PSA testing and receipt of curative treatment in elderly Japanese men with prostate cancer: a population-based study.

OBJECTIVE: This study aimed to evaluate whether regular prostate-specific antigen (PSA) testing is associated with the receipt of curative treatment for prostate cancer among men in Japan and to identify associated factors. METHODS: A retrospective analysis was conducted using the claims data from the National Health Insurance and Late-Stage Elderly Health Care System in Saga Prefecture between March 2014 and March 2022. Patients were categorized into PSA testing (≥ 1 test ≥ 6 months before treatment) and non-PSA testing groups. Curative treatment was defined as either a radical prostatectomy

Integrated Computational Design and Experimental Approach for the Discovery of Novel Steroidal 5α-Reductase Inhibitors for Benign Prostatic Hyperplasia.

Advancements in computational drug design have enabled rapid identification of lead molecules with improved precision and reduced experimental attrition. In this study, an integrated in silico-in vitro approach was employed to design steroidal inhibitors targeting 5α-reductase type 2 (5-AR2), a key enzyme involved in benign prostatic hyperplasia (BPH). A ligand-based pharmacophore model (ADHHHR_1), developed from known inhibitors, guided the design of androstane derivatives. Subsequent 3D-QSAR and molecular docking against 5-AR2 (PDB ID: 7BW1) identified NDPR-2 and NDPR-5 as promising candidat

Primary tumor-derived lactate primes prostate cancer cells for a dormant fate.

The metastatic outcome depends on the state of disseminated cancer cells (DCCs) leading to overt metastasis or undergoing dormancy. Before distant organ colonization, primary tumor microenvironment may shape the decision between proliferation and dormancy. Here, we identify lactic acid (LA), a highly abundant environmental metabolite at the primary site in prostate cancer (PCa), as a key inducer of dormant state. LA-uploading PCa cells undergo a proliferation delay, associated with stemness, therapy tolerance, and invasiveness through the nuclear receptor subfamily 2 group F member 1 (COUP-TF1

The clinical significance of the modified Naples prognostic score for prostate cancer: a population-based study.

PURPOSE: The Naples Prognostic Score (NPS) is a widely recognized scoring system used to assess inflammation and nutrition status, yet its role in prostate cancer (PCa) remains unclear. This study aims to investigate the association between the modified NPS (MNPS) and PCa prevalence and mortality outcomes among participants with self-reported PCa. METHODS: Data from the National Health and Nutrition Examination Survey 1999-2018 were collected. The NPS was calculated using MaxStat analysis, while M1NPS and M2NPS were constructed with X-Tile analysis. Weighted logistic regression was used to es

Endoplasmic reticulum-mitochondria contact sites in benign prostatic hyperplasia: evidence integration and mechanistic hypotheses.

Benign prostatic hyperplasia (BPH) is a common chronic and progressive disease in ageing men. Its pathobiology involves androgen-signaling remodeling, chronic inflammation, oxidative stress, metabolic disturbance, and dysregulated cell fate control; however, the organelle-level communication that connects these processes remains insufficiently defined. Endoplasmic reticulum (ER)-mitochondria contact sites, commonly referred to as mitochondria-associated membranes (MAMs), are dynamic membrane contact platforms that coordinate ER stress with mitochondrial homeostasis. Through the regulation of C

The need for fully-effective federated analytics of data sources for clinical trials.

INTRODUCTION: Clinical trials are usually analysed in a single environment allowing for flexible analysis including adjustment or stratification by subgroup: 'one-stage' analysis of individual-level data. Health systems datasets, often distributed across geography and providers, can streamline clinical trials. Data are increasingly accessible in secure data environments (SDEs). Future trial analyses may involve working across multiple SDEs. Row-level data and identifiable data often cannot leave, requiring a 'two-stage approach', where summary data from each SDE are meta-analysed. OBJECTIVE:

A Graphene Oxide-Doped CuMOF Nanoplatform Enhances Chemodynamic Therapy and Suppresses CYP2J2-Mediated Linoleic Acid Metabolism in Castration-Resistant Prostate Cancer.

