Clinical Outcomes of Patients With Metastatic Prostate Cancer With Microsatellite Instability Treated With Pembrolizumab.

MSI-H prostate cancer is rarely encountered in clinical practice. When it is identified, pembrolizumab is a standard of care therapeutic option in the metastatic castration-resistant setting based on KEYNOTE-158, which included very few prostate cancer patients. A few previously published case series have shown encouraging clinical outcomes for patients with MSI-H mCRPC treated with pembrolizumab, however, there is still a paucity of real-world data. Here we report an institutional case series of 13 patients with MSI-H metastatic prostate cancer identified by next generation sequencing of tumo

In-silico patient-specific modelling of prostate cancer: Predicting PSA dynamics and treatment response.

BACKGROUND AND OBJECTIVE: Prostate cancer remains a significant global health concern, with treatment response varying among patients. Radiotherapy, often combined with hormone therapy, is a key treatment approach, but predicting individual outcomes remains challenging. Computational models have emerged as valuable tools to simulate tumour behaviour and optimise treatment strategies. This study presents a patient-specific computational model designed to predict tumour response by associating Prostate-Specific Antigen (PSA) dynamics with tumour biological behaviour under therapy. METHODS: The

Postdiagnosis physical activity is associated with improved survival in prostate cancer patients treated with surgery but not with radiation therapy.

BACKGROUND: The effects of exercise on cancer outcomes may differ depending on its positioning within different cancer treatment combinations. We examined whether the associations between physical activity (PA) and cancer outcomes varied by cancer treatment modality or timing of PA. METHODS: We conducted a secondary analysis of the Prostate Cancer Cohort Study consisting of 830 men in Alberta, Canada with newly diagnosed prostate cancer. Lifetime prediagnosis PA was assessed by an in-person interview shortly after diagnosis whereas postdiagnosis PA was assessed at 2-3 year intervals by an in-

Use of a BMI-independent biomarker-based prostate cancer risk score to identify and triage individuals at risk of prostate disease.

Prostate cancer (PCa) is the second most common cause of cancer related deaths in men in the UK. A national screening programme for PCa does not exist due to the unsuitability of the total prostate specific antigen (tPSA) test which is not specific for PCa and has a high false positive rate. Serum tPSA was measured in n = 25,356 male Randox Health clients. A biomarker-based (tPSA, EGF, MCP-1, IL-8) prostate cancer risk score (PCRS) was then applied to a retrospective cohort (n = 1,142/25,356) of individuals to assess PCa risk. A comparative analysis between tPSA and PCRS indicated that 90.5% o

Predicting side-specific extraprostatic extension in prostate cancer using an 18F-DCFPyL PSMA-PET/CT-based nomogram.

BACKGROUND: Extraprostatic extension (EPE) in prostate cancer (PCa) has implications for nerve-sparing approaches. mpMRI-based nomograms show modest accuracy, highlighting the need for improved predictive models. This study evaluates 18F-DCFPyL prostate-specific membrane antigen (PSMA) positron emission tomography/ computed tomography (PET/CT) for predicting side-specific EPE using maximum standardized uptake value (SUVmax). METHODS: This single-center cohort study included patients undergoing RALP by a single surgeon (AKT) from January 2022 to September 2024. Baseline variables included demo

Fee cuts for radiology and low-value imaging.

Based on the belief that fee-for-service reimbursement contributes to the overuse of costly medical procedures, policymakers have sought to cut payments for low-value services. Using a difference-in-differences model and Medicare claims linked with cancer registry records, we evaluate the impact of a large payment reduction for an imaging procedure commonly used in prostate cancer patients. We find that the payment cut did not affect imaging use, even in low-risk patients for whom imaging is not recommended. Our results suggest that supply may be relatively insensitive to fee levels when decis

Microphysiological Solid Tumor Models in Hydrogel Beads for CAR T Cell Immunotherapy Evaluation.

Micrometastases are challenging to resect surgically and to detect with in vivo imaging. Immunotherapy is highly anticipated to revolutionize their treatment, but its overall efficacy still remains limited for solid tumors. Here, a 3D micrometastases model is developed to mimic key microenvironmental cues, enabling in vitro evaluation of chimeric antigen receptor (CAR) T cell immunotherapy. Prostate cancer that preferentially metastasizes to, e.g., liver or bone marrow, is utilized as a model. Hydrogel beads with an elastic modulus matching those of soft organs are used to support long-term cu

Elevated NPM1 and FBL expression correlates with prostate cancer aggressiveness and progression.

