Testicular feminization syndrome, irreducible inguinal hernia, bowel obstruction, and testicular torsion were diagnosed and treated in a 13-year-old girl. Diagnosis of this case in such a complex form in an emergency room is a rare occurrence. Through this case, we want to emphasize the coincidence of inguinal hernia and testicular feminization and the X-linked recessive transmission character of the disease.

BACKGROUND: Acute scrotal pain is a common urological emergency. Urgent exploration is the standard means of management, as no investigation can confidently exclude torsion of testis from the differential diagnoses. In our department, all patients up to the age of 40 years, presenting with acute scrotal pain undergo emergency scrotal exploration.

Between January 1970 and December 1989, 171 patients underwent scrotal exploration for acute scrotum (an acute painful swelling of the scrotum or its content), at our hospital. Of 70 children scrotal explorations, torsion of appendages was found in 33 cases (47%). Torsion of testis was found in 24 patients (34%). A total of 31 patients with acute torsion treated over a fifteen-year period are reviewed. The peak incidence of the disease, its seasonal variations, predisposing factors, surgical procedure, and clinical outcome are analyzed. In retrospective study of 90 patients operated on for acute unilateral pains in scrotum region and its content--the so-called acute scrotum--during revision torsion of testis was found 48 times (53.3%), torsion of appendix testis 24 times (26.7%), acute epididymitis 17 times (18.9%) and testis tumour in one patient (1.1%). Data from Hospital Activity Analysis (HAA) for the Wessex Health Region show a 70% increase in hospital admissions for torsion of the testis between 1971 and 1982. In an experimentally established model of torsion of the testis, morphological and immunological observations were evaluated in 180 Wistar rats. A statistical study was performed on new outpatients. The total number of new outpatients in 1983 was 5,901 (male: 3,712, female: 2,189) and the male to versus female ratio was 1.7: 1.