PURPOSE: Castration-resistant prostate cancer (CRPC) responds poorly to current therapies. A tumor microenvironment-responsive nanoplatform was developed to couple chemodynamic therapy with modulation of oncogenic lipid metabolism. METHODS: A graphene oxide-doped copper-based metal-organic framework (GO-CuMOF) was prepared by ultrasonicating solvothermally synthesized CuMOF with graphene oxide, and was characterized physicochemically. Its generation of hydroxyl radicals (•OH) was confirmed by a methylene blue degradation assay. Antitumor activity was evaluated in DU145 and PC3 cells and in a

Integrating Mendelian randomization with single-cell and spatial transcriptomics to investigate gut microbiota-associated genetic signals and molecular characteristics in prostate cancer.

BACKGROUND: Prostate cancer (PRAD) represents one of the most prevalent malignancies among men globally. Accumulating evidence implicates gut microbiota dysbiosis in the initiation and progression of prostate cancer; however, the causal relationship and underlying molecular mechanisms remain incompletely elucidated. This study integrated Mendelian randomization (MR), bioinformatics, single-cell RNA sequencing, and spatial transcriptomics to systematically investigate the genetic associations between gut microbiota and prostate cancer risk and characterize the expression patterns of microbiota-

Recovering anaerobic bacterial genomes in saliva, tumour and blood whole genome sequencing data from patients with prostate cancer.

Microbial constituents in the gastrointestinal tract, the urine and prostate tissue are being linked with clinical differences in prostate cancer, and a recent meta-analysis has linked periodontal disease to this cancer. Several hypotheses exist regarding how specific bacteria can contribute towards the development of prostate cancer. Recovering microbial genomes, and their genomic features, from clinical samples and investigating fine-scale taxonomy is a critical first step in investigating causality. Here, we describe the microbial diversity of samples associated with prostate cancer. Metage

Lineage plasticity-driven immunometabolic state transitions in advanced prostate cancer: biomarkers and therapeutic stratification.

Advanced prostate cancer is commonly described as immunologically cold, yet that label obscures a disease continually remodelled by treatment. Androgen-receptor (AR) pathway inhibition, chemotherapy, DNA-damage-directed therapy and radioligand treatment select resistant clones while altering lineage identity, metabolic dependence, antigen presentation and metastatic niches. AR-high, AR-low, double-negative, amphicrine and treatment-emergent neuroendocrine phenotypes are therefore better viewed as evolving immunometabolic states than as fixed histologies. Here, we develop a lineage-metabolism-i

FMOD marks a low-malignancy epithelial state in prostate cancer: an ECM-related prognostic signature with single-cell and functional validation.

Extracellular matrix (ECM) remodeling promotes prostate cancer progression, while the prognostic value and cellular context of ECM-related genes (ERGs) remain unclear. A 4-gene ERG signature for disease-free interval prediction was built using TCGA-PRAD data, and the hub gene Fibromodulin (FMOD) was validated in an external cohort via scRNA-seq, bulk deconvolution and siRNA knockdown. The signature stratified patient prognosis, and FMOD exerted the strongest protective effect. Despite higher bulk expression in tumor tissues, FMOD predicted better clinical outcomes. FMOD was enriched in a low-c

Subnational inequalities and socioeconomic gradients in prostate cancer burden across Mexico, 2000-2023.

BACKGROUND: Prostate cancer is a major cause of cancer-related morbidity and mortality among men worldwide. In Mexico, substantial socioeconomic disparities across states may influence disease detection, access to care, and clinical outcomes. We evaluated national and subnational trends in prostate cancer burden between 2000 and 2023 and examined their relationship with socioeconomic marginalization. METHODS: We analyzed estimates from the Global Burden of Disease (GBD) Study 2023 for Mexico and its 32 states. Incident cases, deaths, disability-adjusted life years (DALYs), and age-standardize

Increased Expression of Desmin and Vimentin Impairs Mitochondrial Function and Reduces Contractility via G0S2 in Bladder Smooth Muscle.

