Reduced Expression of Magnesium Transport Proteins in the Distal Convoluted Tubule of Clcnkb-Deficient Mice May Explain Urinary Magnesium Wasting in Classical Bartter Syndrome.

AIM: The kidney thick ascending limb (TAL) plays a key role in the transport of sodium, chloride, potassium, calcium, and magnesium. Bartter syndrome is a hypokalemic, salt-losing tubulopathy caused by impaired TAL function. Pathogenic variants in SLC12A1 or KCNJ1 cause antenatal Bartter syndrome, and variants in CLCNKB result in classical Bartter syndrome. Although all variants impair TAL electrolyte transport, their effects on mineral handling differ. In contrast to antenatal forms, classical Bartter syndrome is frequently associated with hypomagnesemia, a feature also found in Gitelman synd

Evaluating the inter-relationships among depression, anxiety and somatic symptoms in haemodialysis patients: a cross-sectional structural equation modelling study.

BACKGROUND: Somatic symptoms frequently occur in haemodialysis patients, whereas depression and anxiety are also common but are often under-recognised issues. This study aims to examine the associations among depression, anxiety and somatic symptoms in haemodialysis patients. These symptoms may be linked together under a general distress construct in haemodialysis patients. METHODS: We surveyed 1085 dialysis patients between 1 March 2018 and 28 February 2023 at 17 dialysis facilities in Northeast Ohio with the 9-item Patient Health Questionnaire (PHQ-9), 7-item Generalized Anxiety Disorder an

Establishing a Standardized Renal Artery Ligation Procedure for Hypertension in Syrian Golden Hamsters.

Hypertension is considered a major global health threat, which can lead to severe complications, including vascular remodeling, heart failure, myocardial infarction, and end-stage renal disease. Syrian golden hamsters, a model with naturally expressing cholesteryl ester transfer protein (CETP), are susceptible to diet-induced hyperlipidemia. To establish a reliable and reproducible hamster model for studying the relationship between renal artery stenosis (RAS), secondary hypertension, and cardiovascular disease, the two-kidney, one-clip (2K1C) Goldblatt model was employed. This study details

Outcomes of renal angioplasty for fibromuscular dysplasia in Asian vs. white patients: a systematic review and meta-analysis.

Percutaneous transluminal renal angioplasty (PTRA) is the first-line treatment for fibromuscular dysplasia (FMD)-related renal artery stenosis. Previous meta-analyses were predominantly based on white populations. We updated the meta-analysis to include Asian data and allow ethnicity-specific comparisons. Seven databases were systematically searched for clinical studies published since 2000 reporting PTRA outcomes in FMD. Data were pooled using random-effects models. Twenty-nine studies (1470 patients) were included: 15 Asian ( n  = 823) and 14 white ( n  = 647). Hypertension cure was signific

Renal denervation in patients with uncontrolled hypertension and renal artery stenosis assessed by invasive physiology.

BACKGROUND: Renal artery stenosis is usually considered as a contraindication for renal denervation. Whether renal denervation can be safe and effective in the presence of a renal artery stenosis once secondary renovascular hypertension has been ruled-out has never been investigated to date. We report our experience with radiofrequency-based renal denervation in patients with moderate-to-severe renal artery stenosis. METHODS: Patients with uncontrolled hypertension eligible for renal denervation referred to our Center were evaluated for this study. Renal artery stenosis >50% as evaluated by q

[Analysis of related factors for the occurrence of cardiovascular-renal-metabolic syndrome stage 4 in patients with primary aldosteronism].

Objective: To explore the related factors of patients with primary aldosteronism (PA) developing into stage 4 of cardiovascular-renal-metabolic syndrome (CKM). Methods: A total of 397 patients diagnosed with PA from January 2016 to December 2024 were retrospectively enrolled. According to the CKM staging criteria, the patients were divided into the early stage group (stages 0-2), stage 3 group and stage 4 group. To analyze the differences in each indicator among the three groups. Multivariate logistic regression models were applied to analyze the correlation between each indicator and the occu

Coexistence of Neuromyelitis Optica Spectrum Disorder and Proliferative Lupus Nephritis: A Rare Case Report.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune demyelinating disease manifesting with transverse myelitis and optic neuritis. Systemic lupus erythematosus (SLE) also commonly affects kidneys and central nervous system. Here, we describe a case of proven NMOSD for almost 16 years with multiple relapses over a period of time, presented to us with acute nephritic syndrome. Her kidney biopsy revealed class 4 lupus nephritis. She responded to steroid, rituximab, and mycophenolate mofetil and achieved complete remission at 3 months. Subsequent relapse at 6 months was initially treat

Blood pressure improvement after stenting in an atherosclerotic renal artery stenosis patient with normal rFFR: A case report.

BACKGROUND: There is no consensus on the optimal hemodynamic assessment of renal artery stenosis patients to identify who is suitable for revascularization. Previous studies demonstrated the predictive value of renal fractional flow reserve (rFFR) and hyperemic systolic gradients in blood pressure (BP) improvement after stenting. This report describes BP improvement after stenting in 1 case of atherosclerotic renal artery stenosis with normal rFFR. CASE PRESENTATION: A male with the complaint of uncontrollable hypertension was diagnosed with secondary hypertension due to atherosclerotic right

Fractional Flow Reserve-Guided Renal Artery Stenting in Renovascular Hypertension: 1-Year Results of the FAIR Study.

