The baseline vaginal immune milieu is an important determinant of the inflammatory response induced by penile-vaginal sex.

Elevated cervicovaginal cytokines/chemokines and epithelial disruption are associated with HIV risk. While penile-vaginal sex induces transient cervicovaginal inflammation and epithelial disruption, there is considerable interindividual heterogeneity. We defined genital inflammation using a multi-cytokine score and hypothesized that sex-induced inflammation would depend on the pre-existing vaginal immune milieu. We quantified HIV-associated immune factors in cervicovaginal secretions of HIV-uninfected women before and 1 h after penile-vaginal sex. Cervicovaginal IgA and IgG increased after sex

Discrimination and the Mediating Role of Self-Esteem on Psychopathology and Retention in Care: A Cross-Sectional Study With People Living With HIV in Myanmar.

People living with HIV (PLHIV) in Myanmar endure a great deal of discrimination. However, self-esteem may serve as the mechanism that facilitates the impact of discrimination on poor health outcomes and behaviors in PLHIV. Our study aimed to quantitatively assess the mediating role of self-esteem in the atemporal associations between discrimination and two key health outcomes, psychopathology and retention in care, among PLHIV in Myanmar. Our cross-sectional study recruited 271 PLHIV through a quasirandom sampling method. Structural equation modeling was employed in R to examine the mediating

Severe Orofacial Manifestations in Monkeypox Infection.

A 30-year-old man with recent diagnosis of HIV-infection was seen in the hospital, because of a 1-month history of swollen lips, painful oral mucosa ulcers, cervical lymph nodes and skin lesions. The medical history showed a previous patient contact with 10 subjects, with positivity for Monkeypox virus in 8 subjects. A week later, he began with lesions on the genitals, managed with acyclovir, and subsequent development of manifestations in the mouth and skin, with dissemination to the entire body. He was admitted to our clinic for increased volume in penis and scrotum, and a disseminated derma

Improved HIV Preexposure Prophylaxis Uptake in New York City.

Preexposure prophylaxis (PrEP) for human immunodeficiency virus (HIV) prevention is a highly effective tool, but it is underutilized, and significant inequities exist regarding which populations are able to access PrEP. Although most PrEP care occurs in specialty settings, such as sexual health and HIV clinics, most individuals eligible for PrEP are seen in primary care; this imbalance represents an opportunity to expand PrEP access. New York City Health and Hospitals (NYC H+H) is a municipal health care system that provides primary and specialty care across the five boroughs of New York City.

Evaluating cost-effectiveness of 9-valent HPV vaccination for men who have sex with men by HIV status in Hong Kong.

BACKGROUND: Human papillomavirus (HPV) is the most common sexually transmitted infection and a leading cause of anal cancer and genital warts, particularly among men who have sex with men (MSM). In Hong Kong, HPV vaccination is currently only offered to school-aged girls, despite the high burden of HPV-related diseases among MSM, especially those living with HIV. Existing cost-effectiveness evaluations of HPV vaccination in Hong Kong primarily focus on female-only strategies and heterosexual men without accounting for differences in HPV infection risks among MSM. In contrast, countries such as

Cytokine concentration and T cell subsets in the female genital tract in the presence of bacterial vaginosis and Trichomonas vaginalis.

Trichomonas vaginalis (TV) and bacterial vaginosis (BV) are highly prevalent vaginal infections. Both are associated with pelvic inflammatory disease and HIV acquisition and transmission, though the underlying mechanisms are incompletely understood. We characterized the effect of TV and BV infection on inflammatory markers in the vagina among reproductive-aged women in Atlanta, Georgia. Cervicovaginal lavage specimens were collected from HIV seronegative women at a baseline visit and again three months later. Eighteen individual cytokines, 17 T cell subsets, BV, and TV were measured at both ti

Predictive risk assessment and targeted testing could enhance STI diagnosis in a high HIV prevalence setting in Eswatini.

Sexually transmitted infections (STIs) present significant public health challenges in resource-limited settings where syndromic management, which often leads to misdiagnosis and over-treatment, remains standard care. We retrospectively evaluated hypothetical STI risk assessment-only (Triage-only) and sequential risk assessment plus STI diagnostic testing (Triage&Test) approaches as potential enhancements to syndromic management in a high-burden setting. A retrospective analysis was conducted using historic data from a cross-sectional study of adults (≥ 18 years) accessing six outpatient care

Enhanced Surveillance of Sexually Transmitted Infections to Foster a Learning Public Health System.

IMPORTANCE: Sexually transmitted infections (STIs) pose a substantial public health challenge, but fragmented reporting systems have limited capacity for disease surveillance approaches that might support a learning public health system, where near real-time data-driven insights are integrated to inform, evaluate, and adapt public health strategies. OBJECTIVE: To examine how health information exchanges and open government data can be leveraged to foster a learning public health system by characterizing and comparing patterns in diagnosis vs testing at a scale commensurate with regional publi

Modeling gonorrhea and HIV coinfection with predictive analytics for disability and mortality risks.

In this article, a model is developed to depict the dynamics of gonorrhea and human immunodeficiency virus coinfection with disability and mortality risks. Estimating the parameter values and validating the model are done using real-world data on cases of gonorrhea. The properties of solutions, such as boundedness and positivity, uniqueness and existence are investigated. The basic reproduction number is calculated using the next-generation matrix. The stability of Gonorrhea-Free and Present Equilibrium states is also analyzed. The Lyapunov function is used to verify global stability at the Go

Effect of decreasing the frequency of screening for Neisseria gonorrhoeae and Chlamydia trachomatis on the incidence of these infections and antimicrobial use among men who have sex with men using HIV PrEP in Belgium: a retrospective cohort study.

BACKGROUND: There is growing evidence that the benefits of systematic screening for Neisseria gonorrhoeae and Chlamydia trachomatis among men who have sex with men (MSM) using HIV pre-exposure prophylaxis (PrEP) are scarce and that screening leads to high antimicrobial consumption. The PrEP clinic of the Institute of Tropical Medicine (ITM) in Antwerp, Belgium, discontinued systematic screening for these infections in MSM taking PrEP in 2023. In this study, we estimated the effect of reducing the frequency of screening on the incidence of these infections and antimicrobial use among MSM using
