Low-intensity extracorporeal shockwave therapy for chronic prostatitis/chronic pelvic pain syndrome: a retrospective single-center observational study in Japan.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a heterogeneous condition that can remain symptomatic despite conventional therapy. Low-intensity extracorporeal shock wave therapy (Li-ESWT) has shown symptomatic benefits in previous clinical studies; however, real-world evidence from Japanese clinical practice remains limited. This study aimed to evaluate the effectiveness and safety of a standardized four-session Li-ESWT protocol in patients with treatment-refractory CP/CPPS at a Japanese outpatient urology clinic. METHODS: This retrospective, single-center observat

Isolated Prostatic Tuberculosis Coexisting with Benign Prostatic Hyperplasia: A Case Report.

BACKGROUND: Prostatic tuberculosis is a rare manifestation of genitourinary tuberculosis and an uncommon form of extrapulmonary tuberculosis. Because its clinical presentation is often nonspecific and closely resembles benign prostatic hyperplasia (BPH), chronic prostatitis, or prostate carcinoma, the diagnosis is frequently delayed and often established only after histopathological examination of prostatic tissue. Reporting such cases is important to increase clinical awareness and facilitate timely diagnosis, particularly in tuberculosis-endemic regions. CASE PRESENTATION: A 70-year-old man

Low-intensity shockwave therapy in men's health: Insights from a systematic review on erectile dysfunction and CP/CPPS in the Middle East.

Low-intensity shockwave therapy (LiSWT) has gained attention as a non-invasive treatment for various men's health conditions, including erectile dysfunction (ED), Peyronie's disease (PD), and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). However, despite its increasing use, robust evidence supporting its effectiveness remains limited. This systematic review aims to evaluate the efficacy and safety of LiSWT in managing men's health conditions within the Middle East to provide region-tailored clinical recommendations. A systematic search was conducted using PubMed to identify studi

Geometric Feature Relationship-Based Knowledge Distillation for Ground Reaction Force Estimation.

No previous study has evaluated systematically the relationship between metabolic syndrome (MetS) and prostate-related symptoms and signs in young infertile men. We studied 171 (36.5 ± 8.3-years-old) males of infertile couples. MetS was defined based on the National Cholesterol Education Program Third Adult Treatment Panel. All men underwent hormonal (including total testosterone (TT) and insulin), seminal (including interleukin-8 (IL-8), seminal plasma IL-8 (sIL-8)), scrotal and transrectal ultrasound evaluations. Because we have previously assessed correlations between MetS and scrotal param

A large-scale multi-ancestry genome-wide association study of chronic prostatitis/chronic pelvic pain syndrome in men.

Chronic prostatitis/chronic pelvic pain syndrome is common, and it impacts men's health and quality of life. The genetic basis of this condition remains largely unknown. Here, we conduct a GWAS using data from the Million Veteran Program of over 590,000 men of European, African, and Hispanic ancestry, including 14,575 chronic prostatitis/chronic pelvic pain syndrome cases. The multi-ancestry analysis identifies eight novel loci associated with chronic prostatitis/chronic pelvic pain syndrome risk, an increase from three significant genome-wide loci found in the European participants alone. We

Reliability of Negative Prostate MRI for Biopsy Decision-Making in the Male Han Chinese Population.

BACKGROUND: Multiparametric magnetic resonance imaging (mpMRI) has been widely utilized in clinical practice for identifying clinically significant prostate cancer (csPCa). Although mpMRI demonstrates a pooled negative predictive value (NPV) of 90%, additional clinical parameters require evaluation to enhance this metric specifically for the Chinese population-given the rising incidence of PCa in China, as well as ethnic differences in average prostate volume (PV) and chronic prostatitis prevalence that may impact mpMRI's diagnostic performance. METHODS: A retrospective analysis was conducted

Current research hotspots and difficulties of chronic prostatitis/chronic pelvic pain syndrome: neuroendocrine mechanism.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) presents a significant clinical challenge in urology. Traditional pathophysiological models emphasize infection and local inflammation; however, the limited efficacy of conventional therapies suggests the involvement of deeper regulatory mechanisms. METHODS: This comprehensive review synthesizes evidence from the past five years regarding neuroendocrine pathways in CP/CPPS. We systematically analyzed literature from the PubMed, Web of Science, and CNKI databases, focusing on neuropeptide functions, hypothalamic-pituitary-a

Adipokine Metabolic Drivers, Gut Dysbiosis, and the Prostate Microbiome: Novel Pathway Enrichment Analysis of the Adiposity-Based Chronic Disease-Prostate Cancer Network.

Adiposity-Based Chronic Disease (ABCD) is known to increase the risk of aggressive prostate cancer (PCa), recurrent disease after treatment for localized PCa, and PCa mortality. A key mechanistic link contributing to this enhanced risk is chronic inflammation originating from excess white visceral adipose tissue (WAT; VAT) and periprostatic adipose tissue (ppWAT). Contributing to systemic inflammation is gut dysbiosis, which itself may be caused by ABCD as well as background local inflammation (prostatitis), which is common in aging men and may be exacerbated by the urinary microbiome.
