Scrotal imaging with technetium-99m sodium pertechnetate consists of a radionuclide angiogram and static scrotal scans. Utilization of this study in patients presenting with an acute scrotum can dramatically reduce the number of surgical explorations for acute epididymitis. It can also aid in other aspects of differential diagnosis in patients presenting with either an acutely enlarged and/or painful scrotum or a scrotal mass. We reviewed 74 cases of scrotal exploration via an inguinal approach that were negative for malignancy. Of these explorations 24 (32 per cent) resulted in radical orchiectomy and 14 (19 per cent) resulted in simple orchiectomy. An additional 17 patients (23 per cent) had some other operation in addition to exploration. The most frequent final diagnoses were epididymitis (26 per cent), testicular torsion (18 per cent) and hydrocele (15 per cent). These data demonstrate that scrotal explorations to eliminate the possibility of malignancy result in a high orchiectomy rate for benign disease.

We present the clinical findings in the thirteenth recorded patient with lymphedema, pleural effusions, and ungual dystrophy--the so-called "yellow nail" syndrome. The rate of pleural-fluid turnover was measured by using radioiodinated albumin to trace the efflux of fluid by lymphatics. The rate of pleural lymphatic flow was low in comparison to previous estimates obtained by the same method in a variety of other conditions. Such slow lymphatic flow is consistent with the postulate that accumulation of pleural fluid in this syndrome is due to defective lymphatic drainage.

Ten cases of archenteronoma are reviewed with a follow-up between 2 mo and 7 yr. Their presentation was similar to that of rhabdomyosarcoma except for testicular tumors that may present as a hydrocele. A good prognosis was found with tumors amenable to primary resection and chemotherapy. Other tumors are best treated initially with chemotherapy before relatively conservative surgery, radiotherapy being used either pre- or postoperatively.

Ultrasound scans are presented in 2 cases of bilateral congenital hydroceles. In both cases, large scrotal sacs were identified and seen to contain prominent fluid collections. In one case, the testes were identified within the fluid. No testes were seen in the other patient, who subsequently proved to have bilateral undescended testicles.

Electron microscopy of testicular biopsies obtained from two adult males with tunica vaginal hydrocele revealed some protrusions from the basal lamina to the germinal epithelium in the seminiferous tubule. The protrusions were of three types: some between the spermatogonia and Sertoli cells, some directly within the Sertoli cell cytoplasm and others inside the spermatogonia. Two infants with spontaneous perforation of the common bile duct are described. One presented with mild jaundice, dark urine, acholic stools, and hydroceles, the other with bilateral inguinal hernia. Differential diagnosis of spermatic cord swellings includes direct inguinal hernia, hydrocele, spermatocele, hematocele, lipoma, tuberculosis, and filariasis. This communication focuses attention on other unusual swellings of the spermatic cord. Three cases of sarcoma, two cases of foreign body in the spermatic cord, and one case of vasitis are presented. Although unusual, these conditions should be considered in the differential diagnosis of spermatic cord swellings.

A new focus of onchocerciasis is reported from Upper Atbara River area in the Sudan. This river is a known seasonal breeding place of Simulium damnosum. The area was sparsely populated in the past but now, following agricultural development, is inhibited mainly by immigrants from Northern Nigeria and Chad.
