Elucidating the mechanism underlying incomplete immune recovery in people living with HIV through CD4⁺ and CD8⁺ T cell multi-omics.

BACKGROUND: A subset of people living with HIV (PLHIV), known as immunological non-responders (INR), fails to achieve adequate CD4⁺ T cell recovery despite viral suppression with antiretroviral therapy (ART). These individuals face an increased risk of adverse clinical outcomes. This study aims to explore the intracellular mechanisms and molecular signatures underlying this incomplete immune recovery using a multi-omics approach. METHODS: The study analysed CD4⁺ and CD8⁺ T cells from 100 ART-naïve PLHIV. Participants were classified into controls (baseline CD4⁺ > 200 cells/µL, n = 49) and cas

Inflammasome-associated immune dysregulation in TB-HIV coinfection.

BACKGROUND: Tuberculosis (TB) and HIV co-infection are characterized by dysregulated inflammation and increased risk of immune reconstitution inflammatory syndrome (IRIS) following antiretroviral therapy (ART) initiation. Inflammasomes are key regulators of innate immune responses, but their role in TB-HIV co-infection and IRIS remains incompletely understood. We investigated inflammasome-related gene expression and cytokine profiles in TB, HIV-1, and TB-HIV individuals, and their association with IRIS development. METHODS: Fifty-nine participants were stratified into four groups: TB-HIV (n =

Activation of cytosolic DNA sensing in NCM460 intestinal epithelial cells induces antiviral factors that restrict HIV infection in macrophages.

INTRODUCTION: The gastrointestinal tract is an important site of HIV persistence and contains susceptible target cells, including macrophages. Crosstalk between intestinal epithelial cells and macrophages may contribute to mucosal innate defense. METHODS: NCM460 human intestinal epithelial cells were transfected with poly(dA). Expression and production of interferons, interferon-stimulated genes, and CC chemokines were assessed by quantitative RT-PCR, western blotting, and ELISA. Primary human macrophages were treated with conditioned supernatants from NCM460 cells, and HIV infection was asse

Descriptive comparison of comprehensive HIV knowledge and condom non-use at last sexual intercourse among unmarried adolescent girls and young women in Nigeria.

INTRODUCTION: Across sub-Saharan Africa, and Nigeria in particular, young women and adolescent girls aged 15 to 24 continue to face a disproportionate risk of HIV - and a large part of that risk comes down to unprotected sex. Knowing about HIV is widely assumed to lead to safer behaviour, yet the evidence that knowledge actually translates into condom use is far from straightforward. This study takes a closer look at that relationship, comparing comprehensive HIV knowledge and condom non-use among unmarried adolescent girls and young women (AGYW) in Nigeria across three points in time. Current challenges in caring for injection drug users with human immunodeficiency virus infection in Pakistan.

The stigmatized status of injecting illicit drugs use and human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome in Pakistan, enlists HIV positive injection drug users (IDUs) at preeminent risk for poor mental health. This study aims to investigate the potential link between enacted HIV stigma, community attachment, and mental health problems in HIV-positive IDUs, along with mediating role of community attachment and moderating role of drug type. Correlation analysis revealed that enacted HIV stigma and community attachment were significantly negatively associated with mental he

Perceptions About the Relationship Between HIV, Antiretroviral Therapy (ART), and Fecundity in Sub-Saharan Africa: Findings from a Systematic Scoping Review.

Advances in antiretroviral therapy (ART) enable women living with HIV to safely conceive and give birth without vertical transmission. However, in sub-Saharan Africa, 1 in 6 people experience infertility and women living with HIV face even higher rates. Research shows that ART and virologic suppression improve fertility/fecundity, yet this benefit is often overlooked in ART adherence education. To map the literature on people's understandings and beliefs about the relationship between HIV, ART, and fecundity, a systematic scoping review of the English and French literature in sub-Saharan Afric

Concordance Between Fingerprick and Venous Whole Blood CD4 Counts Using the Visitect CD4 Advanced Disease Platform.

