Neurobiology of Anxiety and Depression in CP/CPPS: A Narrative Review of Underlying Mechanisms.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent urological disorder characterized by persistent pelvic pain, urinary symptoms, and significant impact on quality of life. In addition to its clinical symptoms, CP/CPPS is frequently associated with psychiatric comorbidities, such as anxiety and depression, indicating complex neurobiological mechanisms. This review explores the mechanisms linking CP/CPPS with affective disorders, emphasizing central nervous system alterations, dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, and neuroimmune interactions. Epigenetic Regulation of Immune Dysfunction in Chronic Prostatitis/Chronic Pelvic Pain Syndrome.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent and debilitating condition with unclear etiology. Increasing evidence implicates immune dysregulation, yet the molecular mechanisms underlying impaired immune regulation remain poorly defined. This study investigated the role of altered immune responses within PBMC populations and DNA methylation in CP/CPPS pathogenesis. METHODS: Post-prostatic massage urine samples designated as Voided Bladder 3 (VB3) urine from CP/CPPS patients and healthy controls were analyzed for CD4+ T cell markers and lineage-defining

The microbiome across the prostate disease continuum: from health and BPH to prostatitis/CPPS and cancer.

Microbial contributions to prostate health and disease extend beyond the mere detection of organisms in urine or tissue. Rather than acting as stable colonisers, microbial influences on the prostate are better conceptualised as converging fluxes: systemically circulating gut-derived metabolites, immune education occurring in distal lymphoid compartments, and intermittent exposure to microbial products from the lower urinary tract. These inputs converge on a limited set of conserved mediator-receptor axes-including short-chain fatty acids, bile acids and indole derivatives-that calibrate epithe

Aquablation for dual burden disease - BPH and chronic prostatitis/chronic pelvic pain syndrome: a single-center prospective pilot study.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) accounts for approximately 90-95% of prostatitis cases and is defined by pelvic pain or discomfort persisting for at least three of the preceding six months in the absence of identifiable infection. Despite its high prevalence, management remains challenging, and no standardized treatment approach has been established. In men with concomitant benign prostatic hyperplasia (BPH), bladder outlet obstruction may contribute to symptom persistence or severity. Aquablation, a robot-assisted, waterjet-based therapy used for the tre

Myricetin attenuates experimental autoimmune prostatitis and inflammatory and oxidative injury in LPS-induced RWPE-1 cells potentially through modulation of MAPK/STAT3/NF-κB/Nrf2 signaling pathway.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent urological condition marked by ongoing pelvic pain, inflammation, and a reduced quality of life. Increasing evidence indicates that dysregulated immune responses and oxidative stress contribute to CP/CPPS pathogenesis. Myricetin (MYR), a natural flavonoid, has been reported to exert anti-inflammatory and antioxidant effects in multiple disease models. However, its role in CP/CPPS remains unclear. METHODS: The protective effects of MYR were evaluated in an experimental autoimmune prostatitis (EAP) mouse model

Advances of Polymerase Chain Reaction Sperm Testing in Men With Chronic Prostatitis and Pelvic Pain Syndrome.

Prostate inflammation is a common condition in men characterized by swelling of the prostate gland, often associated with other prostate diseases. Understanding the role of chronic inflammation in prostatic diseases is important due to the changes in prostatic cells and the persistence when undiagnosed. The evaluation and management of chronic prostatitis (CP) and chronic pelvic pain (CPP) involve specific diagnostic tests. This study aimed to evaluate the efficiency of the polymerase chain reaction (PCR) detection kits that use multiplex real-time PCR in comparison to standard microbiology sp

IL-6 Exacerbates Experimental Autoimmune Prostatitis by Disrupting STAT5a-Mediated Treg Cell Function and Th17/Treg Balance.

OBJECTIVE: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common urological condition in men. Although disruption of the Th17 (T helper 17)/Treg (regulatory T) balance has been implicated in its pathogenesis, the upstream drivers of this immune imbalance remain incompletely understood. This study investigated whether interleukin-6 (IL-6) is associated with altered Th17/Treg homeostasis and impaired Treg function in experimental autoimmune prostatitis (EAP). METHODS: An EAP model was generated in NOD mice through immunization with prostate antigen. Th17/Treg cell profiles were

Aggressive chronic prostatitis mimicking locally advanced prostate cancer with apparent extraprostatic extension: a case report.

BACKGROUND: Multiparametric prostate MRI (mpMRI) is widely used for prostate cancer detection and local staging. Imaging features such as marked diffusion restriction, intense contrast enhancement, and apparent extraprostatic extension are typically associated with aggressive malignancy. However, inflammatory conditions may rarely mimic locally advanced prostate cancer, potentially leading to overstaging and inappropriate treatment. CASE PRESENTATION: A 49-year-old man presented with nonspecific lower urinary tract symptoms and a serum prostate-specific antigen level of 2.7 ng/mL. mpMRI revea

Transcutaneous auricular vagus nerve stimulation for moderate to severe chronic prostatitis/chronic pelvic pain syndrome (TASC-P) in China: study protocol for a randomised controlled trial.

INTRODUCTION: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) affects around 35%-50% of men during their lifetime. The efficacy of current oral medication for CP/CPPS remains limited. Recent studies demonstrated that vagus nerve stimulation may improve chronic pelvic and abdominal pain. Accordingly, transcutaneous auricular vagus nerve stimulation (taVNS) might represent a promising, non-invasive therapeutic approach for the clinical management of CP/CPPS. METHODS AND ANALYSIS: The trial of Transcutaneous Auricular vagus nerve Stimulation for moderate to severe Chronic Prostatitis/
