[A study of urinary immunoglobulin excretion after forced lordosis].

Changes in the urinary excretion of immunoglobulins (IgG and IgA) before and after forced lordosis were studied in 26 children with orthostatic proteinuria (group OA, where proteinuria disappeared within 120 min after lordosis was referred to as group OAA and where if disappeared after more than 120 min was group OAB) and 9 children with chronic glomerulonephritis in the clinically stable state (group GN). Urinary immunoglobulins were measured by ELISA (enzyme linked immunosorbent assay). In resting urine, the urinary excretion rates (total protein, IgG, and IgA) and the urinary protein ratio

Anionic versus cationic immunoglobulin clearance in normal subjects: a novel approach to the evaluation of charge permselectivity.

The excretion of proteins differing in charge (the different immunoglobulin subclasses) and/or size (albumin, immunoglobulins) were investigated in normal subjects in a number of physiological conditions aiming at the evaluation of renal charge permselectivity. In 101 randomly selected normal subjects the urinary excretion rates of albumin, IgG4 (anionic proteins) and of total IgG (mostly cationic) were evaluated in basal conditions; the protein/creatinine urinary ratio and protein clearances were assessed in part of them. In addition, the intra- and interday variations of protein excretion we

Entrapment of left renal vein in children with orthostatic proteinuria.

We found that patients with orthostatic protein-uria had entrapment of the left renal vein (LRV) by the aorta and superior mesenteric artery (SMA). Of 15 patients studied, ultrasonographic examination showed 13 cases of typical LRV entrapment with prestenotic dilatation, and 2 cases of mild LRV compression between the aorta and SMA. Intra-arterial digital subtraction angiography and monitoring of pull-back pressure from LRV to the inferior vena cava (IVC) were performed on 2 patients with 4+ proteinuria. Accumulation of contrast medium was seen with mild back-flow to the collateral veins, and

[Long-term follow up of children with orthostatic proteinuria].

The authors summarize their observations of 18 children with orthostatic proteinuria. The average time of follow-up was 4 and half years. They examined body measures and renal functions of the patients and compared their blood pressure, pulse rate, electrocardiographic curve, morphological and positional conditions of kidneys in lying and standing position, as well as change of quantity and quality of proteinuria (high, middle and low molecular weight) at day time and night both in proteinuric children with and without ptosis renis and in controls. The patients were taller than the average, bu

Contribution to the aetiopathogenesis of orthostatic proteinuria in children: relation to the upper calyx syndrome.

The authors discuss the correlations between the upper calyx syndrome and proteinuria. The syndrome was demonstrated in 33.6 per cent of all children with orthostatic proteinuria hospitalized at the Paediatric Clinic, Martin (Czechoslovakia) during 1978-1985. They consider the possible relations between these conditions that could be mediated by the activation of the renin/angiotensin II system. The direct stimulus of this system could rest in local ischaemization of parenchyma in the region of the proximal pole of the kidney or its partial passive congestion. They assume that these changes be

Proteinuria--what value is the dipstick?

The value of the urinary dipstick in the assessment of proteinuria was investigated in a study correlating laboratory measurements of protein and albumin against the dipstick protein in the same samples of urine; 94 patients (100 admissions) were studied at the Royal Air Force Renal Unit, each patient collecting two 24-h urine samples. Along with each 24-h sample, 10-ml aliquots of urine were obtained at 3 designated times during the day for both ward dipstick testing and laboratory assay; + or more on the dipstick correlated with abnormal proteinuria (greater than or equal to 150 mg/24 h) in

Treatment of hypertension in diabetic patients.

Hypertension, common in diabetic patients, worsens not only the risk of cardiovascular complications, but also that of microangiopathic complications (nephropathy, retinopathy) of diabetes mellitus. It is thus important to ensure the perfect control of even mild hypertension in diabetic patients. However, treatment sometimes becomes difficult given that certain categories of antihypertensive drugs interfere with blood glucose control and/or lipid metabolism, interfere with the symptomatology of hypoglycemia, or promote orthostatic hypotension, a complication of autonomic neuropathy. A study wa

Diabetic neuropathy as a heterogeneous syndrome: multivariate analysis of clinical and neurological findings.