The Visitect CD4 Advanced Disease platform is a rapid, semi-quantitative assay that estimates CD4 results above or below 200 cells/μL and can be performed on venous whole blood or by fingerprick. We conducted a prospective validation of the Visitect CD4 Advanced Disease platform via fingerprick versus venous whole blood. Overall, there was 95.5% agreement (149/156). The high level of agreement between the fingerprick and venous whole blood results may support the use of the fingerprick Visitect CD4 test as a reliable point-of-care diagnostic tool in settings where access to conventional venous

Written on the Skin: Participants' Perspectives on Scarring, Discolouration and Implant Site Reactions Experienced During a Phase I Clinical Trial for a Subdermal HIV Prevention Implant in KwaZulu-Natal, South Africa.

To address challenges associated with the uptake of daily oral pre-exposure prophylaxis (PrEP) by young women at high risk of acquiring HIV, a range of novel, long-acting, slow-release products are being developed and clinically evaluated. One such technology is an annual implant of tenofovir alafenamide (TAF). This study explores the perspectives of participants from a first-in-human trial of an annual TAF implant to inform considerations for future PrEP implant innovation. The study, conducted in Durban, South Africa between 2022 and 2023, included 29 in-depth interviews (IDIs) and three foc

How place, trust, and care location influence rural and non-metropolitan patients' preferences for decentralized long-acting injectable HIV therapy: A mixed-methods study.

BACKGROUND: Long-acting injectable antiretroviral therapy (LAI-ART) offers an alternative to daily oral ART and may reduce pill fatigue, stigma, and daily treatment burden. However, its implementation in rural and non-metropolitan settings is challenged by travel burden, limited local healthcare infrastructure, and privacy concerns. This study evaluated interest in, and preferences for cabotegravir/rilpivirine (CAB + RPV LA) among people with HIV (PWH) living outside a metropolitan area and identified factors influencing uptake. METHODS: From March 2024 through December 2024 we conducted a cr

Mother-to-child transmission of HIV and risk of prematurity according to the time of art initiation.

UNLABELLED: Antiretroviral therapy (ART) is recommended once the HIV infection is diagnosed during pregnancy. However, some antiretroviral regimens have been associated with an increased risk of preterm birth. This retrospective study aimed to assess the rates of prematurity in newborns according to the timing of ART initiation in a cohort of 1,062 mother-infant pairs. Early ART initiation was not associated with an increased risk of prematurity (ART initiation before conception: 16.8%; before 14 weeks of gestation: 15.6%; ART initiation between 14 and 27 weeks: 16.3%; ART initiation after 27

The impact of male circumcision on risk of HIV infection in sub-Saharan Africa: a household fixed-effects analysis of 279,351 men from 29 countries.

INTRODUCTION: While voluntary medical male circumcision (VMMC) reduces the individual-level risk of HIV acquisition by approximately 60% in randomised-controlled trials, little is known about the 'real-world' long-term effect of medical and traditional male circumcision on the cumulative risk of HIV infection. We estimate the association between these for the first time using a quasi-experimental study design-a household fixed-effects analysis-for sub-Saharan Africa, the global region with the largest HIV burden. METHODS: We pooled individual-level cross-sectional data from the nationally-rep

Sexual Orientation and Gender Identity Data and HIV Preexposure Prophylaxis Disparity Detection.

IMPORTANCE: US federal reporting changes in 2025 removed sexual orientation and gender identity (SOGI) measures from health-center reporting, raising concerns about equity surveillance. OBJECTIVE: To quantify how incomplete or absent SOGI data are associated with detection of disparities in documented HIV preexposure prophylaxis (PrEP) orders. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used electronic health record data from a large safety-net health system in Southern California. Data were from patients 18 years or older registered as of October 28, 2025, with documented

Pharmacological and Nonpharmacological Interventions to Address Insomnia Symptoms and Poor Sleep Quality Among People With HIV: Protocol for a Scoping Review.