BACKGROUND: Whether fractional flow reserve (FFR)-guided revascularization improves outcomes in atherosclerotic renal artery stenosis (ARAS) remains uncertain. OBJECTIVES: This study will evaluate whether the benefit of FFR-guided stenting sustained at 1 year including renal outcomes. METHODS: FAIR pilot is an investigator-initiated, multicentre, open-label trial with blinded endpoint adjudication. Adults with ARAS were randomized 1:1 to FFR-guided vs angiography-guided groups. Stenting was performed in the FFR-guided group when FFR <0.80, and it was performed for all in the angiography-guid

Paediatric midaortic syndrome associated with ascending aortic and renal aneurysm managed with complex multidisciplinary medical management and surgical repair.

We present the case of an otherwise previously healthy female in middle childhood with severe renovascular hypertension secondary to midaortic syndrome with ascending aortic dilation. She presented with hypertensive emergency (>200/100 mm Hg) and a marked blood pressure gradient between upper and lower extremities. She underwent successful surgical revascularisation by way of aorto-aortic bypass with a polytetrafluoroethylene graft and revascularisation of the superior mesenteric artery and both renal arteries by way of reimplantation for refractory hypertension despite four antihypertensives

Associations Between Sickle Cell Disease, Pica, and Enuresis in Pediatric Neurodevelopmental Disorders.

Background: Sickle cell disease (SCD) is a hereditary disorder affecting red blood cells' shape and functional capacity. Individuals with SCD report relatively high co-occurrence of neurodevelopmental disorders (NDDs). In addition, these children also have higher rates of enuresis (incontinence) and pica, disorders prevalent in children with developmental delays. Both enuresis and pica can have negative effects on mental health, but their pathophysiology, especially in SCD, remains unclear. Objectives: The objective of this study was to determine the rates of pica and enuresis in a pediatric S

Acute kidney injury: incidence, risk factors, and early outcomes in hospitalized children with sickle cell disease attending a tertiary hospital in Eastern Uganda.

BACKGROUND: Many sickle cell disease (SCD) patients experience renal damage that impairs function, with acute kidney injury (AKI) significantly affecting both immediate and long-term outcomes. The only Ugandan study on this subject reported a high incidence and mortality of AKI among children with SCD, but did not assess other outcomes. METHODS: This hospital-based prospective study was conducted between January and June 2024 at Jinja Regional Referral. AKI was diagnosed using KDIGO criteria, and participants were followed until discharge or death. Outcomes included length of hospital stay, mo

Pneumococcal HUS in infancy masking DGKE-related thrombotic microangiopathy with secondary hemophagocytic lymphohistiocytosis.

Hemolytic uremic syndrome (HUS) in infancy may be difficult to classify because infection-associated HUS and genetic thrombotic microangiopathies (TMAs) can present with similar findings. We report a 7-month-old boy admitted with pneumonia, anemia, thrombocytopenia, and acute kidney injury. Radiologically confirmed pneumonia, elevated CRP levels, positive nasopharyngeal pneumococcal PCR testing initially suggested pneumococcal-associated HUS; however, microangiopathic hemolysis and anuria persisted despite infection control, dialysis, plasma exchange, and complement inhibition therapy. Genetic

Early Transition From Parenteral to Oral Antibiotics for Neonates With Bacteremic Urinary Tract Infections.

We conducted a retrospective cohort study of 49 neonates with bacteremic urinary tract infections (UTIs) to assess the efficacy of transition to oral antibiotics. Forty-one received oral step-down therapy after parenteral antibiotics; 2 had relapsed UTIs. No cases of bacteremia or meningitis occurred within 30 days post-treatment, suggesting transition to oral antibiotics as effective.

Accelerated bone loss increases osteoporosis and fracture risk in moderate to severe chronic kidney disease.

Chronic kidney disease-mineral and bone disorder (CKD-MBD) contributes to bone loss and fractures. Evidence on longitudinal changes in bone mineral density (BMD) in CKD remains limited. This retrospective cohort study evaluated the longitudinal decline in BMD across levels of kidney function and examined the associations between kidney function and the risk of osteoporosis and fractures. Patients with estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 who underwent dual-energy X-ray absorptiometry between 2010 and 2023 were included. Diagnostic agreement of bone histomorphometry for renal bone disease.

INTRODUCTION: An iliac bone biopsy with semi-quantitative histomorphometry is the gold standard for evaluating renal osteodystrophy (ROD). However, lack of standardization of the bone biopsy procedure, sample handling, reading and reporting represent barriers to research synthesis and clinical collaboration. This study assessed diagnostic agreement across European laboratories performing histomorphometry for evaluation of ROD. METHODS: Five-millimeter transiliac biopsy slides from 5 cases were converted to digital images for analysis by experts from 7 laboratories. Parameters included quantit