We quantitatively assessed peripheral and autonomic nerve function in diabetic patients and compared them with various parameters of their diabetic status. Motor and sensory nerve conduction velocity (MCV, SCV), vibratory perception threshold (VPT) and the coefficient of variation of the ECG R-R interval (CV R-R) were measured in 85 diabetic patients aged 20-59 years. These values were compared with those of age-matched healthy subjects. Moreover, in 53 patients, MCV, SCV, VPT and CV R-R were investigated by multivariate analysis in relation to clinical parameters. In diabetics, MCV, SCV and C

Medical complications of eating disorders in adolescents.

Anorexia nervosa and bulimia are occurring with increased frequency among adolescents and preadolescents. To determine the range and severity of medical complications encountered in younger anorectic and bulimic patients, we reviewed the medical records of 65 adolescents and preadolescents, aged 10 to 20 years, who were observed in the Eating Disorders Clinic of the Children's Hospital at Stanford. Significant medical instability was present in the majority of our patients. A total of 55% of anorectic patients and 22% of bulimic patients required hospitalization for medical reasons during the

Microalbuminuria in single kidney patients: relationship with protein intake.

Experimental models have shown that the reduction in renal mass induces an increase in glomerular filtration rate of the remnant nephrons, leading to proteinuria and glomerular sclerosis. Since the presence of microalbuminuria - increased urinary albumin excretion (UAE) undetectable by routine assays - can be an early sign of this phenomenon, UAE in the normo- and microalbuminuric range was measured in 24 single kidney patients with negative Albustix. Nephrectomy had been performed in 22 cases 1 to 28 years before, mostly because of renal lithiasis. Patients were selected as being normotensive

Asymptomatic proteinuria. Benign disorder or harbinger of disease?

Proteinuria may be an indication of underlying disease or may be found in various physiologic states. Careful quantitation of urinary protein and thorough patient evaluation are necessary to determine if proteinuria is cause for concern. Transient and orthostatic proteinuria appear to be benign, but persistent proteinuria may be a manifestation of serious disease.

Hepatic amyloidosis (primary [AL], immunoglobulin light chain): the natural history in 80 patients.

PURPOSE: We wished to study patients with a premortem diagnosis of primary hepatic amyloidosis to determine what clinical and laboratory features might assist in recognizing the disease and assessing prognosis. PATIENTS AND METHODS: A group of 80 patients with liver biopsy proven primary hepatic amyloidosis was followed from diagnosis to death. RESULTS: At presentation, 77 percent of the group had an associated nephrotic syndrome, congestive heart failure, peripheral neuropathy, or orthostatic hypotension. Certain clues suggested the diagnosis of hepatic amyloidosis in patients with liver di

The safety and tolerability of lisinopril in clinical trials.

The safety and tolerability of lisinopril were assessed in 1,476 patients [1,165 hypertensives and 311 patients with congestive heart failure (CHF)] and 211 normal volunteers. The duration of lisinopril therapy ranged from 1 day to 16 months, with a mean duration of 105 days. In the hypertensive population, the most frequent clinical adverse experiences on lisinopril alone were headache, dizziness, cough, and diarrhea. Not all of these adverse experiences were thought to be drug related. Five percent of patients were discontinued because of adverse clinical experiences; cough and dizziness wer

Benign persistent asymptomatic proteinuria with incomplete foot process disease in adolescents: a new clinical entity?

A distinct glomerular damage characterized by the following clinical and pathological findings was verified in 20 children with a proteinuria. The clinical features were: 1) glomerular damage was frequent in adolescent males, the male to female ratio being 3:1; 2) the onset was initiated by asymptomatic proteinuria which remitted spontaneously after 3-5 years, with no resulting nephrotic syndrome; 3) there was no apparent cause for the proteinuria such as postural proteinuria, wandering kidney, hypertension or heart disease; 4) in no patient was there a past history of infection or occurrence
