Intravesical Instillation of Hyaluronic Acid (Cystistat) for the Treatment of Category III Prostatitis: A Retrospective Study.

BACKGROUND: Chronic nonbacterial prostatitis/chronic pelvic pain syndrome (CP/CPPS), classified as National Institutes of Health (NIH) category III prostatitis, is a common urological disorder, accounting for 90%-95% of prostatitis cases. Its pathogenesis involves inflammatory cascades along the bladder-prostate axis, neurogenic inflammation, and impairment of the mucosal barrier. Sodium hyaluronate (Cystistat), a glycosaminoglycan (GAG) layer replenishment agent, has demonstrated efficacy in treating bladder pain syndrome. However, its therapeutic role in CP/CPPS remains underexplored. OBJEC

Dietary patterns and chronic prostatitis: a symptom severity prediction model based on nutritional clustering and machine learning.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) has a multifactorial etiology where diet is considered an important factor. This study aimed to develop a predictive model for CP/CPPS symptom severity by analyzing food frequency questionnaire (FFQ) data with machine learning techniques, providing a basis for personalized nutritional interventions. METHODS: This study included 313 patients with CP/CPPS. We used principal component analysis (PCA) to extract dietary patterns from FFQ data and applied LASSO regression to select key predictors of symptom severity. Subsequentl

Epigenetic Regulation of Immune Dysfunction in Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS).

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent and debilitating condition with unclear etiology. Increasing evidence implicates immune dysregulation, yet the molecular mechanisms underlying impaired immune regulation remain poorly defined. This study investigated the role of adaptive immune responses and DNA methylation in CP/CPPS pathogenesis. METHODS: Voided bladder 3 (VB3) urine samples from CP/CPPS patients and healthy controls were analyzed for CD4⁺ T cell markers and lineage-defining transcription factors. Optimizing the Use of Extracorporeal Shock Wave Therapy for CP/CPPS: A Modality-Based Systematic Review and Meta-Analysis Comparing Focused and Radial Devices.

Background/Objectives: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a multifactorial condition often refractory to standard medical therapy. Low-intensity extracorporeal shock wave therapy (Li-ESWT) is a mechanism-oriented option; however, prior reviews reported substantial heterogeneity, potentially due to pooling different wave-generator modalities despite their distinct physical properties. This study synthesized randomized evidence on Li-ESWT for CP/CPPS and explored a wave-generator modality as a prespecified effect modifier. Methods: PubMed, Embase, Web of Science, and t

CXCR4 inhibition alleviates prostatic inflammation and pelvic pain via suppressing Th17 cell differentiation and oxidative stress in EAP mice.

Background: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) represents a prevalent urological disorder characterized by urinary symptoms, persistent pelvic or perineal discomfort accompanied by intraprostatic leukocyte infiltration. The C-X-C chemokine receptor type 4 (CXCR4) is critically involved in mediating inflammatory responses. Nevertheless, the specific involvement of CXCR4 in the immunoinflammatory mechanisms underlying CP/CPPS pathogenesis remains poorly characterized. Methods: The therapeutic efficacy of AMD3100, a CXCR4 antagonist, in CP/CPPS was evaluated in a murine mo

The exosome-autophagy axis: emerging mechanisms and therapeutic implications in type III prostatitis.

Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS) is characterized by chronic pelvic pain and urinary symptoms that imposes a significant quality-of-life burden, but the pathogenesis of this syndrome is poorly known, preventing effective therapy. Exosomes, small extracellular vesicles, which mediate cellular communication through proteins, cytokines, and miRNAs, are potent immune modulators that promote prostate inflammation and fibrosis. Simultaneously, dysregulated autophagy, a conserved degradation pathway essential for cellular homeostasis, is involved in the chronic inflammation

Slow‑releasing CO donor CORM‑A1 modulates depression‑like behavior in experimental model of chronic prostatitis/chronic pelvic pain syndrome.

Chronic pain conditions are often linked with depression, a common chronic mental health disorder that severely impairs quality of life. This association is particularly present in chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). CP/CPPS is a prevalent urological disorder characterized by pelvic pain accompanied by neuropsychiatric comorbidities such as  depression, for which underlying pathophysiological mechanisms remain unclear. Recently, anti‑inflammatory and neuroprotective effects of carbon monoxide‑releasing molecules (CORMs) have been demonstrated. The aim of this study was

Research progress on the multidimensional mechanisms underlying impaired spermatogenesis induced by pathological microenvironment remodeling in chronic prostatitis.

Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS), a prevalent urological and andrological condition among men of reproductive age, induces persistent pathological alterations. These alterations remodel the microenvironment of the prostate and reproductive tract through multiple pathways, thereby severely impairing sperm spermatogenesis, maturation, and function. By constructing a multidimensional interaction network encompassing "inflammation-oxidation-endocrine-microbiota, " this article elucidates the four core pathological mechanisms by which the CP/CPPS microenvironment damages t

Estimating minimal, moderate, and marked clinically important differences for patient reported outcome measure in randomised controlled trial.

BACKGROUND: Patient-reported outcome measures (PROMs) are increasingly used in clinical trials, yet thresholds that define clinically important difference (CID) within patients and treatment effects beyond placebo remain inconsistently reported, limiting its utility in clinical research and practice. Using chronic pelvic pain and its core PROM as a worked example, the study aims to present a structured framework for CID thresholds that separates within-group change from between-group difference in treatment effect, each further stratified as minimal, moderate, and marked. METHODS: We used dat