The nucleolus is a membrane-less body present in the nucleus of the cell. The nucleolus is mainly involved in ribosomal RNA (rRNA) transcription and ribosome biogenesis for protein translation. During cancer formation, nucleolar morphology is altered, and many nucleolar proteins are expressed at a higher level, leading to enhanced ribosome biogenesis and protein translation, which supports cancer aggressiveness, proliferation, migration, and invasion. In this study, we investigated the association of two nucleolar proteins, nucleophosmin (NPM1), and fibrillarin (FBL), with prostate cancer (PCa

Single-cell transcriptomics redefines focal neuroendocrine differentiation as a distinct prostate cancer pathology.

Neuroendocrine prostate cancer (NEPC) tumours are classified by pathology into several distinct subtypes. Gene expression profiling has revealed transcriptional heterogeneity across NEPC, but this is rarely considered in the context of variation between pathologies. Diagnosis typically relies on immunohistochemical markers (CHGA, SYP, NCAM1) and genomic alterations in RB1, PTEN and TP53. We hypothesized that NEPC pathologies have unique transcriptional features. Single-cell RNA sequencing of 18 632 tumour cells from nine patient-derived xenograft models representing five pathologies (small-cel

Towards practical federated learning and evaluation for medical prediction models.

BACKGROUND: Federated learning (FL) is a rapidly advancing technique that enables collaborative model training while preserving data privacy. This approach is particularly relevant in healthcare, where privacy concerns and regulatory restrictions often prevent centralized data sharing. FL has shown promise in tasks such as disease detection, achieving performance levels comparable to centralized systems. However, its practical usability in real-world applications remains underexplored. METHODS: We evaluate the practical effectiveness of FL in predicting whether patients suspected of prostate

Endothelial-Mesenchymal Transition in Tumor Microenvironment Promotes Neuroendocrine Differentiation of Prostate Cancer.

Neuroendocrine prostate cancer (NEPC) is a highly aggressive and treatment-resistant subtype of castration-resistant prostate cancer (CRPC) that often emerges during progression under androgen-receptor (AR) pathway inhibition. While lineage plasticity in cancer cells has been recognized as a key mechanism of resistance, the role of the tumor microenvironment in driving this transition remains unclear. Among its cellular components, vascular endothelial cells can undergo endothelial-mesenchymal transition (EndoMT), a phenotypic shift associated with tumor progression and fibrosis. Here, we inve

REPAIR_GETUG P16 relapse in previously irradiated prostate bed: a phase I/II study of stereotactic ablative reirradiation potentiated by a metformine study protocol.

INTRODUCTION: Salvage prostatic bed radiotherapy (PBRT) is a standard in case of biochemical recurrence following radical prostatectomy (RP) for prostate cancer (PC). The management of isolated prostatic bed recurrence following RP and PBRT is debated. Reirradiation within stereotactic body radiotherapy (SBRT) guided by metabolic imaging could be a relevant option in this case. In parallel, metformin, an economically viable and well-tolerated oral antidiabetic agent, has demonstrated its radiosensitising properties. This phase I/II clinical trial aims to (i) determine the optimal dose for SBRT

Comparative analysis of the tumor microenvironment in primary CNS and testicular large B-cell lymphomas using digital image analysis and its implications for immunotherapy.

Primary large B-cell lymphomas of immune-privileged sites, including primary central nervous system lymphoma (PCNSL) and primary testicular lymphoma (PTL), exhibit distinct clinicopathologic features contributing to aggressive behavior and immune evasion. While the molecular characteristics of PCNSL and PTL have been extensively studied, the tumor microenvironment (TME) remains insufficiently understood. In particular, no study has directly compared the TME of PCNSL and PTL, highlighting a critical gap in understanding their immunobiology. We analyzed 55 cases of diffuse large B-cell lymphoma

Experimental models for developing oncolytic virotherapy for metastatic prostate cancer.

Cancer has remained the second leading cause of death worldwide for over a century. Despite significant advances, effectively targeting cancer cells and overcoming therapeutic challenges remain critical goals. In this review, we focus on advanced metastatic prostate tumors, where the patients' five-year survival rate is less than 35%. While standard androgen deprivation therapy (ADT) has been effective for most prostate cancer patients, recurrence of aggressive tumors is common, emphasizing an urgent need for new treatment strategies. Immunotherapy has gained attention for its potential to har
