Immune-cell imbalance and immunometabolic reprogramming in chronic prostatitis/chronic pelvic pain syndrome.

Available evidence suggests that disruption of immune homeostasis may contribute to chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) beyond the effects of localized infection. This narrative review uses immune cells as its organizing axis and synthesizes recent evidence from single-cell sequencing, Mendelian randomization, and multi-omics. The available findings suggest that the M1/M2 dichotomy may not fully capture macrophage heterogeneity in prostatic tissues and that activation of the NLRP3 inflammasome and IL-1β release may form a candidate link between innate immunity and the Th

Mechanism of Qianlie Zhitong Formula Against Chronic Prostatitis Based on Proteomics and Metabolomics.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a prevalent pelvic pain disorder in men, yet effective pharmacotherapies remain limited. Emerging evidence suggests that oxidative stress and dysregulated iron metabolism contribute to the pathogenesis of CP/CPPS, with ferroptosis increasingly implicated in CP/CPPS-related pathology. Qianlie Zhitong Formula (QLZTF) has shown therapeutic potential in treating CP/CPPS; however, its underlying mechanisms remain incompletely understood. This study integrated proteomics, metabolomics, and in vivo experiments to investigate the therapeuti

Postvoid Residual Urine as a Clue to the Diagnosis of Acute Bacterial Prostatitis: A Case Report.

Acute bacterial prostatitis (ABP) typically presents with fever and urinary symptoms; however, its diagnosis can be challenging in patients who lack characteristic findings such as perineal pain or prostatic tenderness. Although postvoid residual (PVR) urine may be associated with voiding dysfunction in ABP, its potential role as a diagnostic clue has received little attention. A 44-year-old man presented with a 2-day history of fever ranging from 38 to 39°C, irritative urinary symptoms, back pain, and difficulty initiating urination. At presentation, the patient was afebrile and had no perine

Prostatic abscess in a 16-year-old adolescent secondary to intraurethral water instillation: A case report and review of the literature.

While acute bacterial prostatitis is seen occasionally in adult males, the development of a prostatic abscess (PA) is exceedingly rare in pediatric and adolescent populations. When PA does occur in younger patients, congenital anomalies, immune compromise, trauma, or genitourinary instrumentation are commonly associated. We report a unique case of a 16-year-old boy who developed a prostatic abscess after deliberately instilling shower water into his urethra. This report details the clinical presentation, diagnostic workup, and treatment course of this patient, followed by a literature review o

Case Report: Prostatic infarction mimicking prostate cancer in a patient with markedly elevated prostate-specific antigen.

Prostatic infarction is an uncommon benign ischemic lesion that may closely mimic prostate cancer through marked prostate-specific antigen (PSA) elevation and focal prostate abnormalities. A 69-year-old man with long-standing hypertension presented with progressive lower urinary tract symptoms and urinary retention. Pre-catheterization postvoid residual urine volume was approximately 430 mL. Total PSA was 31.858 ng/mL, free PSA was 1.291 ng/mL, and the free-to-total PSA ratio was 0.04. Magnetic resonance imaging revealed a well-circumscribed left transition-zone nodule with a Prostate Imaging

Low-intensity extracorporeal shockwave therapy for chronic prostatitis/chronic pelvic pain syndrome: a retrospective single-center observational study in Japan.

BACKGROUND: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a heterogeneous condition that can remain symptomatic despite conventional therapy. Low-intensity extracorporeal shock wave therapy (Li-ESWT) has shown symptomatic benefits in previous clinical studies; however, real-world evidence from Japanese clinical practice remains limited. This study aimed to evaluate the effectiveness and safety of a standardized four-session Li-ESWT protocol in patients with treatment-refractory CP/CPPS at a Japanese outpatient urology clinic. METHODS: This retrospective, single-center observat

Assessment of Urological Training Gaps Among Primary Care Residents: A Cross-Sectional Survey Study.

IntroductionAging global populations are driving an increased demand for urological care, yet the healthcare system faces a projected decline in urologist availability per capita. This shift positions primary care providers as the essential frontline for initial evaluation and management. This study aimed to assess clinical exposure, knowledge, and self-reported comfort levels regarding common urological conditions among family and internal medicine residents.MethodsA 76-item cross-sectional survey was administered between August 2023 and June 2024 to family medicine and internal medicine resi

From Bacterial Infection to Emerging Microbiome Signals in Prostatitis: A Bibliometric Analysis, 1985-2026.