Bladder outlet obstruction caused by benign prostatic hyperplasia in humans, as well as partial bladder outlet obstruction in animal models, is linked to reduced contractility and impaired mitochondrial function in bladder smooth muscle (BSM). However, the mechanisms underlying changes in BSM contractility and mitochondrial function are still largely unknown. Our previous study showed that increased expression of intermediate filament proteins desmin and vimentin weakens BSM contractility. In this study, we hypothesize that BSM contractile dysfunction, driven by elevated levels of desmin and v

Development and Validation of the JIKEI-YAYOI Score for Predicting Castration Resistance in Metastatic Castration-Sensitive Prostate Cancer Treated With ARSI-Based Doublet Therapy.

BACKGROUND: Metastatic castration-sensitive prostate cancer (mCSPC) is a heterogeneous disease with variable clinical outcomes despite advances in androgen receptor signaling inhibitor (ARSI)-based therapy. We aimed to develop and externally validate a prognostic model for predicting progression to castration-resistant prostate cancer (CRPC) in patients with mCSPC. METHODS: This multicenter retrospective study included patients with mCSPC treated with ARSI doublet therapy. The development cohort comprised 388 patients, whereas the external validation cohort included 451 patients. Prognostic f

Denosumab Therapy Duration and Osteonecrosis of the Jaw and Skeletal Events in Bone-Metastatic Prostate Cancer.

IMPORTANCE: Denosumab reduces skeletal-related events in bone-metastatic prostate cancer but carries a cumulative risk of medication-related osteonecrosis of the jaw (ONJ). The treatment duration that best balances these risks has not been established in clinical settings. OBJECTIVE: To evaluate the association between duration of denosumab treatment and the 3-year cumulative incidence of ONJ and skeletal events. DESIGN, SETTING, AND PARTICIPANTS: This nationwide retrospective cohort study emulated a target trial using Korean Health Insurance Review and Assessment Service data from January 1

[EARLY DETECTION OF BONE METASTASIS BY WHOLE-BODY MRI AND TRIPLET THERAPY AFTER RADICAL PROSTATECTOMY: A CASE REPORT].

(Background) Although radical prostatectomy is a curative treatment for localized prostate cancer, some high-risk patients experience early recurrence due to occult metastases undetected by conventional imaging. (Case presentation) A 53-year-old man with high-risk prostate cancer (cT2bN0M0, Gleason score 4+4, initial PSA 10.6 ng/mL) underwent robot-assisted radical prostatectomy with pelvic lymph node dissection. Pathology revealed Gleason score 4+5, pT3bN1 disease. At 1 month postoperatively, PSA increased to 13.6 ng/mL, and whole-body MRI detected a solitary right iliac bone metastasis that

Novel approaches to target prostate cancer stem cells driving therapy resistance and cancer progression.

Prostate cancer remains one of the most common malignancies in men, and its global burden continues to rise. Although androgen deprivation therapy and other standard treatments can provide substantial disease control, advanced tumors may progress to castration-resistant prostate cancer, which is associated with poor outcomes. Preclinical and translational evidence implicates prostate cancer stem-like cells (PCSCs) in treatment persistence, progression, and relapse, without establishing them as the sole cause of resistance in patients. PCSC-associated states share functional traits with normal

Spatial and Epigenetic Polarity of the AR Promoter During Prostate Cancer Progression: An In Silico Analysis.

The androgen receptor (AR) is a key regulator of prostate cancer progression, yet the relationship between AR promoter methylation, chromatin state, and receptor expression across distinct tumor phenotypes remains incompletely understood. We performed an integrative in silico analysis of the AR promoter region spanning -2000 to +500 bp relative to the transcription start site (TSS), combining motif predictions for 24 transcription factors (TFs) with DNA methylation, TF ChIP-seq, and histone modification profiles from normal prostate tissue, primary and metastatic tumors, and cell models. Immune-cell imbalance and immunometabolic reprogramming in chronic prostatitis/chronic pelvic pain syndrome.

Available evidence suggests that disruption of immune homeostasis may contribute to chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) beyond the effects of localized infection. This narrative review uses immune cells as its organizing axis and synthesizes recent evidence from single-cell sequencing, Mendelian randomization, and multi-omics. The available findings suggest that the M1/M2 dichotomy may not fully capture macrophage heterogeneity in prostatic tissues and that activation of the NLRP3 inflammasome and IL-1β release may form a candidate link between innate immunity and the Th
