change disease and

Wilson's disease (WD) is a hereditary disorder that impairs copper metabolism. Both WD and its treatment with penicillamine are associated with renal impairment. We report a case of penicillamine-induced podocytopathy with pathological findings of early-stage focal segmental glomerulosclerosis (FSGS), which demonstrated rapid reversibility upon drug withdrawal alone. A 36-year-old female of East Asian origin with a 12-year history of WD presented with nephrotic syndrome after 21 months of penicillamine therapy. Her 24-h proteinuria was 6.58 g. Her renal biopsy revealed a podocytopathy with ear

BACKGROUND Levamisole, a widely used adulterant in illicit cocaine, has emerged as an important cause of cocaine-levamisole-associated vasculopathy (CLAV), a syndrome that can mimic primary ANCA-associated vasculitis. Typical features include neutropenia, atypical ANCA patterns, thrombotic manifestations, diffuse alveolar hemorrhage (DAH), and crescentic glomerulonephritis (GN). Dual MPO-PR3 ANCA positivity is strongly suggestive of drug-induced vasculopathy. CASE REPORT We report the case of a 30-year-old woman presenting with hemoptysis, anemia, and diffuse pulmonary infiltrates. Laboratory

BACKGROUND: Residual kidney risk persists in hypertensive patients despite guideline-directed blood pressure (BP) management. Whether systemic inflammation is preferentially associated with the development of distinct early chronic kidney disease (CKD) phenotypes remains uncertain.
METHODS: This retrospective cohort study included 5,904 hypertensive patients with preserved baseline kidney function. The primary exposure was the C-reactive protein-to-lymphocyte ratio (CLR). Outcomes assessed were incident proteinuric CKD, isolated estimated glomerular filtration rate (eGFR) decline, and any inc

Diabetic kidney disease (DKD) is one of the most common microvascular complications of diabetes. It can be identified by thickening of the glomerular basement membrane, reduced glomerular filtration rate, and persistent proteinuria. Macrophages play a key role in the pathogenesis of DKD, and their phenotype (M1 and M2) is finely regulated by metabolic reprogramming. M1 macrophages exacerbate inflammatory damage and fibrosis in renal tissue by secreting pro-inflammatory mediators and reactive oxygen species (ROS). M2 macrophages (further subdivided into M2a, M2b, M2c and M2d subtypes) primarily

BACKGROUND: In diabetic kidney disease (DKD), DNA hypermethylation-mediated silencing of protective genes constitutes a key mechanism of metabolic memory, driving persistent renal injury. Klotho suppression critically exacerbates renal inflammation and injury. Our preliminary studies documented Klotho promoter hypermethylation in DKD kidneys. However, direct evidence linking its sustained downregulation to this epigenetic aberration and demethylation’s therapeutic benefits remains unestablished. This study investigates whether demethylation treatment-mediated reversal of Klotho promoter hyperm

OBJECTIVES: Chronic hypotension is uncommon among kidney transplant recipients, and may adversely affect graft outcomes.
PATIENTS AND METHODS: Single-center retrospective cohort study including adult recipients who underwent living-donor kidney transplantation, between 2009 and 2024. Of the 1678 recipients screened, 187 met the inclusion criteria. Hypotension was defined as persistent systolic blood pressure (SBP) <100 mm Hg or mean arterial pressure (MAP) <70 mm Hg throughout follow-up; normotensive had SBP ranging from 100 to 130 mm Hg. Early outcomes included primary non-function (PNF), de