Prostatitis research has expanded from conventional bacterial infection models to microbiome approaches. This expansion has not resolved uncertainty about the scale of the newer literature or the visibility of clinical and methodological information in indexed fields. To quantify this expansion, this bibliometric analysis mapped publication patterns and assessed prespecified indexed-field indicators in the modern microbiome, gut microbiome, and Mendelian randomization (MR) subcorpora. The Web of Science Core Collection was searched on June 2, 2026. Eligible records were English-language Articl

Isolated Prostatic Tuberculosis Coexisting with Benign Prostatic Hyperplasia: A Case Report.

BACKGROUND: Prostatic tuberculosis is a rare manifestation of genitourinary tuberculosis and an uncommon form of extrapulmonary tuberculosis. Because its clinical presentation is often nonspecific and closely resembles benign prostatic hyperplasia (BPH), chronic prostatitis, or prostate carcinoma, the diagnosis is frequently delayed and often established only after histopathological examination of prostatic tissue. Reporting such cases is important to increase clinical awareness and facilitate timely diagnosis, particularly in tuberculosis-endemic regions. CASE PRESENTATION: A 70-year-old man

Low-intensity shockwave therapy in men's health: Insights from a systematic review on erectile dysfunction and CP/CPPS in the Middle East.

Low-intensity shockwave therapy (LiSWT) has gained attention as a non-invasive treatment for various men's health conditions, including erectile dysfunction (ED), Peyronie's disease (PD), and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). However, despite its increasing use, robust evidence supporting its effectiveness remains limited. This systematic review aims to evaluate the efficacy and safety of LiSWT in managing men's health conditions within the Middle East to provide region-tailored clinical recommendations. A systematic search was conducted using PubMed to identify studi

Prostatic urethral lift in young men seeking preservation of ejaculatory function and fertility-related parameters: semen analysis up to 18 months and comparative review of MIST.

BACKGROUND: Minimally invasive surgical therapies (MIST) aim to relieve lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH) while preserving sexual and ejaculatory function. We prospectively assessed semen parameters up to 18 months after prostatic urethral lift (PUL) in men under 50 prioritizing ejaculation and contextualized these findings against other MIST. METHODS: In this single-center cohort (N.=20), semen analyses (according to WHO 6th edition) were performed at baseline, 6, and 18 months after PUL. Functional outcomes were prospectively collected through uro

From imaging enhancement to surgical autonomy: the evolutionary role of nanotechnology in AI-driven robotic surgery.

Robot-assisted minimally invasive surgery provides stable visualisation and manipulation, but direct access to local tissue state remains limited. Lesion boundaries, mechanics, molecular activity and microenvironmental changes are often inferred from conventional feedback. This review evaluates how nano-enabled imaging, flexible and local sensing, and micro/nanorobotic intervention may extend signals and localised actions in robot-assisted procedures. We organise the evidence along an information-flow framework: signal acquisition, processing or computational interpretation, task-relevant stat

Geometric Feature Relationship-Based Knowledge Distillation for Ground Reaction Force Estimation.

No previous study has evaluated systematically the relationship between metabolic syndrome (MetS) and prostate-related symptoms and signs in young infertile men. We studied 171 (36.5 ± 8.3-years-old) males of infertile couples. MetS was defined based on the National Cholesterol Education Program Third Adult Treatment Panel. All men underwent hormonal (including total testosterone (TT) and insulin), seminal (including interleukin-8 (IL-8), seminal plasma IL-8 (sIL-8)), scrotal and transrectal ultrasound evaluations. Because we have previously assessed correlations between MetS and scrotal param

Bushen Daozhuo Granules Alleviate Chronic Non-Bacterial Prostatitis in Rats through p38 MAPK and Akt Signaling Pathways Based on Tandem Mass Tag-Based Quantitative Proteomics and Network Pharmacology Analyses.

INTRODUCTION: The traditional Chinese medicine formula, Bushen Daozhuo Granules (BSDZG), is used to treat chronic non-bacterial prostatitis (CNP) clinically. However, its mechanism of action is unclear. The aim of our study was to determine the effect of BSDZG on CNP and its underlying mechanisms. METHODS: Male Wistar rats were randomly assigned to control, CNP, and BSDZG groups. CNP was induced using purified prostaglandin solution and Freund's complete adjuvant, after which the BSDZG group received 1.54 g/kg/d of BSDZG for 30 days. Prostate tissues were used to determine apoptosis and infla
