A Comparative Study of Genital Lichen Sclerosus Transcriptomes.

Genital lichen sclerosus (GLS) is a chronic inflammatory dermatosis that affects the genital skin. Despite different clinical manifestations, the pathogenesis of GLS in men and women is believed to be common and is attributed to a combination of autoimmune and genetic factors. In this study, we compared the transcriptomic profiles of penile (mGLS) and vulvar lichen sclerosus (VLS), aiming to identify commonly deregulated genes. We observed a substantial heterogeneity in the transcriptomic signatures in mGLS samples, which is driven by different compositions of immune infiltrates. In mGLS, gene

Idiopathic neonatal priapism in a 5-day-Old Somali neonate from a low-resource setting: A case report.

Neonatal priapism is an exceptionally rare and usually benign condition, most commonly representing a non-ischemic (high-flow) state that resolves spontaneously. We report a 5-day-old Somali neonate who presented with a painless penile erection persisting for more than 48 hours. Physical examination revealed no penile discoloration, rigidity, or tenderness, and laboratory evaluation showed no hematological abnormalities. Doppler ultrasonography was unavailable; however, clinical findings were consistent with non-ischemic priapism. The infant was managed conservatively with close observation, r

Performance of 5 Large Language Models in Perioperative Consultation for Pediatric Hypospadias: Cross-Sectional Comparative Study.

BACKGROUND: Hypospadias is a common congenital malformation requiring surgery. Caregivers face substantial perioperative information needs, and large language models (LLMs) offer a potential health education channel, but their performance in pediatric urology and the relation between citation accuracy and clinical content safety lack systematic evaluation. OBJECTIVE: This study aimed to evaluate 5 LLMs (ChatGPT-4o, Gemini-2.5-Pro, OpenEvidence, Zhipu Qingyan, and DeepSeek) for pediatric hypospadias perioperative consultation, and to characterize the dimensions clinicians and caregivers priori

Ultrasound-guided sacral multifidus plane block versus caudal epidural block for postoperative analgesia in pediatric hypospadias surgery: A randomized double-blind controlled trial.

INTRODUCTION: Hypospadias repair is associated with moderate to severe postoperative pain in children. Caudal epidural block is traditionally considered the standard regional anesthetic technique for sub-umbilical pediatric surgeries; however, its duration of analgesia and potential adverse effects may be limited. The sacral multifidus plane block (SMPB) is a novel ultrasound-guided fascial plane technique that may provide prolonged analgesia with fewer complications. The aim of this study is to compare the analgesic efficacy and safety of ultrasound-guided sacral multifidus plane block versus

Presentations Due to Priapism in an Urban Hospital in Switzerland.

OBJECTIVE: Priapism, a persisting erection not associated with sexual stimulation, can be ischaemic, with the risk of permanent erectile dysfunction, or nonischaemic. Drugs-e.g., injection therapies for erectile dysfunction, as well as neuroleptics, antidepressants and various other medicines-can also cause priapism. This study aimed to describe presentations due to priapism and provide insights into specific causes, clinical presentations, diagnostic strategies and emergency management. METHODS: A single-centre, retrospective, observational study of patients (≥ 16 years old) presenting to th

Collagenase nanocapsules for the treatment of Peyronie's disease. Evaluation in fibrotic plaques obtained from human patients.

INTRODUCTION: Intraplaque injection of collagenase Clostridium histolyticum (CCH) is a therapeutic approach for treating Peyronie's disease (PD) that may represent an alternative to surgery in some patients. However, the efficacy of collagenase injection is rather variable among patients. Nanocapsules (NC) provide therapeutic advantages by allowing sustained protein release and preserving enzymatic activity. The aim was to evaluate the in vitro efficacy of preparation of polymeric NC containing collagenase in reducing human PD plaque. METHODS: Polymeric nanocapsules containing collagenase typ

Idiopathic neonatal priapism presenting as persistent penile erection: A case report from a low-resource setting.

Neonatal priapism is an exceptionally rare condition, most often non-ischemic and self-limiting. We report a 13-day-old Somali neonate presenting with a painless penile erection lasting 32 hours. The infant was clinically stable, with normal laboratory findings and preserved penile perfusion. In the absence of identifiable secondary causes, a diagnosis of idiopathic non-ischemic priapism was made. Due to reassuring clinical features, conservative observation was undertaken. Spontaneous detumescence occurred without pharmacologic or surgical intervention, and follow-up showed no recurrence or c

Psychotropics in Medication-Induced Priapism: A Case Series and Narrative Literature Review.

Priapism is a urologic emergency involving a sustained and painful penile erection resulting in tissue hypoxia and ischemia. This requires emergent intervention to preserve penile function and avoid complications including fibrosis and permanent erectile dysfunction. Ischemic priapism, or low-flow priapism, accounts for most cases and can be caused by numerous factors including pharmaceuticals. Typical antipsychotics are most often the causative agent, but antidepressants and other psychotropics similarly increase risk for ischemic priapism and must be acknowledged to prevent significant morbi
