N-acetylcysteine alleviates experimental autoimmune prostatitis in rats by activating PI3K/Akt/CREB and Keap1/Nrf2 pathways.

BACKGROUND: The pathogenesis of non-bacterial chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) remains incompletely understood. Aberrant autoimmune responses have emerged as a prominent hypothetical contributor to CP/CPPS development. N-acetylcysteine (NAC), a well-established antioxidant agent, has demonstrated therapeutic efficacy in multiple disease models. This study aimed to systematically investigate the protective effects of NAC in experimental autoimmune prostatitis (EAP) rat models, with parallel exploration of its molecular mechanisms. METHODS: Prostate tissues from ten ra

Acupuncture-Related Interventions for Chronic Prostatitis/Chronic Pelvic Pain Syndrome: A Systematic Review and Network Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Chronic prostatitis (CP) is a common condition for which pharmacological therapies often provide unsatisfactory outcomes. Acupuncture has emerged as a promising supportive therapy. However, further comparative studies are needed to clarify the relative effectiveness of different acupuncture modalities versus standard pharmacological therapies, particularly because most existing trials used active rather than sham-controlled comparisons. METHODS: This network meta-analysis (PROSPERO: CRD420251180258) evaluated acupuncture-related therapies for CP. Randomized controlled trials (RCTs

Comparative Effectiveness of Chinese Patent Medicines for Chronic Prostatitis/Chronic Pelvic Pain Syndrome: A Bayesian Network Meta-Analysis.

OBJECTIVE: Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS) presents with persistent pelvic pain, urinary symptoms, and psychological burden, severely affecting patients' quality of life. While antibiotics and α-blockers are commonly prescribed, their long-term efficacy is limited and adverse reactions are frequent. Chinese patent medicines (CPMs) have emerged as promising alternatives, yet the lack of head-to-head evidence among formulations hinders evidence-based clinical recommendations. METHODS: Eight databases-CNKI, WanFang, VIP, SinoMed, PubMed, Web of Science, Embase, and the

Alteration in cerebral cortex thickness and structural covariance networks in patients with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS).

BACKGROUND: Recent studies have reported regional brain structural alterations in patients with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). However, patterns of cortical thickness (CT) changes and the topological organization of structural covariance networks (SCNs) in CP/CPPS remain insufficiently understood. This study aimed to investigate CT alterations and SCN organization in CP/CPPS using structural magnetic resonance imaging and graph theoretical analysis. METHODS: A total of 31 patients with CP/CPPS and 28 healthy controls (HCs) were included. All participants underwent

Prostatic vibratory stimulation for refractory chronic prostatitis: A case report.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a challenging condition with limited effective treatments. We report a case of a 44-year-old man with refractory CP/CPPS treated with a home-based prostatic vibratory stimulation protocol. After 12 weeks, the NIH-CPSI score improved from 26 to 14, with marked reduction in pain and no adverse events. The patient reported subjective improvement on the Patient Global Impression of Improvement scale. This case suggests that targeted mechanical prostatic stimulation may represent a promising non-pharmacological option for selected patien

Research progress on the mechanism, diagnosis, and treatment of exosomal miRNA in chronic prostatitis.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common condition that significantly impairs men's quality of life. Its pathogenesis remains incompletely understood, and there is a lack of specific diagnostic methods. As important signaling molecules in intercellular communication, microRNAs (miRNAs) in exosomes play a key role in the pathological progression of CP/CPPS. This review proposes that 'exosome miRNA-mediated interactions within the local urogenital microenvironment' constitute the core mechanism driving the chronicity and localization of CP/CPPS. We provide a compreh

Artemisinin ameliorates experimental autoimmune prostatitis by suppressing NLRP3 inflammasome-induced pyroptosis via the Nrf2/HO-1 axis.

Chronic prostatitis (CP) is common in young males. The etiology is unclear and treatment options are limited. Pyroptosis, a newly identified type of cell death, has been linked to CP pathogenesis, warranting further investigation. Here, the therapeutic effects of artemisinin (ART) on chronic non-specific inflammation were assessed. Comprehensive analyses, including evaluation of chronic pain progression, histopathology, and cytokine levels, revealed that ART significantly and dose-dependently suppressed NOD-like receptor protein 3 (NLRP3) inflammasome-mediated pyroptosis and alleviated patholo

Microbiota-derived short-chain fatty acids are associated with symptoms in chronic prostatitis/chronic pelvic pain syndrome through the production of proinflammatory cytokines: Results from a comparative study.

In recent years we have witnessed increasing interest for the role of microbiota in the pathophysiology and management of prostatic diseases. However, there are several aspects to comprehend. Here, we aim to assess the gut microbiota composition and faecal microbiota-derived short-chain fatty acids production (SCFAs) in the pathogenesis of CP/CPPS. All patients with CP/CPPS attending our urology centre, were enrolled in this study, and underwent urological examination, microbiological evaluation, intestinal microbiota analysis and measurement of interleukins in semen. A cohort of subjects who

Definition and Clinical Symptoms of Interstitial Cystitis/Bladder Pain Syndrome.

There is a need for a proper and uniform understanding of the clinical entity, Interstitial cystitis/Bladder pain syndrome (IC/BPS). The disease is primarily a clinical condition and therefore it is important to understand the definition and clinical features of this disease entity. The definitions have evolved, and the subject is still evolving. The definition coined by the Global Interstitial Cystitis/Bladder Pain Syndrome Society (GIBS) has become popular. The clinical features are the way to diagnose, and stepwise history and examination lead a clinician to the diagnosis. The clinical feat
