Prostatic urethral lift in young men seeking preservation of ejaculatory function and fertility-related parameters: semen analysis up to 18 months and comparative review of MIST.

BACKGROUND: Minimally invasive surgical therapies (MIST) aim to relieve lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH) while preserving sexual and ejaculatory function. We prospectively assessed semen parameters up to 18 months after prostatic urethral lift (PUL) in men under 50 prioritizing ejaculation and contextualized these findings against other MIST. METHODS: In this single-center cohort (N.=20), semen analyses (according to WHO 6th edition) were performed at baseline, 6, and 18 months after PUL. Functional outcomes were prospectively collected through uro

The koala (Phascolarctos cinereus) prostate: a proteomic perspective on gland segmentation.

Australia's most iconic animal, the koala (Phascolarctos cinereus), faces significant population decline and while conservation has focused on assisted breeding technology and reproductive pathologies, koala reproductive biology remains poorly understood. In particular, the koala (P. cinereus) prostate has a vital role in the production of seminal plasma and facilitating successful reproduction. Furthermore, prostatitis regularly occurs as a consequence of chlamydiosis, a substantial factor in the ongoing decline of koala populations. Despite this, little is known about the koala prostate's mo

The relationship between seminal microbiome and male reproductive wellness: a systematic review†.

The seminal microbiome, which is composed of different types of bacteria in semen and seminal plasma, has a significant impact on male reproductive health by changing the quality of semen and fertility. Previously regarded as sterile, the male reproductive tract contains microbes originating from the gastrointestinal tract, reproductive organs, and external sources such as sexual partners. Dysbiosis alters sperm parameters, triggers inflammation and oxidative stress, and is associated with conditions such as infertility, HPV infection, prostatitis, prostate cancer, and azoospermia. This system

Advances of Polymerase Chain Reaction Sperm Testing in Men With Chronic Prostatitis and Pelvic Pain Syndrome.

Prostate inflammation is a common condition in men characterized by swelling of the prostate gland, often associated with other prostate diseases. Understanding the role of chronic inflammation in prostatic diseases is important due to the changes in prostatic cells and the persistence when undiagnosed. The evaluation and management of chronic prostatitis (CP) and chronic pelvic pain (CPP) involve specific diagnostic tests. This study aimed to evaluate the efficiency of the polymerase chain reaction (PCR) detection kits that use multiplex real-time PCR in comparison to standard microbiology sp

IKK2, calcium, MAP kinase, and PI3 kinase are required for exocytosis and interleukin-8 production in human mast cells stimulated by Trichomonas vaginalis-derived secretory products.

Trichomonas vaginalis infection causes vaginitis and cervicitis in women, and asymptomatic urethritis and prostatitis in men. Mast cells play a key role in the inflammatory response against T. vaginalis infection. In this study, we examined the signaling pathways involved in mast cell activation induced by T. vaginalis-derived secretory products (TvSP), focusing on IKK2, calcium, MAP kinase (MAPK), and PI3 kinase (PI3K). TvSP stimulation induced phosphorylation and degradation of IκB, indicating NF-κB activation, and triggered phosphorylation of ERK1/2, p38 MAPK, and AKT. TvSP also increased t

Does It Make Sense to Perform Prostate Magnetic Resonance Imaging in Men with Normal PSA (<4 ng/mL)?

Objective: We evaluate the performance and relevance of MRI to detect csPC in men with normal PSA. Methods: Out of our database of patients referred for prostate MRI, we selected men with PSA < 4 ng/mL for whom histopathology or at least 2 years of clinical follow-up data were available as standard of reference. Subgroup analyses were performed for the patients with PSA < 3 ng/mL, <2 ng/mL, and 2-3.9 ng/mL. The reasons for prostate MRI referral despite their normal PSA level were retrieved by exploring the patients' files. The prostate MRIs were reported according to the Prostate Imaging and R

Cepharanthine alleviated chronic prostatitis/chronic pelvic pain syndrome by activating Nrf2 and inhibiting the NF-κB pathway.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common disorder impacting the genitourinary system of adult males, primarily driven by pathogenic mechanisms involving immune and inflammatory mediators, immune cells and molecules, oxidative stress, and apoptosis. Cepharanthine (CEP), a bisbenzylisoquinoline alkaloid, exhibits properties such as immunomodulatory, antioxidant, anti-inflammatory, and antitumor activities. Nevertheless, the effects of CEP on CP/CPPS and its underlying mechanisms remain insufficiently characterized. METHODS: Two experimental models were d

Cell metabolomics to reveal the mechanism of saponins and alkaloids on prostate inflammation via COX and LOX pathways.