BACKGROUND: HIV-related research has increasingly focused on addressing long-term comorbidities affecting health-related quality of life for people with HIV. Sleep-related conditions, such as insomnia, are highly prevalent among people with HIV and are associated with a variety of health challenges, including immune dysregulation, increased mental health burden, and reduced antiretroviral therapy adherence. Although there are several evidence-based pharmacological (eg, hypnotics) and nonpharmacological interventions (eg, cognitive behavioral therapy) to treat insomnia, few have been specifical

Clinical Outcomes of Switching to Bictegravir/Emtricitabine/Tenofovir Alafenamide in People Living With HIV and Low-Level Viraemia.

IntroductionLow-level viraemia (LLV) in people living with HIV (PLWH) remains a clinical challenge, associated with increased risk of virological failure, immune dysregulation, and chronic inflammation. Bictegravir/emtricitabine/tenofovir alafenamide is a single-tablet regimen with a high genetic barrier to resistance, but real-world outcomes after switching in PLWH with LLV remain insufficiently characterized. This study aimed to describe virological, immunological, metabolic, and safety outcomes after switching to bictegravir/emtricitabine/tenofovir alafenamide in PLWH with documented LLV ov

Long-Acting HIV Therapy in African Adults with Adherence Challenges.

BACKGROUND: In eastern and southern Africa, approximately 1 million people living with human immunodeficiency virus (HIV) and receiving oral therapy do not have viral suppression because of adherence challenges. METHODS: We conducted a phase 3b, open-label, randomized trial to compare long-acting injectable cabotegravir plus rilpivirine with oral dolutegravir-based therapy among adults with virally suppressed HIV type 1 (viral load, <200 copies per milliliter) and suboptimal adherence or care engagement. The participants were assigned in a 1:1 ratio to receive intramuscular injections of cabo

Tablet-Based Neurocognitive Tests for Ugandan Adolescents Living With HIV: Cross-Sectional Validation Study.

BACKGROUND: Adolescents living with perinatally acquired HIV (PHIV) remain at risk of neurocognitive difficulties, yet access to valid neurocognitive tests is limited in sub-Saharan Africa (SSA), where most of this population resides. Tests that can be administered by nonspecialist personnel are culturally and linguistically appropriate for SSA's diverse populations and are brief enough for routine use are needed to expand access, but few such tools exist. OBJECTIVE: We adapted NeuroScreen, a brief tablet-based neurocognitive test battery, into Luganda and evaluated its validity and acceptabi

Cross-omics mapping of the human HIV restrictome across clinical states, immune-cell compartments, and protein responses.

Host restriction factors can block HIV at multiple stages, but they are often studied as individual genes or within a single molecular layer. I performed an evidence-structured computational reanalysis of three independent public datasets to determine how the human HIV restrictome varies across clinical states, immune-cell compartments, and protein responses. The analysis used bulk monocyte RNA sequencing (GSE157198), PBMC single-cell/CITE-seq data (GSE256089), and primary CD4 T-cell proteomics (PXD012263) within a prespecified restrictome-immunometabolism framework. In GSE157198, ART-naive mo

Evidence that intrinsic brain connectivity moderates the association between gait and neurocognitive function in people living with HIV.

Globally, a substantial proportion of the approximately 40 million people living with HIV (PLWH) experience some degree of HIV-associated brain injury (HABI), which is often linked to accentuated or accelerated aging in this clinical population. We set out to test the hypothesis that gait speed is associated with cognitive deficits in middle-aged PLWH in a low-middle income (LMIC) setting, and that this is comparable to what is seen in much older healthy adults. In addition, we sought to identify, as a potential marker of HABI, intrinsic functional brain connectivity abnormalities that moderat

Cost comparison of the ShangRing device versus Mogen clamp for early infant male circumcision in Sub-Saharan Africa.

INTRODUCTION: The World Health Organization (WHO) endorses male circumcision as an effective human immunodeficiency virus (HIV) prevention intervention in settings where the expected impact is greatest, including Sub-Saharan Africa. Data on cost implications of early infant male circumcision (EIMC) are limited. The ShangRing (SR) is a WHO pre-qualified device for circumcision. We sought to perform a comprehensive cost assessment alongside a randomized controlled trial (RCT) of the SR versus the standard of care Mogen clamp (MoC) for EIMC to enable future scale-up towards a cost-effective HIV p