Inflammation initiates and progresses due to overexpression of the arachidonic acid cascade mediators. Cyclooxygenase (COX) and lipoxygenase (LOX) are considered to be the primary targets, and the aim of this study was to assess the anti- inflammatory activity of several herbal compounds as dual COX-LOX inhibitors on prostatic diseases. Compounds Timosaponin AⅡ, timosaponin AⅢ, berberine and demethyleneberberine were selected according to our previous studies. Human prostate cancer cells (PC-3) were cultured and the cell viability was detected by CCK-8 method at different concentrations. Ultra

Comparative Effectiveness of Chinese Patent Medicines for Chronic Prostatitis/Chronic Pelvic Pain Syndrome: A Bayesian Network Meta-Analysis.

OBJECTIVE: Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS) presents with persistent pelvic pain, urinary symptoms, and psychological burden, severely affecting patients' quality of life. While antibiotics and α-blockers are commonly prescribed, their long-term efficacy is limited and adverse reactions are frequent. Chinese patent medicines (CPMs) have emerged as promising alternatives, yet the lack of head-to-head evidence among formulations hinders evidence-based clinical recommendations. METHODS: Eight databases-CNKI, WanFang, VIP, SinoMed, PubMed, Web of Science, Embase, and the

KSHV and Human Diseases: Beyond KS, PEL and MCD.

Kaposi's Sarcoma-associated herpesvirus (KSHV) has been etiologically linked to several human cancers, including Kaposi's sarcoma (KS), primary effusion lymphoma (PEL), and multicentric Castleman's disease (MCD). However, recent studies suggest that KSHV infection may also be associated with the development of other diseases or increased risks, such as KSHV inflammatory cytokine syndrome (KICS), diabetes, malaria, heart disease, and other cancers. In this review, we summarize these findings from clinical observations, epidemiological studies or laboratory research, though more studies are need

Development and explainability of a machine learning prediction model for histological prostatic inflammation in surgically treated patients with benign prostatic hyperplasia: a single-center internal validation study.

BACKGROUND: Histological prostatitis is highly prevalent among patients with benign prostatic hyperplasia (BPH) and has been shown to be closely associated with disease progression and postoperative outcomes. However, its diagnosis still mainly relies on postoperative or biopsy pathology, and there is a lack of non-invasive tools to estimate the risk of histological inflammation before surgery or biopsy. METHODS: This was a single-center retrospective machine-learning prediction model study with internal validation, involving 723 patients admitted to the urology department of a tertiary medic

Hemozoin-induced immunomodulation and prostate carcinogenesis: mechanisms and implications.

Hemozoin (Hz) is a crystalline by-product, which is produced when the Plasmodium species destroy haemoglobin, and it is commonly recognised that it leads to how our bodies respond to malaria. Although we understand pretty well its immunological actions in infectious diseases, recent research indicates that chronic exposure to Hz may actually cause cancer by sustaining inflammation and altering the immune system. In this review, we examines how the relationship between the hemozoin induced immune changes contribute to prostate cancer development arises. The presence of Hz within the macrophages

Prostatic vibratory stimulation for refractory chronic prostatitis: A case report.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a challenging condition with limited effective treatments. We report a case of a 44-year-old man with refractory CP/CPPS treated with a home-based prostatic vibratory stimulation protocol. After 12 weeks, the NIH-CPSI score improved from 26 to 14, with marked reduction in pain and no adverse events. The patient reported subjective improvement on the Patient Global Impression of Improvement scale. This case suggests that targeted mechanical prostatic stimulation may represent a promising non-pharmacological option for selected patien

Systemic cancer risk profile in neovascular age-related macular degeneration: insights into shared aging-related mechanisms from a nationwide population-based study.

Neovascular age-related macular degeneration (nAMD) is a leading cause of vision loss in older adults and is increasingly recognized as a manifestation of systemic aging involving vascular and inflammatory pathways. Emerging evidence suggests that nAMD may also be linked to systemic diseases, including malignancies. Using data from the Korean National Health Insurance Service, we conducted a nationwide, population-based cohort study of 334,091 individuals (83,742 with nAMD and 250,349 matched controls) followed for up to 10 years. Patients with nAMD showed a modest but significant increase in
